There is disagreement about the impact of delayed graft function (DGF) on renal allograft outcome. This may depend on several variables including the age of the donor. We evaluated whether DGF could ...have different effects in recipients of kidneys from donors aged more than 60 years versus well-matched recipients of younger kidney donors. Patients were retrospectively subdivided into 3 groups. Immediate graft function (IGF), DGF without dialysis (DGF-ND), DGF requiring dialysis (DGF-D). DGF-ND and DGF-D occurred more frequently among 198 older than 198 younger donors (
P = .016 and
P = .044, respectively). The 5-year patient (96% vs 93%) and pure graft (96% vs 89%) survivals were significantly better in younger recipients, while the incidence of acute rejection was similar. After a mean follow-up of 66 ± 44 months in older donor recipients, the graft survival was significantly better among IGF than patients in the DGF-ND (
P = .046) or DGF-D (
P = .003) groups. Instead, in younger recipients there was no difference in graft survival between IGD and DGF-ND. Only patients with DGF-D showed a significantly worse outcome. Upon multivariate analysis of older donors, their recipients, showed the pattern of graft function recovery to be the only variable associated with allograft outcome. Instead in younger donor recipients, acute rejection and time on dialysis were the main variables associated with a poor outcome. In older donor recipients, DGF was an independent variable associated with a poor graft outcome. In younger donor recipients, duration of dialysis and rejection were the most important predictors of poor graft outcomes.

IgA nephropathy (IgAN) is the most common primary glomerulonephritis in the general population. There is accumulating evidence that immunosuppressive treatment is efficacious in IgAN. However, it is ...critical to define appropriate indicators for this therapy especially in the wake of potentially deleterious side effects to immunosuppressives.
This study retrospectively reviewed IgAN cases collected since 1981 to identify clinical and/or histological parameters for disease progression; 310 patients with biopsy proven IgAN, diagnosed from January 1981 to March 2004, were included.
We defined a clinical prognostic index (CPI) using multivariate analysis, which incorporated these clinical/ histological parameters. Semiquantitative scores were assigned as follows: 2 points if creatinine (Cr) was >1.4 mg/dL, 1 point if proteinuria was >1 g/24 hr, 1 point if a patient was affected by hypertension, and 1 point if a patient was older than 30 yrs. Dividing our population into two groups (scores 0-2 = low CPI group; scores 3-5 = high CPI group), we demonstrated a significantly different 10-yr renal survival rate; in the low CPI group, renal survival since time of biopsy at 10 yrs was 91.7%; in the high CPI group the renal survival at 10 yrs was 35%. We validated the CPI in an independent sample from Verona (validation group) and demonstrated similar results for the CPI.
The CPI is convenient to use for defining the risk of disease progression.

Après avoir longtemps négligé la présence des femmes dans l’histoire littéraire et artistique, l’historiographie s’intéresse à elles depuis déjà trois décennies, en particulier dans le cadre des ...études sur les avant-gardes de la première moitié du xxe siècle. De ce point de vue, ce volume ne vient pas combler un vide mais poursuivre et enrichir une recherche déjà engagée en réfléchissant aux façons dont s’articulent, dans les textes théoriques et les pratiques artistiques des avant-gardes européennes de la première moitié du xxe siècle, des questionnements sur les genres littéraires et artistiques et des questionnements liés au genre sexué (ce que l’anglais désigne par le terme de gender). Si ces mouvements d’avant-garde ont cherché à bouleverser la société, quelle place ont-ils faite en leur sein aux femmes artistes et écrivains ? Comment ont-ils pensé les relations entre les sexes ? Y-a-t-il des genres littéraires et artistiques spécifiques pratiqués par les femmes et/ou par les hommes ? Peut-on identifier dans les avant-gardes un discours genré sur les différents genres et pratiques artistiques ? Telles sont certaines des questions auxquelles ce volume cherche à apporter des réponses.
