Background
Patients with HIV/AIDS on antiretroviral therapy (ART) live longer and now require surgery for indications similar to those described for the general population. They have been previously ...reported to carry higher mortality and complication rates, especially septic complications. The aim of this study was to compare the outcome of major abdominal surgery in three groups of patients with different CD4 counts.
Methods
This is a prospective study comparing HIV-negative patients and two groups of HIV-infected patients on ART with different CD4 counts. The primary outcomes considered were mortality and complication rates after abdominal surgery. We emphasised on the value of CD4 as a predictor of outcome and the impact of the indication for surgery (septic versus non-septic).
Results
We included 63 patients (21 per group). The majority of patients (71 %) were operated on as an emergency and the indications were similar in all groups. The overall and the septic complication rates were both higher in the group with a low CD4 count. This resulted in a significantly longer admission period but did not result in a higher mortality rate. The duration of ART and the World Health Organisation stage of the disease did not significantly influence surgical outcomes.
Conclusions
HIV-infected patients on ART can now safely undergo major abdominal surgery with encouraging results though still relatively poorer than those of HIV-negative subjects. CD4 count remains a significant predictor of outcome and patients with a low CD4 count, however, still require closer pre- and post-operative monitoring.

Objectif Notre objectif était de rechercher les cas d’hypertension artérielle résistante dans une population de diabétiques camerounais et d’évaluer l’effet de l’adjonction de spironolactone à faible ...dose. Matériels et méthodes Nous avons examine consécutivement 672 patients diabétiques suivis en consultation externe à l’Hôpital Central de Yaoundé, dont 292 (56 %) présentaient une hypertension artérielle (HTA) et 63 une HTA résistante confirmée. Parmi eux 24 avaient moins de 75 ans, résidaient à Yaounde et avaient une clairance calculée de la créatinine > 60 ml/min et étaient éligibles. Dix sept ont consenti à participer à un essai clinque randomisé contrôlé en aveugle simple. L’intervention comportait un bras de rajout de spironolactone 25 mg/jour et un bras contrôle d’escalade thérapeutique avec addition de molécule autre que la spironolactone (Candésartan 8 mg/j ; Aténolol 50 mg/j ; Alpha-méthyldopa 750 mg/j). La durée de l’intervention était de 4 semaines avec automesure tensionnelle 6 fois par jour, 2 jours par semaine et un suivi clinique et biologique hebdomadaire principalement ionogramme sanguin et fonction rénale. Résultats La spironolactone en 4e ou 5e ligne de traitement vs escalade thérapeutique alternative, entrainait une réduction significative de la pression artérielle systolique de − 33 vs − 14 mmHg (p < 0,001) et diastolique moyenne de − 14 vs − 5 mmHg (p < 0,001) en automesure, et une variation de 158 ± 17/ 86 ± 11 mmHg à 125 ± 11/ 72 ± 8 mmHg, p = 0,006 dans le groupe spironolactone vs 158 ± 8/ 94 ± 8 mmHg à 144 ± 17/ 89 ± 12 mmHg, NS en mesure clinique. Les variations de natrémie, kaliémie et creatininemie sur 4 semaines n’étaient pas significatives dans les deux groupes. Conclusion Ces résultats montrent une prévalence élevée de l’HTA résistante chez le diabétique dans la population étudiée et suggèrent l’efficacité de la spironolactone à faible dose à court terme sur le contrôle tensionnel.

Introduction Le poumon, du fait de sa richesse en capillaires sanguins et en tissu conjonctif est une cible potentielle des complications microvasculaires du diabète. Le but de notre étude était ...d’évaluer la fonction pulmonaire des patients diabétiques et de rechercher les déterminants d’une altération de la fonction pulmonaire au cours du diabète. Patients et méthodes Nous avons mené une étude transversale au Centre National d’Obésité de l’Hôpital Central de Yaoundé, Cameroun. D’avril à juillet 2012, nous avons recruté 76 individus exempts de pathologies respiratoires et cardiovasculaires dont 35 patients diabétiques de type 2 (56 ± 8 ans), 25 de type 1 (24 ± 7 ans) et 16 témoins (25,7 ± 6,8 ans) appariés pour l’âge, le sexe et l’IMC à 16 des sujets diabétiques de type 1 (25,2 ± 7,2 ans). Le dosage de l’HbA1c, l’examen du fond d’œil et la mesure des paramètres spirométriques ont été réalisés chez tous les participants. Résultats Les sujets diabétiques de types 1 et 2 avaient une diminution du volume expiré maximal en une seconde (VEMS), de la capacité vitale forcée (CVF) et du débit expiratoire de pointe comparés aux valeurs de références camerounaises (P < 0,05). Dans le sous groupe cas-témoin, le VEMS (p = 0,003), la CVF (p = 0,001) et le DEP (p = 0,003) des sujets diabétiques de type 1 étaient diminués comparés à ceux des témoins. Nous avons observé une corrélation négative entre la CVF et l’HbA1c (r = − 0,266 ; p = 0,050). Les paramètres spirométriques n’étaient pas influencés par la durée moyenne du diabète de 5 ± 5 ans et par la présence des complications microvasculaires du diabète. Conclusion Le VEMS, la CVF et le DEP sont diminués chez les sujets diabétiques. La microangiopathie et la durée du diabète ne semblent pas influencer ces paramètres spirométriques. Cependant, la CVF est corrélé à l’HbA1c.

