    Objective: The aim of our research is the analysis of cellular immunity in clinically difficult cases of adult patients at the acute phase of infectious mononucleosis (IM) caused by Epstein-Barr ... virus. The main feature of the acutephase of IM is a self limiting lymphoproliferation involving both T- and B- lymphocytes accompanied by clinical symptoms such as fever, lymphadenopathyand hepatosplenomegaly. Method: We have analysed the expressionof several identification, activation and maturation cellular markers on peripheral blood lymphocytes in 5 patients with IM by flow cytometry. Results: We have determined the percentages and absolute counts of T- and B-lymphocytes, NK cells, CD4+ and CD8+ T-lymphocytes, non-MHC-restricted T-lymphocytes, activated T-lymphocytes (HLA-DR and CD38), naVve and memory CD4+ T-lymphocytes in the peripheral blood of IM patients and, matching healthy controls. The immunophenotyping profile in acute IM was identical in all patients: the percentages of T-lymphocytes and NK cells within normal ranges, decreased percentages of B-lymphocytes, significantly decreased percentages and markedly increased absolute counts of CD4+ T- lymphocytes, significant expansion of CD8+ T-lymphocytes, inverted CD4/CD8 ratios, increased expression of activation markers HLA-DR on CD3' T-lymphocytes and CD38 on CD8+ T-lymphocytes and perturbed ratio of naive and memory CD4+ T-lymphocytes in IM patients compared to healthy controls. Discussion: The results of flow cytometric analysis of peripheral blood lymphocytes in IM will be compared with standard cytological examination. The similarities between the immunophenotyping profiles of patients with acute IM and HIV-1 infection and the possible diagnostic application of flow cytometry in IM will also be discussed.
