There are not many real-world studies evaluating daily insulin doses requirements (DIDR) in patients with type 1 diabetes (T1D) using second-generation basal insulin analogs, and such comparison is ...necessary. The aim of this study was to compare DIDR in individuals with T1D using glargine 300 UI/mL (IGlar-300) or degludec (IDeg) in real clinical practice. An observational, retrospective study was designed in 412 patients with T1D (males: 52%; median age 37.0±13.4 years, diabetes duration: 18.7±12.3 years) using IDeg and IGla-300 ≥6 months to compare DIDR between groups. Patients using IGla-300 (n=187) were more frequently males (59% vs 45.8%; p=0.004) and had lower glycosylated hemoglobin (HbA1c) (7.6±1.2 vs 8.1%±1.5%; p<0.001) than patients using IDeg (n=225). Total (0.77±0.36 unit/kg/day), basal (0.43±0.20 unit/kg/day) and prandial (0.33±0.23 unit/kg/day) DIDR were similar in IGla-300 and IDeg groups. Patients with HbA1c ≤7% (n=113) used significantly lower basal (p=0.045) and total (p=0.024) DIDR, but not prandial insulin (p=0.241), than patients with HbA1c between 7.1% and 8% and >8%. Patients using IGla-300 and IDeg used similar basal, prandial and total DIDR regardless of metabolic control subgroup. No difference in basal, prandial and total DIDR was observed between patients with T1D using IGla-300 or IDeg during at least 6 months in routine clinical practice.
Workplace incivility is a growing problem in nursing. However, most studies that explore this psychosocial risk and its consequences do so considering a single level and a between‐person perspective. ...The aims of the study were to explore whether the effects of experiencing incivility during work‐time could explain the daily levels of well‐being of nurses at home; and to analyze if that relationship could be moderated by their levels of emotional dysregulation as a trait. This is a multilevel study with diary methodology. The study was carried out in 18 primary health‐care centers belonging to Madrid and the Basque Country, in Spain. Ninety‐four nurses completed a general questionnaire and 54 of them a diary booklet over five consecutive working days in two different moments, immediately after work and at bedtime. The results showed that nurses’ emotional dysregulation moderated the relationship between daily workplace incivility and daily fatigue, and positive affect at night at home. However, there were no direct effects of daily incivility on these outcome variables. In conclusion, the presence of difficulties in emotional regulation among nurses can increase the negative effects of daily workplace incivility on their health and well‐being.

Hydroterra was selected in September 2018 as one of the three candidates for the ESA's Earth Explorer-10 mission. Hydroterra aims at observing and improving the understanding of fundamental processes ...of daily water cycle, enhancing prediction capability of extreme rainfall and related flooding. To achieve this goal, the mission concept envisages placing a satellite embarking a synthetic aperture radar instrument in geosynchronous orbit. The satellite is designed to be launched with VEGA-C into low Earth orbit and to be transferred to the operational orbit by means of electric propulsion. In this paper, the multidisciplinary optimization process carried out to obtain the baseline electric orbit raising transfer for Hydroterra will be presented. The computed trajectory will meet the mission requirements, including targeting to the operational orbit, the launcher performance model, and the target on-station mass. Additionally, system-level constraints related to the satellite design that have an influence on the ideally optimized trajectory will be derived and considered in the optimization. In particular, the impact of the solar cell degradation on the thrust and specific impulse levels of the thruster due to the passage through the Van-Allen belts, the need to re-charge the batteries before entering the next eclipse, and the agility requirements will be accounted for. The obtained results show that a feasible electric orbit raising trajectory can be successfully obtained for the Hydroterra mission.

