  • The use of pentoxifylline for triggering the movement of testicular sperm before their use in intracytoplasmic sperm injection in men with azoospermia

    This chapter shows the usefulness of pentoxifylline (PF)-a sperm motility stimulator-in patients with azoospennia. Sperm to be used for insemination of the partner's oocytes by intracytoplasmic sperm ... injection (ICSI) is obtained by needle aspiration or biopsy from the epidydymis or testis. Testicular sperm is poorly motile or totally immotile. Since motility is the only sign of sperm vitality, some time is required for the sperm to start to move in the culture medium. The study shows application of PF in all ICSI cases in which testiculars sperm has to be used without additional waiting for spontaneous sperm movements. The clinical results (clinical pregnancy rate of 33.1% and take-home baby rate of 24.8%) of 145 subsequent ICSI cycles with testicular sperm treated with PF are presented. The movement characteristics of fresh and thawed testicular sperm that had been exposed to PF for a short time were also evaluated and compared with the characteristics of untreated samples. The technique of treatment and manipulation of testicular sperma represented in detail. The study shows that the use of low concentrations of PF in ICSI cycles with testicular sperm is clinically safe and the oocytes or embryos are not compromised. For easier identification of vital sperm and for shortened procedures, the use of PF is recommended in ICSI with testiculars sperm.
