Abstract
The groundbreaking image of the black hole at the center of the M87 galaxy has raised questions at the intersection of observational astronomy and black hole physics. How well can the radius ...of a black hole shadow be measured, and can this measurement be used to distinguish general relativity from other theories of gravity? We explore these questions using a simple spherical flow model in general relativity, scalar Gauss–Bonnet gravity, and the Rezzolla and Zhidenko parameterized metric. We assume an optically thin plasma with power-law emissivity in radius. Along the way we present a generalized Bondi flow, as well as a piecewise analytic model for the brightness profile of a cold inflow. We use the second moment of a synthetic image as a proxy for EHT observables and compute the ratio of the second moment to the radius of the black hole shadow. We show that corrections to this ratio from modifications to general relativity are subdominant compared to corrections to the critical impact parameter, and we argue that this is generally true. In our simplified model the astrophysical parameter uncertainty dominates the gravity theory parameter uncertainty, underlining the importance of understanding the accretion model if EHT is to be used to successfully test theories of gravity.

We develop a financial-economic model for carbon pricing with an explicit representation of decision making under risk and uncertainty that is consistent with the Intergovernmental Panel on Climate ...Change’s sixth assessment report. We show that risk associated with high damages in the long term leads to stringent mitigation of carbon dioxide emissions in the near term, and find that this approach provides economic support for stringent warming targets across a variety of specifications. Our results provide insight into how a systematic incorporation of climate-related risk influences optimal emissions abatement pathways.

