It is increasingly recognized that soil microbes have the ability to decompose old recalcitrant soil organic matter (SOM) by using fresh carbon as a source of energy, a phenomena called priming ...effect (PE). However, efforts to determine the consequences of this PE for soil carbon and nitrogen dynamics are in their early stage. Moreover, little is known about the microbial populations involved. Here we explore the consequences of PE for SOM dynamics and mineral nitrogen availability in a soil incubation experiment (161 days), combining the supply of dual-labeled (
13C and
14C) cellulose and mineral nutrients. The microbial groups involved in PE were investigated using molecular fingerprinting techniques (FAMEs and B- and F-ARISA). We show that mean residence time of SOM pool controlled by the PE decreased from 3130 years in the subsoil, where the availability of fresh carbon is very low, to 17–39 years in the surface layer. This result suggests that the decomposition of this recalcitrant soil C pool is strictly dependent on the presence of fresh C and is not an energetically viable mean of accessing C for soil microbes. We also suggest that fungi are the predominant actors of cellulose decomposition and induced PE and they adjust their degradation activity to nutrient availability. The predominant role of fungi can be explained by their ability to grow as mycelium which allows them to explore soil space and mine large reserve of SOM. Finally, our results support the existence of a bank mechanism that regulates nutrient and carbon sequestration in soil: PE is low when nutrient availability is high, allowing sequestration of nutrients and carbon; in contrast, microbes release nutrients from SOM when nutrient availability is low. This bank mechanism may help to synchronize the availability of soluble nutrients to plant requirement and contribute to long-term SOM accumulation in ecosystems.

The world's soils store more carbon than is present in biomass and in the atmosphere. Little is known, however, about the factors controlling the stability of soil organic carbon stocks and the ...response of the soil carbon pool to climate change remains uncertain. We investigated the stability of carbon in deep soil layers in one soil profile by combining physical and chemical characterization of organic carbon, soil incubations and radiocarbon dating. Here we show that the supply of fresh plant-derived carbon to the subsoil (0.6-0.8 m depth) stimulated the microbial mineralization of 2,567 226-year-old carbon. Our results support the previously suggested idea7 that in the absence of fresh organic carbon, an essential source of energy for soil microbes, the stability of organic carbon in deep soil layers is maintained. We propose that a lack of supply of fresh carbon may prevent the decomposition of the organic carbon pool in deep soil layers in response to future changes in temperature. Any change in land use and agricultural practice that increases the distribution of fresh carbon along the soil profile could however stimulate the loss of ancient buried carbon.

Son ...diagnostic peut être suspecté cliniquement devant une formation kystique, associée à une dent incluse, mais la confirmation repose sur l'examen histopathologie. Il s'agit de deux patients précédemment de 13 et de 37 ans, sans antécédents, qui consultaient suite à une tuméfaction siégeant respectivement au niveau du maxillaire et au niveau de la mandibule. L'examen anatomo-pathologique de ces lésions avait conclu à une tumeur adénomatoïde odontogène. A travers ces deux observations, nous rapportons les caractéristiques anatomo-cliniques, l'évolution et le traitement de ce type de tumeur.

Les anti-TNFα sont très efficaces dans le traitement des maladies inflammatoires chroniques rhumatismales et intestinales mais exposent à un risque élevé d'infections opportunistes, en particulier la ...tuberculose. Ce risque est multiplié par 2 à 10 chez les patients sous anti-TNFα par rapport à la population générale et par 2 à 4 par rapport aux malades sans anti-TNFα. Il s'agit le plus souvent de tuberculoses extrapulmonaires et de formes disséminées. Ces tuberculoses surviennent au cours de la première année de traitement et correspondent dans la majorité des cas à une réactivation de tuberculose latente ; d'où la mise en place de recommandations de dépistage et de prévention. Outre l'anamnèse, l'examen physique et la radiographie du thorax, la recherche d'une tuberculose latente repose aussi sur l'intradermoréaction à la tuberculine (IDR). Mais celle-ci présente certaines limites surtout chez ces malades immunodéprimés (faux négatifs) ou vaccinés par le BCG (faux positifs). Les tests de relargage de l'interféron-γ : IGRA (Interferon-Gamma Release Assays) semblent être plus sensibles et plus spécifiques et devraient être utilisés dans cette indication. La prophylaxie repose sur l'isoniaside seul pendant 9 mois ou l'association isoniazide-rifampicine pendant 3 mois). Elle doit être débutée au moins 3 semaines avant la première cure d'anti-TNFα. Ces stratégies se sont révélées efficaces en diminuant nettement le nombre de tuberculoses sous anti-TNFα en les ramenant aux taux avant anti-TNFα.