We evaluated the performance of the Modification of Diet in Renal Disease (MDRD), Chronic Kidney Disease Epidemiology Collaboration (CKD‐EPI) and Cockcroft–Gault (CG) equations against creatinine ...clearance (CrCl) to estimate glomerular filtration rate (GFR) in 51 patients with Type 2 diabetes.
Methods
The CrCl value was obtained from the average of two consecutive 24‐h urine samples. Results were adjusted for body surface area using the Dubois formula. Serum creatinine was measured using the kinetic Jaffe method and was calibrated to standardized levels. Bland–Altman analysis and kappa statistic were used to examine agreement between measured and estimated GFR.
Results
Estimates of GFR from the CrCl, MDRD, CKD‐EPI and CG equations were similar (overall P = 0.298), and MDRD (r = 0.58; 95% CI: 0.36–0.74), CKD‐EPI (r = 0.55; 95% CI: 0.33–0.72) and CG (r = 0.61; 95% CI: 0.39–0.75) showed modest correlation with CrCl (all P < 0.001). Bias was −0.3 for MDRD, 1.7 for CKD‐EPI and −5.4 for CG. All three equations showed fair‐to‐moderate agreement with CrCl (kappa: 0.38–0.51). The c‐statistic for all three equations ranged between 0.75 and 0.77 with no significant difference (P = 0.639 for c‐statistic comparison).
Conclusions
The MDRD equation seems to have a modest advantage over CKD‐EPI and CG in estimating GFR and detecting impaired renal function in sub‐Saharan African patients with Type 2 diabetes. The overall relatively modest correlation with CrCl, however, suggests the need for context‐specific estimators of GFR or context adaptation of existing estimators.

Glomerular filtration rate (GFR) equations for estimating kidney function are routinely used in sub‐Saharan Africa. There is, however, a paucity of data on the performance of these equations in patients with diabetes mellitus who are, for the most part, undiagnosed and unaware of their condition.

Background. Heart rate variability (HRV) analysis is a useful method for assessing the heart’s ability to adapt to endogenous and exogenous loads. Data from African population on HRV are scarce and ...even more so in sports populations. This study aimed to compare cardiac autonomic modulation response in Cameroonian athletes and sedentary. Methodology. We conducted a prospective and analytical study in sports teams in the city of Yaoundé, Cameroon. The participants in our study were divided in three groups; people who practiced little or no sporting activity (sedentary as group 1) or who were regularly physically active as part of a sports team (footballers or handballers as second and third groups). They had to be aged 18 or over and have given their informed consent. Heart rate (HR) was continuously recorded at rest for ten minutes and then transferred to a computer equipped with Kubios HRV Standard software for analysis. Means ± mean standard errors were compared using the one-way ANOVA test, followed by Tukey’s post-test. The significance threshold was set at 0.05. Results. Of the 60 people selected to participate to our study, 75.0% were sportsmen (40.0% handball players and 35.0% footballers). The resting HR of sedentary people was higher (p<0.001) than that of footballers and handball players. The SDNN, RMSSD, and pNN50 of sedentary people (16.22 ± 1.04; 9.97 ± 0.46; and 0.16 ± 0.06) were lower than those of footballers (30.13 ± 2.93; 20.61 ± 2.46; and 2.99 ± 0.63, with p<0.001) and handball players (29.00 ± 1.86; 16.44 ± 1.16; and 2.15 ± 0.38, with p<0.001 and p<0.05 respectively). Absolute and relative very-low-frequency (VLF) power, absolute low and high-frequency (LF and HF) power, as well as total power (TP) were lower in sedentary people (3.66 ± 0.08 and 16.21 ± 0.64; 5.04 ± 0.15 and 2.50 ± 0.16 and 246.40 ± 18.04) compared to footballers (5.09 ± 0.24 and 26.87 ± 1.76; 5.85 ± 0.32 and 3.92 ± 0.22 and 836.10 ± 103.70, with p<0.001, p<0.01, and p<0.001) and handball players (4.86 ± 0.16 and 30.82 ± 2.67; 6.03 ± 0.19 and 3.46 ± 0.16 and 927.30 ± 94.12, with p<0.001, p<0.05, p<0.01, and p<0.001). The LF/HF ratio was 12.1% and 20.1% lower in sedentary people (7.55 ± 0.58) compared with footballers (8.46 ± 0.50) and handball players (9.07 ± 0.60), respectively. Conclusion. Sportsmen showed greater parasympathetic and global modulation when compared to sedentary people.

