SARS‐CoV‐2 exploits many strategies to subvert innate immune responses allowing the virus to replicate and disseminate within the host. The extent to which the virus replicates within the host, and ...the efficacy of the host innate immune response to eradicate the infection and trigger effective adaptive immune responses, but not hyper‐responses of innate immunity strongly determines the disease outcome. Understanding the innate immune factors that exacerbate vascular complications following infection will be crucial to control severe disease.
Summary
Innate immune sensing of viral molecular patterns is essential for development of antiviral responses. Like many viruses, SARS‐CoV‐2 has evolved strategies to circumvent innate immune detection, including low cytosine–phosphate–guanosine (CpG) levels in the genome, glycosylation to shield essential elements including the receptor‐binding domain, RNA shielding and generation of viral proteins that actively impede anti‐viral interferon responses. Together these strategies allow widespread infection and increased viral load. Despite the efforts of immune subversion, SARS‐CoV‐2 infection activates innate immune pathways inducing a robust type I/III interferon response, production of proinflammatory cytokines and recruitment of neutrophils and myeloid cells. This may induce hyperinflammation or, alternatively, effectively recruit adaptive immune responses that help clear the infection and prevent reinfection. The dysregulation of the renin–angiotensin system due to down‐regulation of angiotensin‐converting enzyme 2, the receptor for SARS‐CoV‐2, together with the activation of type I/III interferon response, and inflammasome response converge to promote free radical production and oxidative stress. This exacerbates tissue damage in the respiratory system, but also leads to widespread activation of coagulation pathways leading to thrombosis. Here, we review the current knowledge of the role of the innate immune response following SARS‐CoV‐2 infection, much of which is based on the knowledge from SARS‐CoV and other coronaviruses. Understanding how the virus subverts the initial immune response and how an aberrant innate immune response contributes to the respiratory and vascular damage in COVID‐19 may help to explain factors that contribute to the variety of clinical manifestations and outcome of SARS‐CoV‐2 infection.

Introduction Le test au monofilament 10 g (MF) est recommandé pour le dépistage des pieds à risque, et par extension souvent utilisé pour le diagnostic rapide de la neuropathie sensitive diabétique, ...mais il est soumis à la subjectivité du patient. Son résultat est il associé à des perturbations objectives des explorations fonctionnelles neurologiques ? Patients et Méthodes Trente-six patients diabétiques ont bénéficié d'un test au MF puis d'une mesure de la sensibilité vibratoire par Neuroesthésiométrie (NES), de la vitesse de conduction nerveuse (VCN) et de la l'amplitude de potentiel (PA) par DPN-check appliqué sur le nerf sural, et de la conductance électrochimique de la sueur par Sudoscan. Nous avons comparé leurs résultats en fonction de la perception ou non du MF, par ANOVA. Résultats Les 36 patients (18 hommes, 18 diabètes type 1 et 18 type 2) étaient âgés de 57 ± 13 ans, durée de diabète 20 ± 16 ans, HbA1c 8,8 ± 2,1 %. Douze patients (33 %) ne percevaient pas le MF (MF-). Ils étaient plus âgés : MF – : 65 ± 12 ans vs vs MF + : 53 ± 14 ans ; p = 0,012) avec un diabète plus ancien (MF – : 28 ± 19 ans vs MF + : 16 ± 10 ans ; p = 0,030). Chez les patients MF –, les valeurs de seuils de sensibilité vibratoire aux hallux étaient plus élevées (Droite : MF – 30 ± 11 mV vs MF + : 9 ± 6 mV p = 0,000 ; Gauche : MF – 29 ± 12 mV ; MF + : 9 ± 6 mV p = 0,000). Leurs vitesses de conduction nerveuse et amplitudes de potentiels étaient réduites (VC N = MF – : 38 ± 8 ms vs MF + : 47 ± 8 ms ; p = 0,03 et PA = MF – : 3,1 ± 1,3 uV vs MF + : 7,1 ± 5, 3uV ; p = 0,019). Leur conductance électrochimique de la sueur au niveau des pieds était plus basse (MF – : 47 ± 29uS vs MF + : 77 ± 8uS ; p = 0,000), avec même une tendance au niveau des mains (MF – : 56 ± 23uS vs MF + : 67 ± 13uS ; p = 0,10). Conclusions Les patients diabétiques qui ne perçoivent pas le monofilament ont des anomalies objectives : vitesses de conduction nerveuse réduites de − 20 %, amplitudes de potentiel réduites de − 57 %, conductance électrochimique sudorale réduite de – 39 %.