Although gastresophageal reflux disease (GERD) is highly prevalent in Western countries, we have very little data about it in African countries. The aim of the study is to determine the ...prevalence and severity of GERD symptoms among Tunisian subjects and report its characteristics, consultation rate, management modes, as well as patients' satisfaction. Five hundred subjects living in Tunisia were interviewed face to face. The study was conducted at seven centers of primary care at Monastir's department by six interviewer doctors. The questionnaire consisted of 30 questions relating to subject attributes, lifestyle factors, medical history, reflux‐related symptom characteristics, consultation behavior, previous treatments for GERD, and description of the last episode. Symptoms were defined as ‘frequent’ if they occurred at least weekly and ‘occasional’ if they occurred less frequently during the last year. The mean age was 42.3 ± 17.3 years and 75.6% were females. Over the previous year, 60% of the respondents reported suffering any GERD symptom. The prevalence of frequent GERD is 24%. Female gender (odds ratio OR: 1.971.15–3.37) and body mass index ≥ 25 (OR: 1.541.042–2.29) were associated with increased risk of GERD symptom. Only 22.3%, sought medical advice about GERD symptoms in the last year. In the univariate and multivariate analysis, work status, frequency and intensity of symptoms, duration of symptom, and association of atypical symptoms were associated with a higher frequency of medical consultation for GERD symptoms. Among the subjects complaining about heartburn, 34% took medications. GERD symptoms are common among Tunisian subjects. Few heartburn sufferers seek medical attention, and most do not take medications for symptomatic control.

Introduction Le diabète est une pathologie fréquemment associée à la cirrhose virale C, le profil évolutif de chacune peut entraver l’autre. Le but de notre travail est de déterminer les ...caractéristiques épidémiologiques, évolutives et thérapeutiques du diabète chez les cirrhotiques virales C. Patients et méthodes Étude rétrospective sur 12 ans ayant inclus tous les cas d’association de diabète à une cirrhose virale C. Résultats Quatre-vingt-quatre cas de cirrhose virale C, 26 % des malades sont diabétiques ( n = 22). La prévalence de diabète en cas de cirrhose virale C est nettement supérieure qu’ en cas de cirrhose virale B (11 % dans notre expérience). Le sex-ratio est de 0,83. L’âge moyen de nos patients est de 63 ans avec des extrêmes entre 47 à 77 ans. Le diabète était découvert avant la cirrhose dans 100 % des cas. Les antécédents familiaux de diabète n’est retrouvé que chez 2 cas. L’obésité est constaté chez 10 patients (45 %). le traitement de diabète est les antidiabétiques oraux dans 8 cas et insuline dans 14 cas, 4 cas d’emblée et 10 de seconde intention. l’équilibre de diabète est bon dans 8 cas, moyen dans 10 cas et mauvais dans 4 cas. Les complications dégénératives étaient à type de rétinopathie ( n = 4) et métabolique type coma hyperosmolaire ( n = 4). Conclusion Il existe une forte prévalence du diabète lors de la cirrhose virale C. Le diabète devrait être systématiquement recherché lors de diagnostic ou lors de suivi de l’hépatite C.
