  • Periferna arterijska bolezen pri bolnikih z arterijsko hipertenzijo = Peripheral artery disease in patients with arterial hypertension
    Gasparini, Mladen
The prevalence of peripheral arterial disease in people aged over 55 years is about 20% and increases with age. Most of affected individuals are asymptomatic and only a minority have claudication ... pain or critical ishemia. Patients with peripheral arterial disease alone have the same relative risk ofdeath from cardiovascular causes as those with coronary or cerebrovascular disease, and are six times more likely to die within 10 years than patients without the disease. About half of patients with peripheral arterial disease (defined by an abnormal ankle-brachilal index) have symptomatic coronary or cerebral vascular disease. The goals of treatment are to prevent future major coronaryand cerebrovascular events and to improve leg symptoms. Noninvasive therapy for intermittent claudication involves risk-factor modification, exercise, and pharmacologic therapy. There is no evidence that any antihypertensive drug could have an advantage over the others in patients withperipheral arterial disease. Beta-blockers could be safely used in mild tomoderate disease but caution is recommended in more severe disease.