Introduction Certaines recommandations internationales exigent la mesure de la vitesse de conduction nerveuse(VCN) pour établir le diagnostic de neuropathie diabétique. Nous la mesurons par ...DPN-Check, au lit du patient. La VCN est elle influencée par sa glycémie ? Patients et Méthodes 23 sujets diabétiques hospitalisés ont eu des mesures répétées par DPN Check, en hyperglycémie (à l'admission) puis en normoglycémie (trois jours après). 10 sujets contrôles ont aussi eu des mesures répétées afin de vérifier la reproductibilité. Tous bénéficiaient de 4 mesures par DPN Check, appliqué au nerf sural : VCN et amplitude de potentiel(PA), bilatérales. Nous avons comparé nos mesures, aux deux niveaux glycémiques, par tests t pour échantillons appariés et entre les groupes par ANOVA. Résultats Chez les 10 sujets normaux(20 % d'hommes, âge 22,6 ± 1,2 an, glycémies digitales à 0,91 ± 0,17 g/L), la reproductibilité était excellente pour les VCN (ICC à 0,84 (0,40-0,96) à droite et 0,99 (0,97-1,00) à gauche) et bonne pour les PA (ICC à 0,63 (0,09-0,89) à droite et 0,75 (0,26-0,93) à gauche). Les 23 patients diabétiques(47,8 % hommes) étaient âgés de 56,7 ± 15,0 ans, durée de diabète 18 ± 16 ans et HbA1c 8,9 ± 2,7 %. Leurs mesures étaient réalisées en hyperglycémie (2,47 ± 0,41 g/L) puis en normoglycémie(1,17 ± 0,24 g/L ; p < 0,05). Les VCN et PA étaient améliorées en normoglycémie : les VCN de 42 ± 9 à 47 ± 7 m/s ( p = 0,012), les PA de 6,6 ± 5,6 à 7,7 ± 6,0 V ( p = 0,065). En hyper comme en normoglycémie, ces valeurs étaient significativement altérées par rapport à celles des contrôles non diabétiques (VC N = 58 ± 4 ; PA : 14,7 ± 4,0 ; p < 0,005) 11 des sujets diabétiques avaient une sensibilité réduite (Neuroesthésiométrie > 15V aux hallux), leurs VCN étaient significativement altérées en hyperglycémie à 37 ± 6 m/s par rapport aux 12 qui avaient une sensi-bilité vibratoire normale (47 ± 8 ; p = 0,04) et s'amélioraient en normoglycémie à 44 ± 6 (50 ± 7 si sensibilité normale ; p = 0,03). Conclusions Le DPN-Check fournit des mesures reproductibles de VCN et PA, réalisables en 10 minutes au lit du patient. Les résultats sont altérés en cas de neuropathie. Les mesures de VCN sont influencées par le niveau glycémique et s'améliorent de 6 m/s en normoglycémie.

Detection of population genetic structure of zooplankton at medium‐to‐small spatial scales in the absence of physical barriers has remained challenging and controversial. The large population sizes ...and high rates of gene flow characteristic of zooplankton have made resolution of geographical differentiation very difficult, especially when using few genetic markers and assuming equilibrium conditions. Next‐generation sequencing now allows simultaneous sampling of hundreds to thousands of genetic markers; new analytical approaches allow studies under nonequilibrium conditions and directional migration. Samples of the North Atlantic Ocean planktonic copepod, Centropages typicus, were analysed using restriction site‐associated DNA (RAD) sequencing on a PROTON platform. Although prior studies revealed no genetic differentiation of populations across the geographical range of the species, analysis of RAD tags showed significant structure across the North Atlantic Ocean. We also compared the likelihood for models of connectivity among NW Atlantic populations under various directional flow scenarios that replicate oceanographic conditions of the sampled domain. High‐density marker sampling with RAD sequencing markedly outperformed other technical and analytical approaches in detection of population genetic structure and characterization of connectivity of this high geneflow zooplankton species.

