In the mechanical transmission system, the gear is one of the most widely used transmission components. The failure of the gear will cause serious accidents and huge economic loss. Therefore, the ...remaining life prediction of the gear is of great importance. In order to accurately predict the remaining life of the gear, a new type of long-short-term memory neural network with macroscopic-microscopic attention (MMA) is proposed in this article. First, some typical time-domain and frequency-domain characteristics of vibration signals are calculated, respectively, such as the maximum value, the absolute mean value, the standard deviation, the kurtosis, and so on. Then, the principal component of these characteristics is extracted by the isometric mapping method. The importance of fusional characteristic information is filtered via a proposed MMA mechanism so that the input weight of neural network data and recursive data can reach multilevel real-time amplification. With the new long short-term memory neural network, the health characteristics of gear vibration signals can be predicted based on the known fusion features. The experimental results show that this method can predict the remaining life of gears and bearings well, and it has higher prediction accuracy than the conventional prediction methods.
In the mechatronic system, rolling bearing is a frequently used mechanical part, and its failure may result in serious accident and major economic loss. Therefore, the remaining useful life (RUL) ...prediction of rolling bearing is greatly indispensable. To accurately predict the RUL of the rolling bearing, a new kind of gated recurrent unit neural network with dual attention gates, namely, gated dual attention unit (GDAU), is proposed. With the acquired life-cycle vibration data of a rolling bearing, a series of root mean squares at different time instants are calculated as the health indicator (HI) vector. Next, the to-be HI sequence is predicted by GDAU according to the existing HI vector, and then the RUL of the rolling bearing is estimated. The experimental results show that the proposed GDAU can effectively predict the RULs of rolling bearings, and it has higher prediction accuracy and convergence speed than the conventional prediction methods.
Syphilis is a sexually and vertically transmitted bacterial infection caused by the bacterium Treponema pallidum. Its prevalence is high in low-income and middle-income countries, and its incidence ...has increased in high-income countries in the last few decades among men who have sex with men. Syphilis is a major cause of adverse pregnancy outcomes in low-income and middle-income countries. Clinical features include a primary chancre at the point of inoculation, followed weeks later by the rash of secondary syphilis, a latent period, and in some cases, involvement of the eyes, CNS, and cardiovascular systems. It is diagnosed serologically. A single intramuscular dose of long-acting benzathine penicillin is recommended for people who have had syphilis for less than 1 year and longer courses for people with late latent syphilis. Control strategies include screening and treatment of all pregnant women, and targeted interventions for groups at high risk. Vaccine development, research on antibiotic prophylaxis, and digital messaging as prevention strategies are ongoing.

The remaining useful life (RUL) prediction of servo motors plays a greatly indispensable role in preventing abnormal and inefficient operation of servo drive system and even major safety accident. In ...this article, a novel variant of recurrent neural networks, called gated adaptive hierarchical attention unit (GAHAU), is constructed to efficiently predict the RULs of servo motors, which can not only pay attention to the outputs of reset and update gates but also adaptively divide the hierarchy of the sequence information in attention gate. The updating formulas of GAHAU are derived. With the raw vibration signals of servo motors, a quadratic function-based deep convolutional autoencoder is used for automatically generating the health indicator vector. Based on the constructed HI vector, the RULs of servo motors are accurately predicted by GAHAU. The experiments on servo motors demonstrate that the presented GAHAU has remarkable prediction performance especially for the long-term prediction. Compared to the existing typical RUL prediction methods, the proposed methodology has stronger predictive ability according to the comparative results.
Lithium-ion batteries (LIBs) are currently recognized as one of the most popular power sources available. To construct advanced LIBs exhibiting long-term endurance, great attention has been paid to ...enhancing their poor cycle stabilities. As the performance of LIBs is dependent on the electrode materials employed, the most promising approach to improve their life span is the design of novel electrode materials. We herein describe the rational design of a three-dimensional (3D) porous MnO/C-N nanoarchitecture as an anode material for long cycle life LIBs based on their preparation from inexpensive, renewable, and abundant rapeseed pollen (R-pollen) via a facile immersion-annealing route. Remarkably, the as-prepared MnO/C-N with its optimized 3D nanostructure exhibited a high specific capacity (756.5 mAh·g^-1 at a rate of 100 mA·g^-1), long life span (specific discharge capacity of 513.0 mAh·g^-1, -95.16% of the initial reversible capacity, after 400 cycles at 300 mA·g^-1), and good rate capability. This material therefore represents a promising alternative candidate for the high-performance anode of next-generation LIBs.