Introduction La neuropathie autonome cardiaque (NAC) est associée à un risque de mortalité élevé, cependant les dépistages de la NAC sont rares en afrique. Notre objectif était de déterminer la ...prévalence de la NAC, ses facteurs associés Matériels et méthodes 95 patients diabétiques de type 2 camerounais consécutifs suivis à l’Hôpital Central de Yaoundé (57 femmes et 38 hommes). Tous ont eu un examen clinique, HbA1c, profil lipidique, la protéinurie, ECG de repos, les manœuvres de diagnostic de la NAC (la mesure de la variabilite de la fréquence cardiaque sur 10 minutes, l’arythmie sinusale respiratoire ASR, la manœuvre de Valsalva, et l’épreuve d’orthostatisme EO) faits. Résultats L’âge moyen était de 56 ± 9 ans, et à la découverte du diabète 51 ± 9 ans, l’indice de masse corporel 28,9 ± 6,7 kg/m2 , la PAS 138 ± 24 mmHg, la PAD 82 ± 12 mmHg, avec 65,3% d’HTA traités. HbA1c était de 7,9 ± 1,9% ; 31,6% avaient une protéinurie. L’ASR était anormal chez 37,8%, la fréquence cardiaque maximale en inspiration de 77 ± 13btm/min et minimale en expiration 65 ± 13btm/min. Le quotient de Valsalva était de 0,81 ± 0,11 ; anormal chez tous. L’EO était anormale chez 70,5%, la moyenne des fréquences cardiaques du 30e intervalle RR 81 ± 15btm/min, du 15e intervalle RR 84 ± 15btm/min. Globalement, 16,1% des patients avaient un test anormal (NAC suspectée), 60,2% avaient 2 des 3 tests perturbés (NAC confirmée), 23,7% avaient les 3 épreuves perturbées (NAC sévère). Il n’existait aucune différence significative de pression artérielle, des complications chroniques du diabète en fonction des stades de NAC. Cependant 86% des sujets ayant un taux de cholestérol LDL > 150 mg avaient une NAC confirmée ou sévère, 13% une NAC suspectée. Conclusion En conclusion, tous les patients étudiés étaient au minimum suspects de NAC, suggérant que le diagnostic du diabète est tardif dans notre population, ou que les valeurs seuils utilisées dans le dépistage de la NAC pourraient être différente dans notre population.

Introduction le diagnostic du diabète est basé sur le dosage du glucose plasmatique, mais dans les milieux à ressource limitée, le sang capillaire est utilisé comme alternatif. Le facteur de ...conversion de 11 % a été suggéré, basé sur des études impliquant une étendue restreinte de concentration circulant du glucose. Il est de savoir s’il s’applique à des étendus plus large. Patients et Méthodes Pour vérifier l’équivalence entre les concentrations mesurées dans le plasma, sang total capillaire et veineux, nous avons développé un protocole comprenant une injection d’insuline suivi d’une perfusion graduée de solution de glucose variant la glycémie de 0,5 à 4,0 g/l. Chez 12 volontaires adultes sains, nous avons collecté simultanément à des cibles prédéfinies (0,80-1,20 g/l, 1,80-2,20 g/l, 2,80-3,20 g/dl, 3,80-4,20 g/l). Nous avons aussi mesuré simultanément, chez 32 patients diabétiques se présentant spontanément avec nos cibles glycémiques prédéfinies, la concentration de glucose sur sang total capillaire, veineux et plasma. Pour les mesures sur sang total veineux et capillaire, nous avons utilisé l’Hemocue 201 et une méthode de glucose oxydase pour le plasma. Résultats Nous n’avons pas trouvé de différence statistiquement significative entre les concentrations de glucose sur sang total capillaire et plasma veineux à tous les niveaux des cibles prédéfinies (p > 0,144). Il n y avait pas de différence statistiquement significative en comparant les concentrations de glucose sur sang total veineux et plasma (p > 0,661). Les concentrations de glucose sur plasma et sang total étaient fortement corrélé (rs = 0,953, p Conclusion Nos résultats ont montré une bonne corrélation mais mauvaise concordance entre les concentrations de glucose sur plasma et sang total. Le diagnostic du diabète avec des échantillons non-plasmatiques devrait être très prudent.

