Aims and Objectives: The aim of the study is to detect the pattern of genetic mutation, i.e., Inh A or Kat G or both (Inh A and kat G) in isoniazid (INH) monoresistant mycobacteria and to correlate ...with the pattern in multidrug-resistant (MDR) isolates. Materials and Methods: In this study, a quantitative research approach was used. The research design was descriptive observational study. The study was conducted at the Department of Respiratory Medicine, JLN Medical College, Ajmer, Rajasthan, and Intermediate Referral Laboratory, State TB Demonstration Centre, Ajmer. A total of 298 samples found to have resistant strains of Mycobacterium tuberculosis were enrolled with purposive sampling. Results: The mean age of patients was 40.27 ± 13.82 years. There were 250 (83.9%) males, while 48 (16.1%) were females. One hundred ninety-two (64.4%) were resistant for INH only, while the rest were resistant to both INH as well as rifampicin (MDR-tuberculosis). The most common mutation in INH monoresistance was kat G (125; 65.1%) as compared to inh A (54; 28.1%) and both inh A and kat G (13; 6.7%). Among kat G mutations, the most common gene pattern was the absence of WT (S315T) and the presence of MUT1 (S315T1). Conclusion: Knowledge about mutation patterns of different INH resistant strains is important in the present era where there is a provision of separate regimens for INH monoresistant TB. Since these mutations are very closely related to high- or low-degree resistance to INH, the therapeutic regimens cannot be generalized.

Introduction. The intensive care unit (ICU) plays a pivotal role in providing specialized care to patients with severe illnesses or injuries. As a critical aspect of healthcare, ICU admissions demand ...immediate attention and skilled care from healthcare professionals. However, the intricacies involved in this process necessitate analytical solutions to ensure effective management and optimal patient outcomes.
Aim. The aim of this review was to highlight the enhancement of the ICUs through the application of analytics, artificial intelligence, and machine learning.
Methods. The review approach was carried out through databases such as MEDLINE, Embase, Web of Science, Scopus, Taylor & Francis, Sage, ProQuest, Science Direct, CINAHL, and Google Scholar. These databases were chosen due to their potential to offer pertinent and comprehensive coverage of the topic while reducing the likelihood of overlooking certain publications. The studies for this review involved the period from 2016 to 2023.
Results. Artificial intelligence and machine learning have been instrumental in benchmarking and identifying effective practices to enhance ICU care. These advanced technologies have demonstrated significant improvements in various aspects.
Conclusions. Artificial intelligence, machine learning, and data analysis techniques significantly improved critical care, patient outcomes, and healthcare delivery.
Introduction: Acute exacerbation of COPD is one of the leading causes of death worldwide. Aim: This study was aimed to study the role of clinical tools and biochemical markers which have significant ...impact on short-term mortality in COPD patients presenting with acute exacerbation. Material and Methods: A total of 50 patients were studied. Patients of age >40 years having COPD exacerbation and presenting with two of the three cardinal features of exacerbation: increase in amount of cough, increase in purulence of cough, increase in baseline dyspnoea which were severe enough to necessitate a hospital admission, were evaluated after admission. A thorough history taking and clinical examination was performed. Along with this, all necessary laboratory parameters were studied. Study population was followed for 30 days. Data were analysed using Chi-square test and multivariate regression analysis using SPSS version 16.0. Results: Out of 50 patients, 30% (15/50) did not survive despite treatment. Parameters such as, history of mechanical ventilation in past one year, low GCS (Glasgow Coma Scale), raised JVP (Juglar Venous Pressure), low pH and raised IL-6 levels were significantly associated with mortality in multivariate analysis (p-value <0.05). Conclusion: Severity of disease as well as severity of present exacerbation was related with short-term mortality. Interleukin-6 is an independent predictor of mortality. Parameters like past history of mechanical ventilation and raised JVP point towards the need of overall better management of COPD patients not only during exacerbations.

Introduction: Neck imaging has always been a diagnostic challenge. High-resolution sonography being inexpensive and safe has become the initial investigation of choice for neck pathologies; it ...provides information regarding the origin and extent of the lesion. Computed tomography (CT) has a unique capacity for displaying soft tissue, bone and airway details and provides critical anatomical information about lesions involving the neck. This study aims to determine the role of USG and CT in neck masses for pre-operative characterization based on location, extent, morphological characteristics and enhancement pattern and to compare USG and CT findings with histopathological diagnosis.
Material and Methods: A total number of 60 adult patients with suspected neck masses were studied in the department of Radiodiagnosis, SSIMS&RC, Davanagere and inputs from Dr. B. R. Ambedkar Medical College, Bengaluru over a period of 24 months. Ultrasound was done using GE Voluson E6 machine & Contrast enhanced CT was done using GE Revolution Multidetector CT scan machine. Post processing techniques (like volume rendered techniques) were applied wherever useful.
Results: The findings on USG and CT were conclusive in nearly 90% of cases. Enhancement patterns and presence/absence of bony invasion were found to be more specific in differentiating benign/malignant nature.
Conclusion: High-resolution sonography can be used as the first-line modality for evaluating cervical soft tissue masses, it helps the clinicians to characterize the lesion. Multi-detector computed tomography is a sensitive diagnostic tool for diagnosing neck masses and differentiating between benign and malignant lesions with high accuracy. The extent of pathology with local/ contiguous spread predicted by CT examination was conclusive. The overall findings suggest that MDCT and Ultrasound, when used in conjunction with pathological correlation, can be effective tools for accurately characterizing and diagnosing the neck masses.
