K2-138 is a moderately bright (V = 12.2, K = 10.3) main-sequence K star observed in Campaign 12 of the NASA K2 mission. It hosts five small (1.6-3.3 ) transiting planets in a compact architecture. ...The periods of the five planets are 2.35, 3.56, 5.40, 8.26, and 12.76 days, forming an unbroken chain of near 3:2 resonances. Although we do not detect the predicted 2-5 minute transit timing variations (TTVs) with the K2 timing precision, they may be observable by higher-cadence observations with, for example, Spitzer or CHEOPS. The planets are amenable to mass measurement by precision radial velocity measurements, and therefore K2-138 could represent a new benchmark system for comparing radial velocity and TTV masses. K2-138 is the first exoplanet discovery by citizen scientists participating in the Exoplanet Explorers project on the Zooniverse platform.

In 2017, dentists in the United States had the highest antibiotic prescribing rate per 1,000 population and Australia had the lowest rate. The penicillin class, particularly amoxicillin, was the most frequently prescribed for all countries. The second most common agents prescribed were clindamycin in the United States and British Columbia (Canada) and metronidazole in Australia and England. Broad-spectrum agents, amoxicillin-clavulanic acid, and azithromycin were the highest in Australia and the United States, respectively.
Extreme differences exist in antibiotics prescribed by dentists in Australia, England, the United States, and British Columbia. The United States had twice the antibiotic prescription rate of Australia and the most frequently prescribed antibiotic in the US was clindamycin. Significant opportunities exist for the global dental community to update their prescribing behavior relating to second-line agents for penicillin allergic patients and to contribute to international efforts addressing antibiotic resistance. Patient safety improvements will result from optimizing dental antibiotic prescribing, especially for antibiotics associated with resistance (broad-spectrum agents) or C. difficile (clindamycin). Dental antibiotic stewardship programs are urgently needed worldwide.

Prescription opioid analgesic consumption was estimated for 76 countries between 2009 and 2019 using IQVIA MIDAS data. We reported country-level consumption trends in morphine milligram equivalents (MMEs), assessed differences in consumption between high-income (HICs), upper-middle income (UMICs), and low- and lower-middle income countries (LMICs), and identified country-level socioeconomic drivers of consumption using fixed-effects panel regression models.
Global opioid consumption rate declined from 216·3 to 151·5 morphine milligram equivalents per 1,000 inhabitants per day (MID) between 2009 and 2019, with consumption declines in the US and Germany. Overall, consumption rates increased in HICs by a median 36·6 MID (IQR, -7·5 -124·5) with substantial heterogeneity between countries. Median consumption rates were lower in UMICs (23·6 MID) and LMICs (8·3 MID) compared to HICs (345·1 MID) and increased by median 10·4 and 3·7 MID from 2009-2019, respectively. Consumption rates were associated with income (coefficient 18·84, 95% confidence interval 3·8-33·9) and trade (13·59, 1·3-25·8) in UMICs, and physician density (1·95, 1·2-2·7) in LMICs. Tramadol consumption rate increased in the study period and accounted for a relatively large proportion of total opioid volume consumed across all country-income groups.
Substantial heterogeneity in global opioid consumption patterns reflect the challenges involved with providing adequate access to opioid treatment while avoiding potential misuse.

Abstract
Large modern surveys require efficient review of data in order to find transient sources such as supernovae, and to distinguish such sources from artefacts and noise. Much effort has been ...put into the development of automatic algorithms, but surveys still rely on human review of targets. This paper presents an integrated system for the identification of supernovae in data from Pan-STARRS1, combining classifications from volunteers participating in a citizen science project with those from a convolutional neural network. The unique aspect of this work is the deployment, in combination, of both human and machine classifications for near real-time discovery in an astronomical project. We show that the combination of the two methods outperforms either one used individually. This result has important implications for the future development of transient searches, especially in the era of Large Synoptic Survey Telescope and other large-throughput surveys.
Quality Function Deployment (QFD) is a process in which customer needs are operationalised into deliverable Technical Characteristics (TCs) at the design stage. A system of matrices known as the ...House of Quality (HOQ) works collectively to produce Final Weightings (FWs) for TCs, enabling prioritisation and focusing design activity. In prioritising TCs, QFD practitioners often fail to fully integrate the diverse information within the HOQ. In this article, we address the inclusion of 'Roof Matrix Correlations' (RMCs). We show that, while other heuristics have been developed to integrate RMCs, they each have limitations and only result in changes to the FW values. We present a methodology based on the Manhattan Distance Measure (MDM) that integrates RMC data into the FWs, but also measures the overall nature and level of intercorrelation within the matrix. This facilitates a more efficient selection of TCs because the MDM provides a consistent informational basis for substituting negatively correlated TCs with better alternatives, and reducing duplication of effort in cases of highly positively correlated TCs. Application of the method is illustrated through re-analysis of a well-known, published QFD example. Our approach can help practitioners to avoiding duplicating effort or to address contradictions between TCs in a timely fashion.

