  • Nuhalna svetlina ploda, fß-hCG in PAPP-A kot presejalni test v prvem trimesečju nosečnosti za odkrivanje nepravilnosti ploda

Background. There are several methods of antenatal screening for aneuploidy, mostly Down's syndrome (DS). In Slovenia we identify women at increased risk of DS on the basis of maternal age or by ... maternal age and fetal nuchal translucency (NT) measurement between 10 and 13 weeks of gestation. In published studies the screening test using the combination of maternal age, maternal serum free beta human chorionic gonadothropin (fbeta-hCG), pregnancy-associated plasma protein A (PAPP-A) and NT has proved to be more effective than both tests currently in use in Slovenia. However, the fmdings of published studies about the influence of maternal weight, smoking habits, parity, gravidity and fetal gender on the markers are controversial. To the knowledge of the author, no studies have been published about the differences in medians between healthy pregnant women and those with hypertension or chronic renal disease. The aim of this study was to evaluate the efficiency ofscreening using a combination of maternal age, fbeta-hCG, PAPP-A and NT for calculating the risk for DS in the first trimester of pregnancy in the women from Slovenia. Besides, we wanted to study the influence of maternal weight, smoking habits, parity, gravidity and fetal gender on the markers, and fmd possible differences between healthy pregnant women and those with hypertension or chronic renal disease. Methods. We included 1264 women with singleton pregnancy in this observational study. NT was measured between 10 and 13 6/7 weeks; at the same time fbeta-hCG and PAPP-A were measured in bloodsamples. From chromosomally normal pregnancies we calculated nonnal medians for pregnant women from Slovenia. The risks of DS were calculated using the published methods. At risk cut-off 1 in 300 the screening test was considered positive. We compared NS, fbeta-hCG and PAPP-A between healthy pregnant women and those with hypertension or chronic renal disease.