Introduction Les patients diabétiques de type 2 en insuffisance rénale terminale présentent des altérations de la sensibilité et de la clairance de l’insuline modifiées transitoirement par ...l’hémodialyse. Les ajustements des traitements hypoglycémiants pré et post hémodialyse sont de ce fait difficiles et il n’existe pas de recommandations de pratique clinique. Cette étude visait à déterminer la variation des besoins en insuline pré et post hémodialyse (HD). Patients et Méthodes Nous avons développé un clamp euglycémique sur 24 h incluant 3 repas et 2 collations d’apports quotidiens standardisés ajustes au poids et au sexe et contenant 55 % d’hydrates de carbone, permettant de déterminer les besoins en insuline par un ajustement des débits de perfusion d’insuline toutes les 30 min pour le maintien de la glycémie capillaire entre 5,0 et 6,0mmol/L. Les apports alimentaires de chaque participant étaient identiques les jours pré et post HD respectivement. Dix patients (6H, 4F) ayant un diabète de type 2 connu depuis 12 ± 5 (SD) ans, un IMC de 22,7 ± 4,5 kg/m2 , et une maladie rénale chronique stade 5, sous 2-3 séances/semaine d’hémodialyse depuis 2,3 ± 2,3 ans, ont été soumis a ce protocole 24 h pré-HD, per-HD et 24 h post-HD. Les concentrations circulantes d’urée et de créatinine et la composition corporelle ont été mesurées avant et après la séance d’hémodialyse. Résultats La glycémie capillaire moyenne obtenue était de 5,5 ± 0,3mmol/L pré-HD, et 5,3 ± 0,2mmol/L post-HD avec aire sous courbe glycemique identique (p = 0,39). Les doses d’insuline nécessaire pour l’euglycémie étaient de 23,6 ± 7,7U/24 h pré-HD et 19,9 ± 4,9U/24 h post-HD, soit une baisse post HD de 15,3 % (p = 0,09). Les besoins en infusion basale d’insuline ont diminué de 0,4 ± 0,1U/h pré-HD a 0,3 ± 0,1U/h post-HD, soit -25,0 % (p = 0,01). La variation des besoins en bolus prandiaux n’était pas significative (-2,2 ± 3,1U/24 h, p = 0,15). Les variations des concentrations circulantes d’urée sanguine n’étaient pas significativement corrélées aux variations des besoins en insuline (r = 0,1, p = 0,79). Conclusion Ces résultats suggèrent qu’à apports alimentaires équivalents, les besoins en insuline exogene basale des patients diabétiques de type 2 hémodialysés chroniques pourraient être réduits de 25 %, sans modification des bolus prandiaux au cours des 24 heures suivant une séance d’hémodialyse.

OBJECTIVE:High Blood Pressure (HBP) is a worldwide public health problem. It can be particularly severe in the Black race. Recent studies in Cameroon, showed an alarming prevalence (29.7%), leading ...us to want to study the epidemiological, clinical, therapeutic and evaluative aspects of severe (BP up to 180/110 mm Hg), recently diagnosed HBP in Yaoundé. Our objective was to determine the clinical presentations, evolution on treatment and observance.
DESIGN AND METHOD:We conducted nine months prospective cohort study, from January to September 2016 in Yaounde teaching Hospital. We recruited from members of the public who accepted voluntary blood pressure screening offered in various localities in Yaounde, and were aged 18 years or over.
RESULTS:Of a total of 6,519 people who participated in the screening, 1,875 (28.8%) presented a HBP and 363 (5.6%) had severe HBP. Our cohort comprised 153 (42.1%) of these individuals with sustained severe hypertension, not on medication, who accepted the invitation to participate in the study. The range 45–54 years and 55–64 years were the most represented; the sex ratio was 0.9. The cardiovascular risk factors number range from 5 to 8 with a median of 6. Systolic BP ranged from 184 to 225 mmHg with a median of 200 mmHg; while the diastolic BP ranged between 111–132.5 mmHg with a median of 119 mmHg. Kidney injury (78.8%) was the main complication. We identified 3 clinical formshypertensive emergencies 121 (79.1%) cases and hypertensive crises 32 (20.9%) cases. In these two groups, 33 (21.6%) patients presented with ‘super HBP’ (a blood pressure op to 250/150 mmHg). The average rate of BP control over 6 months was 39%. The main cause of poor BP control was lack of therapeutic compliance. We registered one death at the 3rd month of follow up due to acute kidney injury.
CONCLUSIONS:Severe high blood pressure is frequent in Yaounde and the main complication is renal feature. The lack of therapeutic observance remain the major problem of BP control.

Severe HBP prevalence in Yaounde is high in the active fraction of the population insidiously affected. Particularly, unsuspected renal impairment appears to be the major complication. The bad blood pressure control is linked to poor therapeutic observance and persistence.