Fibromyalgia is a chronic condition characterized by musculoskeletal pain. The aim of this study was to synthesize scientific evidence on the effects of aquatic exercise programs on pain and quality ...of life in individuals with fibromyalgia. This review was carried out using the following databases in January 2024: Cochrane Library, PEDro, PubMed, SCOPUS, and Web of Science. Four clinical trials focusing on aquatic exercise as a treatment for patients with fibromyalgia were selected. These trials were published in English between 2019 and 2024. Pain recorded using the Visual Analog Scale (VAS) and quality of life with the Short Form-36 Health Survey (SF-36) or Fibromyalgia Impact on Quality of Life (FIQ) were the most commonly analyzed variables. This review was carried out according to the PRISMA statement and was registered in PROSPERO (CRD42024510219). The results in terms of pain and quality of life were positive. In conclusion, these findings support the incorporation of aquatic exercise into fibromyalgia physical therapy treatment. However, the benefits could be equivalent to those of other exercise modalities, underscoring the need for individualized adaptation to each patient's needs.
A single congenital muscular torticollis (CMT) is a postural musculoskeletal deformity and is characterized by the shortening or stiffness of the sternocleidomastoid muscle. The reported incidence of ...CMT ranges from 0.2% to 2%. The objective is to evaluate the effect of physical therapy programs on CMT. For the search, PubMed, Scopus, Web of Science, PEDro and Cochrane databases were used. Randomized controlled trials published between 2018 and 2023 have been included. This study follows the PRISMA 2020 statement and has been registered in the PROSPERO database. Finally, six studies were included. The cervical range of motion (ROM) in rotation was the most analyzed variable, followed by the ultrasound evaluation; one of the studies included the analysis of children's motor development with the Alberta scale. All research found benefits associated with soft tissue mobilization, passive stretching techniques and manual therapy of the cervical spine. In conclusion, it is possible to recommend manual therapy and passive stretching techniques for the treatment of CMT, with significant results on the cervical ROM.
Background
Possible COVID-19 pneumonia patients (ppCOVID-19) generally overwhelmed emergency departments (EDs) during the first COVID-19 wave. Home-confinement and primary-care phone follow-up was ...the first-level regional policy for preventing EDs to collapse. But when X-rays were needed, the traditional outpatient workflow at the radiology department was inefficient and potential interpersonal infections were of concern. We aimed to assess the efficiency of a primary-care high-resolution radiology service (pcHRRS) for ppCOVID-19 in terms of time at hospital and decision’s reliability.
Methods
We assessed 849 consecutive ppCOVID-19 patients, 418 through the pcHRRS (home-confined ppCOVID-19 with negative—group 1- and positive—group 2-X-rays) and 431 arriving with respiratory symptoms to the ED by themselves (group 3). The pcHRRS provided X-rays and oximetry in an only-one-patient agenda. Radiologists made next-step decisions (group 1: pneumonia negative, home-confinement follow-up; group 2: pneumonia positive, ED assessment) according to X-ray results. tests to analyse changes in the ED workload, time-to-decision differences between groups, potential delays in patients acceding through the ED, and pcHRRS performance for deciding admission.
Results

Conclusions
Our pcHRRS may be a more efficient entry-door for ppCOVID-19 by decreasing ED patients and making expedited decisions while guaranteeing social distance.

Anisakiasis, a zoonotic disease that can lead to small intestine obstruction, has seen a significant rise in Spain. The country has become the first in Europe with an annual incidence of 8000 cases, ...primarily due to the popularity of consuming exotic dishes of undercooked or raw fish and the impact of climate change. The clinical presentation of anisakiasis can mimic symptoms of acute appendicitis or intestinal obstruction, leading to potential misdiagnosis. This case report describes a 37-year-old patient with no significant medical history who presented abdominal distension and intense pain in the right lower quadrant. The patient underwent surgery and received physiotherapy treatment, including therapeutic exercises and pulsed-pressure myofascial vacuum therapy, to facilitate functional recovery. The increasing incidence of anisakiasis in Spain underscores the need to consider it in the differential diagnosis of digestive diseases, given the high consumption of poorly prepared or raw fish in the region.

) farming, the by-products have been identified as a source of collagen that could be used for the development of dermocosmetics or pharmaceutical products. However, the ...characteristics of collagen related to a specific strain or culture must be well defined prior to its application. Collagen was extracted from the skin of three strains of tilapia: red YY males (YY: two Y-type sex chromosomes), XX gray females, and the F1: offspring of crossing red YY males with XX gray females; at different ages in the adult phase, using acetic acid and pepsin enzyme. The characteristics of acid-soluble collagen (ASC) and pepsin-soluble collagen (PSC) were shown by SDS-PAGE band profiles to be similar to bovine collagen type I (SIGMA), the PSC of gray tilapia being more fragile to temperature changes, consistent with the results of fractional viscosity. The characteristics of the F1 progeny were prioritized for being a commercially productive and sustainable source for the extraction of collagen, and the ASC form, being the one with the greatest stability and advantage over PSC, of importance to our investigations, leads to a controlled digestion as in the case of peptide induction, and also in the development of natural products in the pharmaceutical and/or dermocosmetic industry. Evaluations of the triple helix structure by FT-IR, X-ray diffraction and UV-visible spectroscopy give similar results between the strains: red, gray, and F1, and between ages in the adult form F1 (15, 24, and 36 months of age). Consequently, the skin of tilapia in adult form is recommended sustainably for up to 24 months of age where the collagen is obtained with the use of acetic acid without enzymatic treatment.