Lack of access to quality diagnostics remains a major contributor to health burden in resource-limited settings. It has been more than 10 years since ASSURED (affordable, sensitive, specific, ...user-friendly, rapid, equipment-free, delivered) was coined to describe the ideal test to meet the needs of the developing world. Since its initial publication, technological innovations have led to the development of diagnostics that address the ASSURED criteria, but challenges remain. From this perspective, we assess factors contributing to the success and failure of ASSURED diagnostics, lessons learnt in the implementation of ASSURED tests over the past decade, and highlight additional conditions that should be considered in addressing point-of-care needs. With rapid advances in digital technology and mobile health (m-health), future diagnostics should incorporate these elements to give us REASSURED diagnostic systems that can inform disease control strategies in real-time, strengthen the efficiency of health care systems and improve patient outcomes.

A decrease in the global estimate of four curable sexually transmitted infections (STIs), including chlamydia, gonorrhoea, trichomoniasis, and syphilis, was observed in 2012 by comparison with that ...of 2008, but 2015 saw an estimated 357 million new cases for the four infections combined.1 This overall decrease mainly reflects the contribution of the reduction of syphilis seropositivity among pregnant women over time in countries that consistently report seropositivity data to WHO.2 In the Lancet Infectious Diseases' Commission on STIs,3 Magnus Unemo and colleagues summarise the current challenges for research, practice, and policy on the common STIs-chlamydia, gonorrhoea, and bacterial vaginosis-and comprehensive strategies for STIs, highlighting the introduction of rapid diagnostic tests to improve STI services and respond to epidemics of STIs among men who have sex with men (MSM) in an era of prophylactic treatment or treatment as prevention for HIV. The ADJUVANT study reported the comparative superiority of adjuvant gefitinib over chemotherapy in disease-free survival of resected EGFR-mutant stage II-IIIA non-small cell lung cancer (NSCLC). ...However, not all patients experienced favorable clinical outcomes with tyrosine kinase inhibitors (TKI), raising the necessity for further biomarker assessment. In this work, by comprehensive genomic profiling of 171 tumor tissues from the ADJUVANT trial, five predictive biomarkers are identified (TP53 exon4/5 mutations, RB1 alterations, and copy number gains of NKX2-1, CDK4, and MYC). Then we integrate them into the Multiple-gene INdex to Evaluate the Relative benefit of Various Adjuvant therapies (MINERVA) score, which categorizes patients into three subgroups with relative disease-free survival and overall survival benefits from either adjuvant gefitinib or chemotherapy (Highly TKI-Preferable, TKI-Preferable, and Chemotherapy-Preferable groups). This study demonstrates that predictive genomic signatures could potentially stratify resected EGFR-mutant NSCLC patients and provide precise guidance towards future personalized adjuvant therapy.
Gonorrhea remains one of the most common sexually transmitted diseases worldwide. Successful treatment has been hampered by emerging resistance to each of the antibiotics recommended as first-line ...therapies. We retrospectively analyzed the susceptibility of gonorrhea to azithromycin and ceftriaxone using data from the China Gonococcal Resistance Surveillance Programme (China-GRSP) in order to provide evidence for updating the treatment recommendations in China.
In this study, we included 3,849 isolates collected from patients with a confirmed positive Neisseria gonorrhoeae (N. gonorrhoeae) culture at clinic visits during the period of 1 January 2013 through 31 December 2016 in 7 provinces. Antimicrobial susceptibility testing of gonorrhea isolates using agar dilution was conducted to determine minimum inhibitory concentration (MIC). Resistance to azithromycin (RTA) was defined as MIC ≥ 1.0 mg/l, and decreased susceptibility to ceftriaxone (DSC) was defined as MIC ≥ 0.125 mg/l. The prevalence of isolates with RTA was 18.6% (710/3,827; 95% CI 17.4%-19.8%). The percentage of patients with DSC fluctuated between 9.7% and 12.2% over this period. The overall prevalence of isolates with both RTA and DSC was 2.3% (87/3,827; 95% CI 1.9%-2.8%) and it increased from 1.9% in 2013 to 3.3% in 2016 (chi-squared test for trend, P = 0.03). Study limitations include the retrospective study design and potential biases in the sample, which may overrepresent men with symptomatic infection, coastal residents, and people reporting as heterosexual.
To our knowledge, this is the first national study on susceptibility of N. gonorrhoeae to azithromycin and ceftriaxone in China. Our findings indicate high rates of RTA and DSC from 2013 to 2016. Although dual therapy with azithromycin and ceftriaxone has been recommended by WHO and many countries to treat gonorrhea, reevaluation of this therapy is needed prior to its introduction in China.
