This study focuses on the methodology used to assess children's inferential skills in a narrative comprehension context with children aged between 5 to 7 years.
The aim of this study is to test the ...validity of a pictorial task based on children's literature to probe children's ability to make inferences at different times during the listening of fictional stories.
The task involves selecting one of three vignettes which best illustrates what is currently happening in the story (i.e., inferential vs. literal vs. off-topic). The choices expressed here are compared to those made during a preference task performed by the same students, which consists of selecting the vignette they like the most among each series presented in the comprehension task.
First, based on the analysis of the data collected in the comprehension task, children most often select inferential vignettes. Second, the choices expressed in the comprehension task differ from those expressed in the preference task.
These results confirm the relevance of using children's literature associated with a pictorial task to test children's inferential skills, even with preschoolers.
Cette étude vise à analyser la méthodologie utilisée pour évaluer les capacités inférentielles des enfants de grande section à CE1 lors de l’écoute d’une histoire narrative.
L’objectif principal est donc de vérifier la validité d’une tâche imagée, à l’aide de matériel issu de la littérature jeunesse, pour évaluer la capacité des enfants à produire des inférences à différent moment de la narration.
Cette tâche consiste à sélectionner parmi trois vignettes (i.e., inférentielle, littérale et hors sujet) celle qui représente au mieux ce qu’il se passe dans l’histoire. Les choix exprimés durant la tâche de compréhension (i.e., tâche d’évaluation des productions inférentielles) sont ensuite comparés à ceux exprimés en tâche de préférence (i.e., tâche de choix des mêmes vignettes sur un critère de préférence esthétique uniquement).
Premièrement, au vu des analyses de résultats lors de la tâche de compréhension, il apparaît que les enfants choisissent le plus souvent la vignette inférentielle. Deuxièmement, les choix exprimés en tâche de compréhension diffèrent de ceux exprimés en tâche de préférence esthétique.
Ainsi, les résultats permettent de confirmer la pertinence de l’utilisation d’histoires issues de la littérature jeunesse et d’un matériel imagé comme élément de réponse pour évaluer les capacités inférentielles des enfants apprentis lecteurs.

Les auteurs décrivent une cure chirurgicale de hernie ombilicale étranglée réalisée en urgence au moyen d’une anesthésie par bloc paraombilical associé à une infiltration locale. Chez ce patient ASA ...3–4, cirrhotique, en occlusion avec vomissements itératifs et trouble de l’hémostase, la réalisation d’une anesthésie générale ou rachidienne semblait à très hauts risques. Le bloc paraombilical, utilisé parfois pour l’anesthésie et/ou l’analgésie périopératoire de la chirurgie réglée de la hernie ombilicale, surtout chez l’enfant, a permis la réalisation de l’acte opératoire dans de bonnes conditions et a procuré une analgésie postopératoire de longue durée. Ce bloc, de réalisation facile, est une alternative intéressante en situation d’urgence chez le patient à risques pour l’anesthésie générale ou rachidienne.
The authors describe a strangulated umbilical hernia surgery performed in emergency with a paraumbilical block associated with a local infiltration. For this patient, 3–4 ASA status, in occlusion, with iterative vomiting and coagulation disorders, general or spinal anaesthesia were high-risk technics. Paraumbilical block, sometimes used for anaesthesia or/and analgesia for programmed umbilical hernia surgery, allowed surgery with good conditions and procured prolonged postoperative analgesia. This block, easy to perform, is an interesting alternative in emergency for general or spinal anaesthesia in high-risk patients.

Il s'agit d'un patient de 52 ans, de classe ASA 3–4, dénutri et éthylotabagique présentant une bronchopathie chronique obstructive et une cirrhose, ayant eu dans les suites opératoires d'une ...chirurgie carcinologique cervicale une pneumopathie d'inhalation ayant entraînée le décès en réanimation par décompensation hépatique. Compte tenu du terrain et des impératifs chirurgicaux, cette infection qualifiée de nosocomiale, semblait difficilement évitable. Les multiples facteurs de risque, les rares mesures préventives utilisables et la jurisprudence actuelle ont été analysés. Dans ce contexte, la tendance médiatique et judiciaire à rendre les médecins responsables des infections nosocomiales devrait être révisée.
We report the case of a 52-year-old man, ASA 3–4, malnourished, heavy smoker and drinker at the stage of chronic obstructive pulmonary disease and cirrhosis. The postoperative course of a cervical carcinologic surgery was complicated by a pneumonia with fatal outcome in the intensive care unit. Taking into account the patient's history and surgical requirements, this nosocomial infection did not appear easily preventable. The multiple risk factors and the few preventive measures usable were analyzed. In this context, the media and legal trend to make the doctors responsible for the nosocomial infections should be revised.