Las universidades deben tener un instrumento que permita medir el perfil emprendedor del futuro profesional, de manera que pueda acceder rápidamente al entorno laboral. El estudio evaluó la escala de ...perfil emprendedor en universitarios peruanos mediante el análisis psicométrico de validez y confiabilidad. Se adaptó un cuestionario validado previamente en otro contexto y se aplicó a 354 estudiantes de una universidad pública de Trujillo (Perú) con edades de 19 a 23 años. Se determinó la validez mediante la evaluación del contenido y constructo; mientras que, para la confiabilidad, se utilizó el Alfa de Cronbach. Se encontró un V Aiken de 0,94; la prueba de Kaiser-Meyer-Olkin y la prueba de esfericidad de Bartlett arrojó un valor mayor a 0,5; comunalidades con valores mínimo y máximo de 0,586 y 0,881; con la identificación de 9 dimensiones, analizados por descomposición de la varianza total (73,942%) y componentes rotados; en el análisis de correlación se encontró diferencia significativa en la dimensión – test, ítem – dimensión e ítem – test. En la confiabilidad se encontró un valor de Alfa de Cronbach de 0,833. Se concluye que, la escala de perfil emprendedor es válido y confiable para el contexto peruano.
Introduction. Tetralogy of Fallot is a congenital heart disease and the most common cyanotic heart defect in children. It is clinically characterized by a ventricular septal defect, pulmonary ...stenosis, overriding aorta over ventricular septal defect and right ventricular hypertrophy. There is little or no information about the oral manifestations in patients with this pathology. A report and discussion of a pediatric patient diagnosed with Tetralogy of Fallot, its clinical manifestations, oral findings and dental management are presented. Case Report. A four-year-old male patient diagnosed with Tetralogy of Fallot and epileptic attacks. The patient has deciduous teeth with many severe early childhood caries, stomatitis and cyanotic mucous, radicular remains, periapical abscess and noticeable enamel hypoplasia. Conclusions. In agreement with other authors, children with systemic diseases such as Tetralogy of Fallot have a higher rate of caries, poor oral hygiene, high susceptibility to other infections and bacterial endocarditis, cyanotic mucous membranes and enamel hypoplasia. Primary prevention is critical, proper dental hygiene, regular dental check-ups and the use of antibiotic prophylaxis are particularly important, especially in high-risk patients.
Introducción. La tetralogía de Fallot es la patología cardíaca congénita y defecto cardiaco cianótico más común en niños. Clínicamente se caracteriza por una comunicación interventricular, estenosis pulmonar, cabalgamiento de la aorta sobre la comunicación interventricular e hipertrofia del ventrículo derecho. Existe poca o nula información sobre las manifestaciones orales de pacientes con esta patología. Se relata el informe y discusión de un paciente pediátrico diagnosticado con tetralogía de Fallot, sus manifestaciones clínicas, hallazgos orales y manejo dental. Refiriendo además la condición de salud oral del paciente antes y después de los procedimientos dentales. Reporte del Caso. Paciente masculino de 4 años, diagnosticado con Tetralogía de Fallot y crisis epilépticas. Presenta dentición temporal con múltiples caries de la infancia temprana severa, estomatitis y mucosas cianóticas, restos radiculares, absceso periapical y una evidente hipoplasia del esmalte. Conclusiones. En concordancia con otros autores, los niños con enfermedades sistémicas como tetralogía de Fallot tienen un mayor índice de caries, higiene oral deficiente, alta susceptibilidad a otras infecciones y a endocarditis bacteriana, mucosas cianóticas e hipoplasia del esmalte. La prevención primaria es decisiva, higiene dental apropiada, revisiones odontológicas habituales y uso de antibioprofilaxis son significativos sobre todo en pacientes de alto riego.
