Oceans cover over two-thirds of the Earth's surface, and the particles emitted to the atmosphere by waves breaking on sea surfaces provide an important contribution to the planetary albedo. During ...the International Chemistry Experiment in the Arctic LOwer Troposphere (ICEALOT) cruise on the R/V Knorr in March and April of 2008, organic mass accounted for 15-47% of the submicron particle mass in the air masses sampled over the North Atlantic and Arctic Oceans. A majority of this organic component (0.1 - 0.4 μ m⁻³) consisted of organic hydroxyl (including polyol and other alcohol) groups characteristic of saccharides, similar to biogenic carbohydrates found in seawater. The large fraction of organic hydroxyl groups measured during ICEALOT in submicron atmospheric aerosol exceeded those measured in most previous campaigns but were similar to particles in marine air masses in the open ocean (Southeast Pacific Ocean) and coastal sites at northern Alaska (Barrow) and northeastern North America (Appledore Island and Chebogue Point). The ocean-derived organic hydroxyl mass concentration during ICEALOT correlated strongly to submicron Na concentration and wind speed. The observed submicron particle ratios of marine organic mass to Na were enriched by factors of ~10²-~10³ over reported sea surface organic to Na ratios, suggesting that the surface-controlled process of film bursting is influenced by the dissolved organic components present in the sea surface microlayer. Both marine organic components and Na increased with increasing number mean diameter of the accumulation mode, suggesting a possible link between organic components in the ocean surface and aerosol-cloud interactions.

was typically within ∼40% and, in the unpolluted regions, within 30%. The analysis of the model results suggests that the MCM might underestimate the concentrations of some acyl peroxy radicals and other small peroxy radicals. The model underestimated the measurements of NO
3
by 60–70%, possibly because of rapid heterogeneous uptake of N
2
O
5
. The MCM model results were used to estimate the composition of the peroxy radical pool and to quantify the role of DMS, isoprene and alkenes in the formation of RO
2
in the different regions. The measurements of HO
2
+ RO
2
and NO
3
were also used to calculate the gas-phase budget of NO
3
and quantify the importance of organic peroxy radicals as NO
3
sinks. RO
2
accounted, on average, for 12–28% of the total gas-phase NO
3
losses in the unpolluted regions and for 1–2% of the total gas-phase NO
3
losses in the polluted regions.

Aerosol chemical, physical, and optical measurements were made aboard the NOAA R/V Ronald H. Brown off the coast of New England from July 12 through August 10, 2002, as part of the New England Air ...Quality Study (NEAQS). Measurements (generally 20 to 100 km from the coast) were made downwind of urban centers (New York City, Boston) and rural areas, and in air masses that had not been in contact with land for several days. On average during NEAQS, 75 ± 8% of the sub‐10 μm aerodynamic diameter dry aerosol mass sampled 18 m above the sea surface was in the sub‐1 μm fraction (size cut at 55% RH). The major submicrometer aerosol components were ammonium sulfate and particulate organic matter (POM, defined here as 1.6 times the mass concentration of organic carbon) comprising more than 92 ± 4% of the total mass. Under northwesterly flow with an average submicrometer total mass concentrations of 11 ± 4.5 μg m−3, POM was the dominant component (68 ± 8%) followed by (NH4)xHySO4 (23 ± 8%), inorganic oxidized material (IOM) (6 ± 4%), and EC (3 ± 1%). Under southwesterly flow with an average submicrometer total mass concentrations of 30 ± 11 μg m−3, (NH4)xHySO4 was the dominant component (54 ± 9%) followed by POM (41 ± 9%), IOM (3 ± 2%), and EC (2 ± 1%). Mie calculations using submicrometer nonrefractory (NR) POM and NR (NH4)xHySO4 + H2O size distributions to calculate submicrometer light scattering (σsp) at a wavelength of 550 nm suggest that POM was a dominant chemical component contributing to aerosol light scattering (haze) during NEAQS 2002, and contributed 60 ± 6 % and 57 ± 11 % to σsp at 55% RH during two pollution episodes off the New England Coast. These results are similar to those from the mid‐Atlantic states during TARFOX but contrary to the long‐term monitoring measurements over the continental northeast United States that show the New England haze is primarily a result of sulfate aerosol.

. Here we markedly expand the structural coverage of the proteome by applying the state-of-the-art machine learning method, AlphaFold
, at a scale that covers almost the entire human proteome (98.5% of human proteins). The resulting dataset covers 58% of residues with a confident prediction, of which a subset (36% of all residues) have very high confidence. We introduce several metrics developed by building on the AlphaFold model and use them to interpret the dataset, identifying strong multi-domain predictions as well as regions that are likely to be disordered. Finally, we provide some case studies to illustrate how high-quality predictions could be used to generate biological hypotheses. We are making our predictions freely available to the community and anticipate that routine large-scale and high-accuracy structure prediction will become an important tool that will allow new questions to be addressed from a structural perspective.

