Cerebrovascular diseases (including strokes) are currently the second leading cause of death in the world 1,2. In Poland, statistical data are quite diverse, depending on their source. Stroke ...patients are often at risk of chronic disability. A significant number of them are doomed to lose their independence and self-care for the rest of their lives. The aim of the study is to present selected nursing problems and ways to solve them in a patient after ischemic stroke who requires round-the-clock care in the field of nursing and care services as part of inpatient long-term care. The subject of the study is the process of nursing a patient diagnosed with ischemic stroke who requires round-the-clock care in the field of nursing and care services as part of long-term care. The study uses a research method - a case study, supported by individual observation, measurements, interview and analysis of medical records. Two scales were used to assess the patient's condition: Barthel and Norton. Conclusions: The individual care plan takes into account the most important problems arising from the specificity of the disease entity, needs and individual situation of the patient. The patient's care problems after a stroke are presented and proposed an effective solution to ensure the best possible care. Nursing and care difficulties have been properly recognized, which has enabled appropriate nursing interventions. Due to systematic prophylactic measures, the patient did not suffer from complications. A detailed interview and careful observation made it possible to plan and implementation of interventions (not only nursing) to provide comprehensive care. The urgent procedure of admitting the patient to the care and treatment facility contributed to the improvement of the quality and availability of health care services, which translated into an improvement in the patient's general condition.
Higiena rąk jest najważniejszym elementem strategii w walce z zakażeniami związanymi z opieką zdrowotną oraz podstawowym zabiegiem służącym ich redukcji. Procedury higieny rąk w pełnym zakresie ...przeprowadza niespełna połowa pracowników opieki zdrowotnej, a dane z prowadzonych badań potwierdzają ciągle niski współczynnik zgodności dla higieny (na poziomie 1,4–6). Lista czynników mających wpływ na przestrzeganie procedury higieny rąk (według wytycznych WHO) przez personel obejmuje niewłaściwe przestrzeganie zasad higieny rąk oraz przeszkody utrudniające właściwą higienę rąk. Wśród przyczyn niestosowania w wystarczającym zakresie techniki mycia i dezynfekcji rąk wskazywane są: zapominanie o wykonaniu procedury, nadmierne obciążenie pracą, pośpiech w trakcie rutynowych czynności pielęgnacyjnych, brak wzorca postępowania wśród innych pracowników i przełożonych, brak odpowiednich nawyków, niedobór środków do dezynfekcji, jednorazowych ręczników na sali chorych, a także podrażnienie skóry rąk wywoływane przez środki odkażające. Higiena rąk jest obszarem wymagającym ustawicznego działania oraz ciągłej edukacji zarówno personelu ochrony zdrowia, jak i odbiorów świadczeń. Czynnikami wpływającymi na brak przestrzegania higieny rąk są m.in. niewystraczający poziom wiedzy i nieprawidłowe nawyki personelu medycznego. Problematyka ta wymaga ciągłej aktualizacji, ponieważ wskaźniki wykonywania procedury są zależne od aktualnej sytuacji.
Cel Celem badań była ocena poziomu wiedzy personelu medycznego z zakresu zasad zapobiegania zakażeniom szpitalnym, w tym postępowania po ekspozycji zawodowej na krew i inny potencjalnie zakaźny ...materiał. Materiał i metody Badanie przeprowadzono w 2018 roku w grupie 101 osób personelu medycznego zatrudnionego w jednym ze śląskich szpitali, z wykorzystaniem metody sondażu diagnostycznego, techniki ankietowania i narzędzia, którym był autorski kwestionariusz ankiety, składający się z metryczki i pytań do oceny wiedzy. Zgromadzone wyniki poddano analizie z wykorzystaniem analizy procentowej oraz statystycznej. Wyniki W grupie badanej 48,51% ogółu stanowiły pielęgniarki, 14,85% lekarze, po 12,87% rehabilitanci i ratownicy medyczni, a 10,89% opiekunowie medyczni. 40,59% badanych posiadało wiedzę na temat najbardziej zakaźnego wirusa w następstwie ekspozycji na krew osoby zakażonej, wskazując HBV. Odpowiednio 44,55% ankietowanych odpowiedziało prawidłowo na pytanie dotyczące ryzyka zakażenia HBV w przypadku zakłucia igłą, a na fakt, że do ekspozycji najczęściej dochodzi podczas nakładania zatyczki na igłę po wykonaniu iniekcji, poprawnie wskazało 54,46% ankietowanych. W zakresie wiedzy ustalono, że badani najczęściej prezentowali bardzo wysoki lub dostateczny poziom wiedzy (po 31,7%). Najwięcej respondentów posiadało wiedzę na temat obowiązkowego szczepienia pracowników opieki zdrowotnej (100,00%), procedury dezynfekcji rąk (97,03%) i czynności, jakie należy wykonać po zakończeniu pobierania krwi (90,10%). Wnioski 1. Poziom wiedzy personelu medycznego na temat zakażeń szpitalnych i podstawowych zasad ich profilaktyki oceniono jako dobry. 2. Poziom wiedzy personelu medycznego z zakresu zasad zapobiegania zakażeniom szpitalnym, w tym postępowania po ekspozycji zawodowej na krew i inny potencjalnie zakaźny materiał, korelował istotnie z: wykształceniem, częstością prowadzenia higieny rąk, poprawnym postępowaniem z użytymi ostrymi narzędziami i materiałem skażonym po użyciu, częstością nakładania zatyczki ochronnej na igłę po wykonaniu iniekcji, wyrzucaniem użytego ostrego sprzętu do przepełnionego pojemnika na odpady medyczne zakaźne oraz zwiększoną ostrożnością podczas kontaktu z pacjentem zakażonym HIV, HBV lub HCV. 3. Na poziom wiedzy badanych nie miały wpływu: miejsce pracy, używanie przez badanych rękawiczek ochronnych i stosowanie innych środków ochrony osobistej. 4. Konieczne i zasadne jest prowadzenie ciągłej edukacji dotyczącej zakażeń szpitalnych, w tym postępowania po ekspozycji zawodowej oraz dzielenia się wiedzą w ramach prowadzonych badań.
