AIM: To determine whether the combination of platelet count(PLT) with spleen volume parameters and right liver volume(RV) measured by magnetic resonance imaging(MRI) could predict the Child-Pugh ...class of liver cirrhosis and esophageal varices(EV).METHODS: Two hundred and five cirrhotic patients with hepatitis B and 40 healthy volunteers underwent abdominal triphasic-enhancement MRI and laboratory examination of PLT in 109/L. Cirrhotic patients underwent endoscopy for detecting EV. Spleen maximal width(W), thickness(T) and length(L) in mm together with spleen volume(SV) and RV in mm3 were measured by MRI, and spleen volume index(SI) in mm3 was obtained by W × T × L. SV/PLT, SI/PLT and RV × PLT/SV(RVPS) were calculated and statistically analyzed to assess cirrhosis and EV.RESULTS: SV/PLT(r = 0.676) and SI/PLT(r = 0.707) increased, and PLT(r =-0.626) and RVPS(r =-0.802) decreased with the progress of Child-Pugh class(P < 0.001 for all). All parameters could determine the presence of cirrhosis, distinguish between each class of Child-Pugh class, and identify the presence of EV the areas under the curve(AUCs) = 0.661-0.973. A m o n g p a ra m e t e r s, R V P S c o u l d b e s t d e t e r m i n e presence and each class of cirrhosis with AUCs of 0.973 and 0.740-0.853, respectively; and SV/PLT could best identify EV with an AUC of 0.782.CONCLUSION: The combination of PLT with SV and RV could predict Child-Pugh class of liver cirrhosis and identify the presence of esophageal varices.

Eleven compounds were separated and identified as p-methoxycinnamic acid(I), protocatechuic aldehyde(II), quercetin(III), isorhamnetin(IV), quercetin 3-O-beta-D-galactopyranoside (V),9-0-beta-D-glucopyranoside-3,4,5-trimethoxy cinnamyl alcohol(VI), syringaresinol-4'-O-beta-D-monoglucoside(VII), ntin(VIII), sibiraic acid(IX), sibiscolacton(X), methyl ferulic acid(XI).

Bilateral ...subdermal soft tissue "C" flaps and "lambda" flap were designed to repair secondary deformities of nasal base and reconstruct vermilion tubercle in patients after BCCL repair.
Good results were achieved in all the patients with primary healing. No flap necrosis happened. The result was satisfactory.
With bilateral subdermal soft tissue "C" flaps and " lambda" flap, nasal base depression deformities and lip whistle deformities can be corrected. It is an ideal method for correction of deformities after BCCL repair.
Leptomeningeal involvement is usually reported as a secondary event in advanced gastric carcinoma.Leptomeningeal carcinomatosis(LMC),as the initial manifestation of asymptomatic gastric cancer,is ...exceedingly rare with only a few cases reported in recent years.The presenting neurologic symptoms include headache,vomiting and seizures and are usually clinically atypical.The diagnosis of LMC is made via identification of malignant cells that originate from epithelial cells in the cerebrospinal fluid by cytological examination and provides cues to track the primary tumor.Endoscopic examinations are crucial to confirm the presence of gastric cancer,and imaging studies,especially gadolinium-enhanced magnetic resonance imaging of the brain,are sometimes helpful in diagnosis.Thus far,there is no standard therapy for LMC,and despite all measures,the prognosis of the condition is extremely poor.Here,we report on the clinical features and diagnostic procedures for a patient with occult gastric cancer with Bormann typeⅠmacroscopic appearance and poor differentiation in pathology,who presented with LMC-induced neurological symptoms as the initial clinical manifestation.Additionally,we review the similar cases reported over the past years,making comparison among cases in order to provide more information for the future diagnosis.
The aim of this study was to investigate the efficiency and safety of ibutilide for cardioversion of persistent atrial fibrillation (AF) during radiofrequency ablation.
