  • Nadomeščanje kortikosteroidov v primeru bolezenskih stanj = Corticosteroid replacement therapy in disease states

    Corticosteroids influence many metabolic pathways in the body and have an important immune and antiinflammatory function. Replacement therapy with glucocorticoids is indicated in primary and ... secondary adrenal insufficiency, but occasionally mineralocorticoid replacement is also required. The corticosteroid dosage should be increased during acute illnesses or stress. Pharmacological doses of corticosteroids used in the treatment of autoimmune inflammatory diseases cause suppression of the hypothalamo-pituitary-adrenal axis. After prolonged therapy recovery of the hypothalamo-pituitary-adrenal axis is slower and tapered doses of corticosteroids are indicated. Adrenocortical function should be tested before discontinuation of the therapy. During surgical procedures patients receiving corticosteroid replacement therapy or those recently withdrawn from pharmacological therapy need cover with stress doses according to individual protocols. The administration of steroids to patients with sepsis or septic shock is still questionable.
