Background: The optimal time for treatment of Class III malocclusion is the period of deciduous or early mixed dentition. In our study, we analysed the subclasses of this disorder occurring in our ... patients referred for orthodontictreatment and assessed the suitability and timeliness of care provided to these patients. Our aim was to draw attention to the need for early detection and treatment of these anomalies. Patients and methods: Seventy-two subjects with Class III malocclusion were referred to the Department of Orthodontics in Ljubljana in the years 2004-2005. The diagnosis was made on the basis of history, clinical examination and analysis of facial photographs, study casts and teleradiographs of the head. The subclass of the anomaly was determined in each subject. The patient's age at the start of treatment and the treatment methods used were recorded. Results: A combinationof skeletal and dentoalveolar irregularities was present in 74% of patients; only dentoalveolar abnormalities were detected in 4%. A large proportion of patients (29%) had a retrognathic position of the maxilla associated with an orthognathic position of the mandible. In 54% of patients, orthodontic treatment was begun between the ages of 10 and 18 years; 14% of patients were over 19 years of age at the start of treatment and required a combination of orthodontic and surgical measures. Conclusion: Most patients with Class III malocclusion were referred for othodontic care later than appropriate. This resulted in complicated treatment programmes, increased costs and prolonged duration of care, and questionable stability of the treatment outcome.
