    Backctround. Our purpose was to determine the ability of different indicationsfor cone biopsy to predict the presence of disease in the cone specimen. Methods. A retrospective review was performed of ... all patients records, who underwent cold knife cone biopsy at Gynecology Obstetrics Department in General Hospital Izola between 1990 and 2003. Data were gatheredregarding cytologic studies, cervical biopsy with endocervical curettage and histologic findings in cone specimen. The indications for the cone procedures were grouped as biopsy proved disease (indication A), discrepancy between cytologic and histologic diagnoses (indication 8), positive endocervical curettage (indication C), suspicion on cytology only (indication D). Results: Study included 682 patients undergoing cold knife cone biopsy. 555 (81,7%) cone biopsies done for indication A. 103 (15,2%) conebiopsies done for indication B. 12 (1,8%) cone biopsies done for indication C. 9 (1,3%) cone biopsies done for indication D. For 3 (0.4%) patients, none of these indications were found. The respective proportions of women with benign findings; CIN 1 or CIN II; CIN III or cancer were 1,1; 7,6; 86,5 and 4,9% for indication A, i,9; 14,6; 80,6 and 2,9% for indication B, 0; 16,7; 33,3 and 50,0` for indication C, 11,1; 11,1; 66,7 and 11,1% for indication D. Outcomes were significantly worse among women with positive endocervical curettage (p< 0,0001). Women with CIN 111 disease in cone specimen were much more likely to have biopsy proved disease preoperatively (p< 0,0001), which was also the mosf common indication. Conclusions. The likelihood of identifying CIN III or cancer in cone biopsy specimen is high, especially in biopsy proved group. The risk of cancer is highest in positive endocervical curettage group.
