    Background. Cardiopulmonary bypass (CPB) is a standard procedure used in cardiac surgery. Despite the widespread use some deliterious effects are the consequence of it's use: activation of ... leucocytes, activation of complement, damaging effects on platelets and effects on complex brain functiona. Influence of minimally and less invasive cardiac surgery in the last few years with the operations on the beating heart rised the thoughts on off-pump cardiac surgery and cardiac surgery on the beating heart. Conclusions. Today cardiac surgeons are in the possibility to provide complete revascularization of myocardium on the beating heart without the use of CPB. Open heart procedures can be made on CPB with the heart beating with retrograde coronary sinus perfusion.