The two cluster components have a pair-wise velocity of \(\lessapprox\)100 kms\(^{-1}\), favoring an orientation in the plane of the sky with a transverse velocity of 100-1700 kms\(^{-1}\). At the same time, the brightest cluster galaxy is offset in velocity from the systemic mean value. New LOFAR and VLA radio maps uncover the BCG and a large red galaxy in the northeastern side to be head-tail galaxies, suggesting that this component has already traversed southwestern side and is now exiting the cluster to the northeast.
Sepsis and septic shock remain drivers for mortality in critically ill patients. The heterogeneity of the syndrome hinders the generation of reproducible numbers on mortality risks. Consequently, ...mortality rates range from 15 to 56%. We aimed to update and extend the existing knowledge from meta-analyses and estimate 30- and 90-day mortality rates for sepsis and septic shock separately, stratify rates by region and study type and assess mortality rates across different sequential organ failure assessment (SOFA) scores.
We performed a systematic review of articles published in PubMed or in the Cochrane Database, between 2009 and 2019 in English language including interventional and observational studies. A meta-analysis of pooled 28/30- and 90-day mortality rated separately for sepsis and septic shock was done using a random-effects model. Time trends were assessed via Joinpoint methodology and for the assessment of mortality rate over different SOFA scores, and linear regression was applied.
Four thousand five hundred records were identified. After title/abstract screening, 783 articles were assessed in full text for eligibility. Of those, 170 studies were included. Average 30-day septic shock mortality was 34.7% (95% CI 32.6-36.9%), and 90-day septic shock mortality was 38.5% (95% CI 35.4-41.5%). Average 30-day sepsis mortality was 24.4% (95% CI 21.5-27.2%), and 90-day sepsis mortality was 32.2% (95% CI 27.0-37.5%). Estimated mortality rates from RCTs were below prospective and retrospective cohort studies. Rates varied between regions, with 30-day septic shock mortality being 33.7% (95% CI 31.5-35.9) in North America, 32.5% (95% CI 31.7-33.3) in Europe and 26.4% (95% CI 18.1-34.6) in Australia. A statistically significant decrease of 30-day septic shock mortality rate was found between 2009 and 2011, but not after 2011. Per 1-point increase of the average SOFA score, average mortality increased by 1.8-3.3%.
Trends of lower sepsis and continuous septic shock mortality rates over time and regional disparities indicate a remaining unmet need for improving sepsis management. Further research is needed to investigate how trends in the burden of disease influence mortality rates in sepsis and septic shock at 30- and 90-day mortality over time.
Comet C/2016 R2 (PanSTARRS) has a peculiar volatile composition, with CO being the dominant volatile, as opposed to H2O, and one of the largest N2/CO ratios ever observed in a comet. Using ...observations obtained with the Spitzer Space Telescope, NASA's Infrared Telescope Facility, the 3.5 m Astrophysical Research Consortium telescope at Apache Point Observatory, the Discovery Channel Telescope at Lowell Observatory, and the Arizona Radio Observatory 10 m Submillimeter Telescope, we quantified the abundances of 12 different species in the coma of R2 PanSTARRS: CO, CO2, H2O, CH4, C2H6, HCN, CH3OH, H2CO, OCS, C2H2, NH3, and N2. We confirm the high abundances of CO and N2 and heavy depletions of H2O, HCN, CH3OH, and H2CO compared to CO reported by previous studies. We provide the first measurements (or most sensitive measurements/constraints) on H2O, CO2, CH4, C2H6, OCS, C2H2, and NH3, all of which are depleted relative to CO by at least 1-2 orders of magnitude compared to values commonly observed in comets. The observed species also show strong enhancements relative to H2O, and, even when compared to other species like CH4 or CH3OH, most species show deviations from typical comets by at least a factor of 2-3. The only mixing ratios found to be close to typical are CH3OH/CO2 and CH3OH/CH4. The CO2/CO ratio is within a factor of 2 of those observed for C/1995 O1 (Hale-Bopp) and C/2006 W3 (Christensen) at a similar heliocentric distance, though it is at least an order of magnitude lower than many other comets observed with AKARI. While R2 PanSTARRS was located at a heliocentric distance of 2.8 au at the time of our observations in 2018 January/February, we argue, using sublimation models and comparison to other comets observed at similar heliocentric distance, that this alone cannot account for the peculiar observed composition of this comet and therefore must reflect its intrinsic composition. We discuss possible implications for this clear outlier in compositional studies of comets obtained to date and encourage future dynamical and chemical modeling in order to better understand what the composition of R2 PanSTARRS tells us about the early solar system.
Understanding the profile of oncogene and tumor suppressor gene mutations with their interactions and impact on the prognosis of multiple myeloma (MM) can improve the definition of disease subsets ...and identify pathways important in disease pathobiology. Using integrated genomics of 1273 newly diagnosed patients with MM, we identified 63 driver genes, some of which are novel, including IDH1, IDH2, HUWE1, KLHL6, and PTPN11. Oncogene mutations are significantly more clonal than tumor suppressor mutations, indicating they may exert a bigger selective pressure. Patients with more driver gene abnormalities are associated with worse outcomes, as are identified mechanisms of genomic instability. Oncogenic dependencies were identified between mutations in driver genes, common regions of copy number change, and primary translocation and hyperdiploidy events. These dependencies included associations with t(4;14) and mutations in FGFR3, DIS3, and PRKD2; t(11;14) with mutations in CCND1 and IRF4; t(14;16) with mutations in MAF, BRAF, DIS3, and ATM; and hyperdiploidy with gain 11q, mutations in FAM46C, and MYC rearrangements. These associations indicate that the genomic landscape of myeloma is predetermined by the primary events upon which further dependencies are built, giving rise to a nonrandom accumulation of genetic hits. Understanding these dependencies may elucidate potential evolutionary patterns and lead to better treatment regimens.

Patients with newly diagnosed multiple myeloma (NDMM) with high-risk disease are in need of new treatment strategies to improve the outcomes. Multiple clinical, cytogenetic, or gene expression ...features have been used to identify high-risk patients, each of which has significant weaknesses. Inclusion of molecular features into risk stratification could resolve the current challenges. In a genome-wide analysis of the largest set of molecular and clinical data established to date from NDMM, as part of the Myeloma Genome Project, we have defined DNA drivers of aggressive clinical behavior. Whole-genome and exome data from 1273 NDMM patients identified genetic factors that contribute significantly to progression free survival (PFS) and overall survival (OS) (cumulative R
= 18.4% and 25.2%, respectively). Integrating DNA drivers and clinical data into a Cox model using 784 patients with ISS, age, PFS, OS, and genomic data, the model has a cumlative R
of 34.3% for PFS and 46.5% for OS. A high-risk subgroup was defined by recursive partitioning using either a) bi-allelic TP53 inactivation or b) amplification (≥4 copies) of CKS1B (1q21) on the background of International Staging System III, comprising 6.1% of the population (median PFS = 15.4 months; OS = 20.7 months) that was validated in an independent dataset. Double-Hit patients have a dire prognosis despite modern therapies and should be considered for novel therapeutic approaches.