Cel: Celem pracy było określenie częstości zachorowalności na naczyniowe uszkodzenie mózgu (NUM) w populacji regionu (powiatu ostrowieckiego), jak również wykazanie związku występowania NUM z płcią ...chorych, ich wiekiem, miejscem zamieszkania oraz czynnikami ryzyka. Materiał i metody: W ramach badań przeanalizowano dokumentację 8603 pacjentów przyjętych do Oddziału Neurologii w Ostrowcu Świętokrzyskim w latach 2003-2008. Spośród tej grupy u 4203 osób stwierdzono NUM. Powyższa grupa stanowiła aż 49% wszystkich osób leczonych w oddziale neurologii. Wyniki: W populacji chorych z NUM kobiety stanowiły 54%, zaś mężczyźni 46%. Stwierdzono, iż najczęściej NUM występuje u osób w wieku ponad 60 lat (74,8% osób). U kobiet średnia wieku wyniosła 76,6 lat, natomiast u mężczyzn 66,5 lat. Odnosząc się do rodzaju NUM stwierdzono, iż udar niedokrwienny stanowił średnio 68% wszystkich postaci NUM, udar krwotoczny – 8,0%, zaś przemijające niedokrwienie mózgu (TIA) potwierdzono u 24% pacjentów. Z chorób współistniejących najczęściej występowało nadciśnienie tętnicze, które stwierdzono u 60,8% chorych z NUM. Hiperlipidemię stwierdzono u 24,5% osób, miażdżycę u 19,8%, cukrzycę u 15,1% chorych, a migotanie przedsionków u 14% badanych. Spośród 4203 osób z NUM 58,4% mieszkało w mieście, zaś 41,6% na wsi. Wnioski: 1. Częstość występowania NUM jest większa niż w innych regionach; 2. Wiek (powyżej 60 roku życia) jest czynnikiem ryzyka wystąpienia NUM; 3. W grupie osób chorych na NUM dominują kobiety; 4. Zamieszkiwanie w mieście predysponuje do występowania zaburzeń o charakterze NUM; 5. Dominującym typem NUM jest udar niedokrwienny; 6. Współistniejące z NUM choroby to nadciśnienie tętnicze, hiperlipidemia, migotanie przedsionków, miażdżyca i cukrzyca.
Fermentation is widely used in the processing of dairy, meat, and plant products. Due to the growing popularity of plant diets and the health benefits of consuming fermented products, there has been ...growing interest in the fermentation of plant products and the selection of microorganisms suitable for this process. The review provides a brief overview of lactic acid bacteria (LAB) and their use in fermentation of legumes and legume-based beverages. Its scope also extends to prebiotic ingredients present in legumes and legume-based beverages that can support the growth of LAB. Legumes are a suitable matrix for the production of plant-based beverages, which are the most popular products among dairy alternatives. Legumes and legume-based beverages have been successfully fermented with LAB. Legumes are a natural source of ingredients with prebiotic properties, including oligosaccharides, resistant starch, polyphenols, and isoflavones. These compounds provide a broad range of important physiological benefits, including anti-inflammatory and immune regulation, as well as anti-cancer properties and metabolic regulation. The properties of legumes make it possible to use them to create synbiotic food, which is a source of probiotics and prebiotics.
