  • Porast srčne frekvence ob pljučnem edemu pri starostnikih : magistrska naloga

Background and the aim of the study. Clinical experience shows that symptoms and signs of acute pulmonary edema in elderly differ from the younger patients. The recognition of this life-threatening ... event is thus rendered moredifficult. Principal finding is diminution of sympathetic effects among which heart rate is one of the easiest variables to measure. To the knowledge of the author, there are no reports of this observation in the literature. Theaim of the study was to establish the relationship between the patientćs age and the change in the heart rate during pulmonary edema. We were also interested in prognostic value of the heart rate increase regarding to the duration of treatment. Methods. This is a longitudinal, prospective, cohort, observational study. In patients with pulmonary edema we established heart rate at the time of first diagnosis (ECG recording from the emergency department) and at the time of recompensation (later during hospitalization). A linear regression method was used to determine the relationship between the heart rate difference and patientćs age. Patients with shock, evidence of infection or chronic hemodialysis treatment were excluded from the study. We also defined the correlation between the heart rate increase and the duration of intensive care as well as the duration of treatment until recompensation. Results. In the group of 94 patients, aged 50 to 93 yeare, the properties of regression line between heart rate increase and age were: k = -0.5 and r = 0.21 (p < 0.05). In the subgroup of 38 patients without diabetes mellitus or atrial fibrillation k = -0.7 and r = 0.46 (p < 0.005). In the subgroup of 21 diabetic patients k = -0.97 and r = 0.47 (p < 0.05). Prognostic value of the heart rate increase regarding to the duration of treatment until recompensation was defined for the complete group of 94 patients by correlation with r = 0.34(p < 0.001).
