  • Diagnostična torakoskopija pri malignem plevralnem mezoteliomu = Diagnostic thoracoscopy in malignant pleural mesothelioma

    Background. We examined diagnostic procedures including diagnosis thoracoscopyof patients with malignant pleural mesothelioma admitted at our hospital in the years 2000-2003. Methods. Patients' data ... were collected from computer systems Medo and Birpis and the hospital documentation. In 4 years malignant mesothelioma was confirmed in 70 patients, 46 male, 24 female with the mean age 64+l-10 years. Patients were examined with chest X-ray, computer tomography and in selected cases with the ultrasound of the thorax. We examined microscopically pleural effusion, blind needle biopsy of parietal pleura, diagnostic thoracoscopy and in some cases even surgical pleural biopsy. Tissue samples were examined with conventional and immunohistochemicalmethods. Sensitivity of cytological and histological examinations of different biopsy samples was correlated by hiz test. For the calculation of survival we applied Kaplan - Meier method using SPSS computer system. Results. 36 (51%) patients were exposed to asbestos. The most frequentsymptoms were dyspnoea 62 (89%), dull thoracic pain 53 (76%) and cough44 (63%). Chest x-ray and computer tomography of the thorax showed pleural effusion 61 (87%) and pleural thickening 42 (60%). Histological confirmation with immunophenotype was made in 68 (97%) patients. Histological types of mesothelioma were: epitheloid (n=35), biphasic (n=10), sarcomatoid (n=6), undetermined (n=19). Sensitivity of macroscopic finding during thoracoscopy was 67,5% (27/40). Sensitivity of histological examination of tissue samples of surgical pleural biopsy was 100% (6/6), thoracoscopy 92,5% (37/40), blind needle biopsy of parietal pleura 50% (13/26) and cytological examination of pleural effusion 13% (8/62).