Rituximab has emerged as an efficacious option for drug-resistant myasthenia gravis (MG). However, reports published only describe the short-term follow-up of patients treated and little is known ...about their long-term clinical and immunologic evolution. Our objective was to report the clinical and immunologic long-term follow-up of 17 patients (6 MuSK+MG and 11 AChR+MG) and compare the response between AChR+MG and MuSK+MG patients.
Myasthenia Gravis Foundation America postintervention status and changes in treatment and antibody titers were periodically determined. Lymphocyte subpopulations, total immunoglobulin, immunoglobulin G (IgG) anti-MuSK subclasses, and anti-tetanus toxoid IgG before and after treatment were also studied.
After a mean post-treatment period of 31 months, 10 of the AChR+MG patients improved but 6 of them needed reinfusions. In contrast, all MuSK+MG patients achieved a remission (4/6) or minimal manifestations (2/6) status and no reinfusions were needed. Consequently, in the MuSK+MG group, prednisone doses were significantly reduced and concomitant immunosuppressants could be withdrawn. Clinical improvement was associated with a significant decrease in the antibody titers only in the 6 MuSK+MG patients. At last follow-up MuSK antibodies were negative in 3 of these patients and showed a decrease of over 80% in the other 3.
In view of the long-lasting benefit observed in MuSK+MG patients, we recommend to use rituximab as an early therapeutic option in this group of patients with MG if they do not respond to prednisone.
This study provides Class IV evidence that IV rituximab improves the clinical and immunologic status of patients with MuSK+MG.
In this report, 528 cases of monkeypox across 16 countries are described. Rash, mucosal lesions, fever, lethargy, and lymphadenopathy were common clinical findings. Few patients were hospitalized.
A monkeypox (MPX) outbreak has expanded worldwide since May 2022. We tested 147 clinical samples collected at different time points from 12 patients by real-time PCR. MPX DNA was detected in saliva ...from all cases, sometimes with high viral loads. Other samples were frequently positive: rectal swab (11/12 cases), nasopharyngeal swab (10/12 cases), semen (7/9 cases), urine (9/12 cases) and faeces (8/12 cases). These results improve knowledge on virus shedding and the possible role of bodily fluids in disease transmission.

Chez les 23 sujets diabétiques (47,8 % d’hommes de 56,7 ± 15 ans, avec une durée de diabète de 18 ± 16 ans et HBA1c 8,9 ± 2,7 %), les mesures étaient réalisées en hyperglycémie : 2,47 ± 0,41 g/L puis en normoglycémie : 1,17 ± 0,24 g/L ( p < 0,05). Objective To compare the duration of parenteral nutrition, growth, and morbidity in extremely premature infants fed exclusive diets of either bovine milk–based preterm formula (BOV) or donor human ...milk and human milk-based human milk fortifier (HUM), in a randomized trial of formula vs human milk. Study design Multicenter randomized controlled trial. The authors studied extremely preterm infants whose mothers did not provide their milk. Infants were fed either BOV or an exclusive human milk diet of pasteurized donor human milk and HUM. The major outcome was duration of parenteral nutrition. Secondary outcomes were growth, respiratory support, and necrotizing enterocolitis (NEC). Results Birth weight (983 vs 996 g) and gestational age (27.5 vs 27.7 wk), in BOV and HUM, respectively, were similar. There was a significant difference in median parenteral nutrition days: 36 vs 27, in BOV vs HUM, respectively ( P = .04). The incidence of NEC in BOV was 21% (5 cases) vs 3% in HUM (1 case), P = .08; surgical NEC was significantly higher in BOV (4 cases) than HUM (0 cases), P = .04. Conclusions In extremely preterm infants given exclusive diets of preterm formula vs human milk, there was a significantly greater duration of parenteral nutrition and higher rate of surgical NEC in infants receiving preterm formula. This trial supports the use of an exclusive human milk diet to nourish extremely preterm infants in the neonatal intensive care unit.