In this paper, we empirically test the theoretical model underlying the Toyota Way (TW), based on data obtained from Toyota's logistics, sales and marketing functions across 27 countries. TW is the ...result of Toyota attempting to codify its culture to the global community. Using structural equation modelling techniques we show that the TW-associated measures challenge, kaizen, genchi genbutsu, respect and teamwork do adequately operationally define the TW; the first three measures corresponding to the construct 'process improvement' and the final two measures corresponding to the construct 'people development'. Empirically, people development is found to have no direct effect on how the TW is deployed across a business unit. However, people development is found to be indirectly related to TW deployment through the mediating effect of process improvement. Our study provides quantitative evidence that while the intangible aspects of the TW (modelled as people development) may not directly relate to the results, they are an integral component of a complete implementation of the TW and related 'Lean' systems. By logical extension, this provides support for adoption of a holistic and long-term strategy, integrating soft and hard elements, by those organisations attempting to implement and sustain Toyota-style systems.

Context. Strong lenses are extremely useful probes of the distribution of matter on galaxy and cluster scales at cosmological distances, however, they are rare and difficult to find. The number of ...currently known lenses is on the order of 1000. Aims. The aim of this study is to use crowdsourcing to carry out a lens search targeting massive galaxies selected from over 442 square degrees of photometric data from the Hyper Suprime-Cam (HSC) survey. Methods. Based on the S16A internal data release of the HSC survey, we chose a sample of ∼300 000 galaxies with photometric redshifts in the range of 0.2 < zphot < 1.2 and photometrically inferred stellar masses of log M* > 11.2. We crowdsourced lens finding on this sample of galaxies on the Zooniverse platform as part of the Space Warps project. The sample was complemented by a large set of simulated lenses and visually selected non-lenses for training purposes. Nearly 6000 citizen volunteers participated in the experiment. In parallel, we used YATTALENS, an automated lens-finding algorithm, to look for lenses in the same sample of galaxies. Results. Based on a statistical analysis of classification data from the volunteers, we selected a sample of the most promising ∼1500 candidates, which we then visually inspected: half of them turned out to be possible (grade C) lenses or better. By including lenses found by YATTALENS or serendipitously noticed in the discussion section of the Space Warps website, we were able to find 14 definite lenses (grade A), 129 probable lenses (grade B), and 581 possible lenses. YATTALENS found half the number of lenses that were discovered via crowdsourcing. Conclusions. Crowdsourcing is able to produce samples of lens candidates with high completeness, when multiple images are clearly detected, and with higher purity compared to the currently available automated algorithms. A hybrid approach, in which the visual inspection of samples of lens candidates pre-selected by discovery algorithms or coupled to machine learning is crowdsourced, will be a viable option for lens finding in the 2020s, with forthcoming wide-area surveys such as LSST, Euclid, and WFIRST.

There are many challenges involved in online participatory humanitarian response. We evaluate the Planetary Response Network (PRN), a collaboration between researchers, humanitarian organizations, ...and the online citizen science platform Zooniverse. The PRN uses satellite and aerial image analysis to provide stakeholders with high-level situational awareness during and after humanitarian crises. During past deployments, thousands of online volunteers have compared pre- and post-event satellite images to identify damage to infrastructure and buildings, access blockages, and signs of people in distress. In addition to collectively producing aggregated "heat maps" of features that are shared with responders and decision makers, individual volunteers may also flag novel features directly using integrated community discussion software. The online infrastructure facilitates worldwide participation even for geographically focused disasters; this widespread public participation means that high-value information can be delivered rapidly and uniformly even for large-scale crises. We discuss lessons learned from deployments, place the PRN's distributed online approach in the context of more localized efforts, and identify future needs for the PRN and similar online crisis-mapping projects. The successes of the PRN demonstrate that effective online crisis mapping is possible on a generalized citizen science platform such as the Zooniverse. Purpose
For any improvement tool to be successfully integrated into an organizations’ quality improvement or risk management programme, it needs to be relatively easy-to-use and proven to provide ...benefits to the customer and organization. Many healthcare organizations are facing fiscal constraints and increasing complexity of tests, putting strains on resources, particularly for those on “the shop floor” who are “hands on” in the design, delivery and improvement of products or services. Within a laboratory setting, there is often limited time for formal extensive process reviews; with the pressure to meet “turn-around times” for often “clinically urgent” results. Preventative and corrective actions are often identified through audits or root-cause analysis in some cases after an event has occurred. The paper aims to discuss these issues.
Design/methodology/approach
Failure modes effect analysis (FMEA) is a risk management tool, used to identify prospective failures within processes or products, before they occur. Within laboratory healthcare, risk management for prevention of failure (particularly an inaccurate result) is imperative, and underpins the design of all steps of sample handling. FMEA was used to review a laboratory process for a “gene mutation test” initially considered to have few opportunities for improvement. Despite this perception, a previous review of the process, and the time restrictions for review, new improvements were identified with implications to patient management.
Findings
This study shows that FMEA can yield benefits, for prospective risk management and general process improvement, within a laboratory setting where time and team input is restricted, and within a process that was considered to have few “problems”.
Originality/value
The study was undertaken in a large metropolitan public health system laboratory – one of the largest in the country. This laboratory is a significant contributor to the health outcomes of patients in the local region, and through its contribution to national laboratory testing and reporting. This was the first use of FMEA in this laboratory setting.