Eighteen patients (16 males) ...with persistent atrial fibrillation were enrolled in this study. All patients underwent circumferential pulmonary vein ablation guided by a Carto three-dimensional mapping system. In addition, linear ablation at the top of the left atrium and the isthmus of mitral valves and complex fractionated atrial electrogram (CAFE) ablation were performed. All patients were still in either atrial fibrillation or atrial flutter after ablation, the patients were treated with 1 mg intravenous ibutilide injection within 10 minutes after unsuccessful ablation. Intravenous injection was stopped in case of sinus rhythm (SR) restoration or occurrence of severe adverse reactions such as ventricular tachycardia. Background：Improving islet graft revascularization has become a crucial task for prolonging islet graft survival.Endothelial cells （ECs） are the basis of new microvessels in an isolated islet,and EC ...coating has been demonstrated to improve the vascularization and survival of an islet.However,the traditional method of EC coating of islets has low efficiency in vitro.This study was conducted to evaluate the effect of a polyglycolic acid （PGA） scaffold on the efficiency of islet coating by ECs and the angiogenesis in the coated islet graft.Methods：A PGA fibrous scaffold was used for EC coating of islet culture and was evaluated for its efficiency of EC coating on islets and islet graft angiogenesis.Results：In in vitro experiments,we found that apoptosis index of ECs-coating islet in PGA group （27% ± 8%） was significantly lower than that in control group （83% ± 20%,P 〈 0.05） after 7 days culture.Stimulation index was significantly greater in the PGA group than in the control group at day 7 after ECs-coating （2.07 ± 0.31 vs.1.80 ± 0.23,P 〈 0.05）.vascular endothelial growth factor （VEGF） level in the PGA group was significantly higher than the coating in the control group after 7 days culture （52.10 ± 13.50 ng/ml vs.16.30 ± 8.10 ng/ml,P 〈 0.05）.Because of a tight,circumvallated,adhesive and three-dimensional growth microenvironment,islet cultured in a PGA scaffold had higher coating efficiency showing stronger staining intensity of enzyme than those in the control group after 14 days of culture following ECs-coating.For in vivo study,PGA scaffold significantly prolonged the average survival time of EC-coated islet graft after transplantation compared with control group （15.30 ± 5.60 days vs.8.30 ± 2.45 days,P 〈 0.05）.The angiogenesis and area of survived grafts were more in the PGA group compared with the control group by measuring the mean microvessel density （8.60 ± 1.21/mm2 vs.5.20 ± 0.87/mm2,P 〈 0.05）.In addition,expression of VEGF and tyrosin-protein kinase receptor （Tie-2） gene increased in PGA scaffold group than that in control group by real-time reverse transcription-polymerase chain reaction analysis.Conclusions：These results demonstrate that the efficiency of EC coating of islets was successfully increased by culturing ECs on a PGA scaffold.This method enhances the function,survival,and vascularization of isolated islets in vitro and in vivo.
Sepsis often leads to significant morbidity and mortality due to severe myocardial injury. As is known, the activation of NOD-like receptor family pyrin domain-containing 3 (NLRP3) inflammasome ...crucially contributes to septic cardiomyopathy (SCM) by facilitating the secretion of interleukin (IL)-1β and IL-18. The removal of palmitoyl groups from NLRP3 is a crucial step in the activation of the NLRP3 inflammasome. Thus, the potential inhibitors that regulate the palmitoylation and inactivation of NLRP3 may significantly diminish sepsis-induced cardiac dysfunction.
The present study sought to explore the effects of the prospective flavonoid compounds targeting NLRP3 on SCM and to elucidate the associated underlying mechanisms.
The palmitoylation and activation of NLRP3 were detected in H9c2 cells and C57BL/6 J mice.
Echocardiography, histological staining, western blotting, co-immunoprecipitation, qPCR, ELISA and network pharmacology were used to assess the impact of vaccarin (VAC) on SCM in mice subjected to lipopolysaccharide (LPS) injection. From the collection of 74 compounds, we identified that VAC had the strongest capability to suppress NLRP3 luciferase report gene activity in cardiomyocytes, and the anti-inflammatory characteristics of VAC were further ascertained by the network pharmacology. Exposure of LPS triggered apoptosis, inflammation, oxidative stress, mitochondrial disorder in cardiomyocytes. The detrimental alterations were significantly reversed upon VAC treatment in both septic mice and H9c2 cells exposed to LPS. In vivo experiments demonstrated that VAC treatment alleviated septic myocardial injury, indicated by enhanced cardiac function parameters, preserved cardiac structure, and reduced inflammation/oxidative response. Mechanistically, VAC induced NLRP3 palmitoylation to inactivate NLRP3 inflammasome by acting on zDHHC12. In support, the NLRP3 agonist ATP and the acylation inhibitor 2-bromopalmitate (2-BP) prevented the effects of VAC.
