  • Research on effectiveness of endosinusal treatment of chronic maxillary sinusitis
    Drviš, Petar ...
The aim of the study was to evaluate cellular marker levels of eosinophil cationic protein (ECP), myeloperoxidase (MPO) and tryptase in sinus lavage specimens in patients with chronic rhinosinusitis ... (CRS) and subjective outcomes following endosinusal steroid/antibiotic treatment. Thirty patients with symptoms of CRS were recruited for the study. The inclusion criteria weresinusitis symptoms lasting over three months and mucosal thickening of themaxillary sinus mucosa > 6 mm, i. e. maxillary sinusitis. All patients withasthma, polyposis, recent infection, systemic steroid therapy or previous sinus surgery were excluded. The patients were treated endosinusally with 2 mgof dexamethasone and 40 mg of gentamycin per maxillary sinus for 5 days. Patients rated their CRS disease-specific symptoms and completed a self-administered questionnaire concerning sinusitis symptoms at inclusion andafter 30 days. Sinus lavage specimens collected at inclusion and after endosinusal treatment were analyzed for ECP, MPO and tryptase concentrations. Significant improvement was noted for the overall sinusitis symptoms score (p < 0.01) and for obstruction, postnasal drip, headache, sneezing and cough (p <0.05) in the study group as a whole. There is a positive correlation between the cellular marker (ECP, MPO and tryptase) concentrations in sinus lavage specimens. There were no statistically significant changes in sinus lavage concentrations of these cellular markers between inclusion specimens and specimens obtained after endosinusal treatment. The results indicate that endosinusal steroid/antibiotic therapy is an effective method of treatment forpatients with chronic maxillary sinusitis.