The objectives of this study were to compare the properties of the yogurt-type bean-based beverages B and BG produced from the nongerminated and germinated beans, respectively, by high-pressure ...homogenization (HPH) and fermentation with three starter cultures. Optical techniques were used to evaluate the particle size distribution (PSD), color parameters, and instability during storage, while rheological tests were used to evaluate the shear viscosity, flow behavior, and viscoelastic properties. The BG compared to B, irrespective of the starter culture used, showed a higher mean diameter and Span of PSD (d4,3 ≈ 76.8–84.2, Span ≈ 2.24–2.35 for BG vs. d4,3 ≈ 38.2–47.0, Span ≈ 1.90–2.00 for B). The BG vs. B showed lower viscosity (0.47 Pa·s for BG vs. 0.81 Pa·s for B at shear rate 75 s−1) and slightly lower but satisfactory stability (after 21 days at 6 °C, the Turbiscan Stability Index TSI ≈ 1.3–2.0 for BG vs. TSI ≈ 0.6–0.9 for B). Both B and BG were characterized by light-yellow color and showed the characteristics of a viscoelastic fluid. The HPH and germination mainly affected the properties of the tested plant tissue, which has a direct impact on the properties of the final products.

In recent times, consumers have shown increasing interest in plant substitutes for fermented dairy products. This study aimed to investigate the properties of yogurt-type rice-based beverages ...fermented with lactic acid bacteria and Propionibacterium. The changes in pH, viable population of bacteria, physical properties, and carbohydrate content of these beverages were tested. Fermentation using only Propionibacterium was insufficient to obtain a product with an acidity level similar to that of milk-based yogurt (pH < 4.5). After fermentation, the tested beverages had a high number of Lactobacillus sp. (7.42−8.23 log10 CFU/mL), Streptococcus thermophilus (8.01−8.65 log10 CFU/mL), and Bifidobacterium animalis subsp. lactis (8.28−8.50 log10 CFU/mL). The hardness (2.90−10.40 N) and adhesiveness (13.79−42.16 mJ) of the samples after 14 days of storage at 6 °C varied depending on the starter culture used. The syneresis of all samples ranged between 29% and 31%, which was lower or close to that of milk-based yogurts. The content of individual sugars in the samples also varied depending on the starter culture used for fermentation. The results suggest that the combination of lactic and propionic fermentation helps in the production of rice-based yogurt-type milk substitutes.

Abstract
The market for plant-based beverages (PBBs) is relatively new; hence, to enable its further development, it is important to use new raw materials and improve production technology. The use ...of natural biotechnological processes can diversify the segment of PBBs, which may offer products with better functionality than those available in the market. Therefore, the present study aimed to determine the effects of fermentation and germination on the nutritional properties of bean-based beverages (BBs) and lentil-based beverages (LBs). The applied processes significantly (p ≤ 0.05) influenced the characteristics of PBBs. Fermentation improved the antioxidant properties (e.g., by increasing the level of 1,1-diphenyl-2-picrylhydrazyl radical scavenging activity by 2–6% and 3–7% for BBs and LBs, respectively) and modified the fatty acid (FA) profile of PBBs. This process increased the share of polyunsaturated FAs in the sn2 position in triacylglycerols, which may promote its absorption in the intestine. The simultaneous use of germination and fermentation was most effective in decreasing oligosaccharide content (< 1.55 mg/kg), which may reduce digestive discomfort after consuming PBBs. We recommend that the designing of innovative legume-based beverages should include the application of fermentation and germination to obtain products with probiotic bacteria and improved nutritional properties.

This study aimed at investigating the influence of different variants of bacterial starter cultures on the metabolism of the bacteria used, cheese protein digestibility, and fatty acid profile. The ...results revealed that lactic acid bacteria had a significant effect on the proportions of fatty acids in cheeses, with saturated fatty acids being predominant in in all cheese variants. Fatty acid proportions are complex and depend on the type of cheese culture and monoculture used. Additionally, the analysis of fatty acid composition showed variations in the proportion of saturated and unsaturated fatty acids, impacting the values of atherogenic and thrombogenic indices. Notably, the atherogenic index was highest in samples of mature cheeses obtained from a typical mesophilic cheese culture, whereas it was lowest in samples of fresh milk and mature cheeses obtained from a mesophilic cheese culture and monocultures of Lacticaseibacillus casei and Propionibacterium. The study also highlighted the influence of lactobacilli on the content of available free lysine, glycine, and methionine in cheese proteins. Mature cheeses obtained with Propionibacterium and L. casei starter cultures exhibited higher free lysine and glycine content compared with fresh cheeses and those obtained solely with the cheese culture. Additionally, mature cheeses obtained with starter cultures of mesophilic cheese culture, Propionibacterium, and L. casei had the highest free methionine content. Based on these findings, it is evident that the choice of cheese making cultures and monocultures can significantly affect the fatty acid composition and amino acid content of cheese and fresh milk, potentially bearing important health implications.