Our findings suggest that VAC holds promise in protecting against SCM by mitigating cardiac oxidative stress and inflammation via priming NLRP3 palmitoylation and inactivation. These results lay the solid basis for further assessment of the therapeutic potential of VAC against SCM.

The present study analyzed the predictive value of combined analysis of collapsin response mediator protein 4 （CRMP4） methylation levels and the Cancer of the Prostate Risk Assessment （CAPRA-S） ...Postsurgical score of patients who required adjuvant hormone therapy （AHT） after radical prostatectomy （RP）, We retrospectively analyzed 305 patients with prostate cancer （PCa） who received RP and subsequent androgen deprivation therapy （ADT）. Two hundred and thirty patients with clinically high-risk PCa underwent immediate ADT, and 75 patients with intermediate risk PCa underwent deferred ADT. CRMP4 methylation levels in biopsies were determined, and CAPRA-S scores were calculated. In the deferred ADT group, the values of the hazard ratios for tumor progression and cancer-specific mortality （CSM） in patients with 〉15% CRMP4 methylation were 6.81 （95% CI, 2.34-19.80） and 12.83 （95% CI. 2.16-26.10）, respectively. Receiver-operating characteristic curve analysis indicated that CRMP4 methylation levels 〉15% served as a significant prognostic marker of tumor progression and CSM. In the immediate ADT group, CAPRA-S scores ：〉6 and CRMP4 methylation levels ：〉15% were independent predictors of these outcomes （uni- and multi-variable Cox regression analyses）. The differences in the 5-year progression-free survival between each combination were statistically significant. Combining CAPRA-S score and CRMP4 methylation levels improved the area under the curve compared with the CRMP4 or CAPRA-S model. Therefore, CRMP4 methylation levels ：〉15% were significantly associated with a poor prognosis and their combination with CAPRA-S score accurately predicted tumor progression and metastasis for patients requiring AHT after RP.
A general method for efficient and selective extraction of a target compound from complex natural products remains elusive, despite decades of research. By introducing a functional amido group on the ...surface of dispersity-enhanced magnetic nanoparticles, a nanoparticle receptor to selectively recognize Sibiskoside (a monoterpene) from the aerial portion of Sibiraea angustata by hydrogen bond interaction was synthesized. The superparamagnetic Fe3O4 nanoparticles were successively modified with tetraethyl orthosilicate (TEOS), amino and amido functional groups, and 4-vinylbenzoic acid (VBZA) was used as the functional monomer. A thin layer of poly (VBZA) imprinted with Sibiskoside was immobilized on the surface of magnetic carriers. Attributing to the amido group introduced into the magnetic particles, the template could attract and bind to the surface and promote the formation of a hydrogen bond system between the carrier, template molecules and functional monomer. High-density molecular recognition sites grew on the surface of magnetic substrate. The adsorption reached equilibrium at approximately 150 min, while fast adsorption occurred during the first 60 min. The maximum adsorption capacity has been found to be 13.75 mg g−1 according to calculation with the Langmuir isotherm. The selectivity coefficients of Molecular imprinting polypers (MIPs) for Sibiskoside with respect to Andrographolide, Loganin, Gastrodin, geraniol-1-O-α-l-rhamnopyranosyl-(1 → 6)-1-β-d-glucopyranoside (GRG), Sibiscolacton and Sibiraic acid were 2.26, 1.43, 1.701.56, 1.05, 0.73 and, respectively. The results indicated that the MIPs possessed good specific adsorption capacity and selectivity toward Sibiskoside and had the potential to be a candidate for the separation and purification of monoterpenes from Sibiraea angustata, which is of great significance to obesity management.