Venous thromboembolism is recognized as a leading cause of maternal death in the United States. Thromboprophylaxis has been highlighted as a key preventive measure to reduce venous ...thromboembolism-related maternal deaths. However, the expanded use of thromboprophylaxis in obstetrics will have a major impact on the use and timing of neuraxial analgesia and anesthesia for women undergoing vaginal or cesarean delivery and other obstetric surgeries. Experts from the Society of Obstetric Anesthesia and Perinatology, the American Society of Regional Anesthesia, and hematology have collaborated to develop this comprehensive, pregnancy-specific consensus statement on neuraxial procedures in obstetric patients receiving thromboprophylaxis or higher dose anticoagulants. To date, none of the existing anesthesia societies' recommendations have weighed the potential risks of neuraxial procedures in the presence of thromboprophylaxis, with the competing risks of general anesthesia with a potentially difficult airway, or maternal or fetal harm from avoidance or delayed neuraxial anesthesia. Furthermore, existing guidelines have not integrated the pharmacokinetics and pharmacodynamics of anticoagulants in the obstetric population. The goal of this consensus statement is to provide a practical guide of how to appropriately identify, prepare, and manage pregnant women receiving thromboprophylaxis or higher dose anticoagulants during the ante-, intra-, and postpartum periods. The tactics to facilitate multidisciplinary communication, evidence-based pharmacokinetic and spinal epidural hematoma data, and Decision Aids should help inform risk-benefit discussions with patients and facilitate shared decision making.

We describe the operation and improvement of AlphaFold, the system that was entered by the team AlphaFold2 to the “human” category in the 14th Critical Assessment of Protein Structure Prediction ...(CASP14). The AlphaFold system entered in CASP14 is entirely different to the one entered in CASP13. It used a novel end‐to‐end deep neural network trained to produce protein structures from amino acid sequence, multiple sequence alignments, and homologous proteins. In the assessors' ranking by summed z scores (>2.0), AlphaFold scored 244.0 compared to 90.8 by the next best group. The predictions made by AlphaFold had a median domain GDT_TS of 92.4; this is the first time that this level of average accuracy has been achieved during CASP, especially on the more difficult Free Modeling targets, and represents a significant improvement in the state of the art in protein structure prediction. We reported how AlphaFold was run as a human team during CASP14 and improved such that it now achieves an equivalent level of performance without intervention, opening the door to highly accurate large‐scale structure prediction.

Abstract Objective To assess the state of health information technology (HIT) adoption and use in seven industrialized nations. Design We used a combination of literature review, as well as ...interviews with experts in individual nations, to determine use of key information technologies. Main outcome measures We examined rates of electronic health record (EHR) use in ambulatory care and hospital settings, along with current activities in health information exchange (HIE) in seven countries: the United States (U.S.), Canada, United Kingdom (UK), Germany, Netherlands, Australia, and New Zealand (NZ). Results Four nations (the UK, Netherlands, Australia, and NZ) had nearly universal use of EHRs among general practitioners (each >90%) and Germany was far along (40–80%). The U.S. and Canada had a minority of ambulatory care physicians who used EHRs consistently (10–30%). While there are no high quality data for the hospital setting from any of the nations we examined, evidence suggests that only a small fraction of hospitals (<10%) in any single country had the key components of an EHR. HIE efforts were a high priority in all seven nations but the early efforts have had varying degrees of active clinical data exchange. Conclusion We examined HIT adoption in seven industrialized nations and found that many have achieved high levels of ambulatory EHR adoption but lagged with respect to inpatient EHR and HIE. These data suggest that increased efforts will be needed if interoperable EHRs are soon to become ubiquitous in these seven nations.

Objective
To conduct a systematic review and meta‐analysis of artificial urinary sphincter (AUS) placement after radical prostatectomy (RP) and external beam radiotherapy (EBRT).
Patients and Methods
...There were 1 886 patients available for analysis of surgical revision outcomes and 949 for persistent urinary incontinence (UI) outcomes from 15 and 11 studies, respectively. The mean age (sd) was 66.9 (1.4) years and the number of patients per study was 126.6 (41.7). The mean (sd, range) follow‐up was 36.7 (3.9, 18–68) months. A systematic database search was conducted using keywords, according to Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA) guidelines. Published series of AUS implantations were retrieved, according to the inclusion criteria. The Newcastle–Ottawa Score was used to ascertain the quality of evidence for each study. Surgical results from each case series were extracted. Data were analysed using CMA® statistical software.
Results
AUS revision was higher in RP + EBRT vs RP alone, with a random effects risk ratio of 1.56 (95% confidence interval CI 1.02–2.72; P < 0.050; I2 = 82.0%) and a risk difference of 16.0% (95% CI 2.05–36.01; P < 0.080). Infection/erosion contributed to the majority of surgical revision risk compared with urethral atrophy (P = 0.020). Persistent UI after implantation was greater in patients treated with EBRT (P < 0.001).
Conclusions
Men receiving RP + EBRT appear at increased risk of infection/erosion and urethral atrophy, resulting in a greater risk of surgical revision compared with RP alone. Persistent UI is more common with RP + EBRT.
