An assessment of current and future emissions, air concentrations, and atmospheric deposition of mercury worldwide is presented on the basis of results obtained during the performance of the EU GMOS ...(Global Mercury Observation System) project. Emission estimates for mercury were prepared with the main goal of applying them in models to assess current (2013) and future (2035) air concentrations and atmospheric deposition of this contaminant. The combustion of fossil fuels (mainly coal) for energy and heat production in power plants and in industrial and residential boilers, as well as artisanal and small-scale gold mining, is one of the major anthropogenic sources of Hg emissions to the atmosphere at present. These sources account for about 37 and 25 % of the total anthropogenic Hg emissions globally, estimated to be about 2000 t. Emissions in Asian countries, particularly in China and India, dominate the total emissions of Hg. The current estimates of mercury emissions from natural processes (primary mercury emissions and re-emissions), including mercury depletion events, were estimated to be 5207 t year−1, which represents nearly 70 % of the global mercury emission budget. Oceans are the most important sources (36 %), followed by biomass burning (9 %). A comparison of the 2035 anthropogenic emissions estimated for three different scenarios with current anthropogenic emissions indicates a reduction of these emissions in 2035 up to 85 % for the best-case scenario. Two global chemical transport models (GLEMOS and ECHMERIT) have been used for the evaluation of future mercury pollution levels considering future emission scenarios. Projections of future changes in mercury deposition on a global scale simulated by these models for three anthropogenic emissions scenarios of 2035 indicate a decrease in up to 50 % deposition in the Northern Hemisphere and up to 35 % in Southern Hemisphere for the best-case scenario. The EU GMOS project has proved to be a very important research instrument for supporting the scientific justification for the Minamata Convention and monitoring of the implementation of targets of this convention, as well as the EU Mercury Strategy. This project provided the state of the art with regard to the development of the latest emission inventories for mercury, future emission scenarios, dispersion modelling of atmospheric mercury on a global and regional scale, and source–receptor techniques for mercury emission apportionment on a global scale.
Human activities have altered the biogeochemical cycle of mercury (Hg) since precolonial times, and anthropogenic activities will continue to perturb the natural cycle of Hg. Current estimates ...suggest the atmospheric burden is three to five times greater than precolonial times. Hg in the upper ocean is estimated to have doubled over the same period. The Minamata convention seeks to reduce the impact human activities have on Hg releases to the environment. A number of the Articles in the Convention concern the development of detailed inventories for Hg emissions and releases. Using the global Hg chemical transport model, ECHMERIT, the influence of the anthropogenic emission inventory (AMAP/UNEP, EDGAR, STREETS) on global Hg deposition patterns has been investigated. The results suggest that anthropogenic Hg emissions contribute 20–25% to present-day Hg deposition, and roughly two-thirds of primary anthropogenic Hg is deposited to the world’s oceans. Anthropogenic Hg deposition is significant in the North Pacific, Mediterranean and Arctic. The results indicate immediate reductions in Hg emissions would produce benefits in the short term, as well as in the long term. The most impacted regions would be suitable to assess changes in Hg deposition resulting from implementation of the Minamata convention.

Context: Protamine is routinely administered to neutralize the anticlotting effects of heparin, traditionally at a dose of 1 mg for every 100 IU of heparin-a 1:1 ratio protamine sparing effects-but ...this is based more on experience and practice than literature evidence. The use of Hemostasis Management System (HMS) allows an individualized heparin and protamine titration. This usually results in a decreased protamine dose, thus limiting its side effects, including paradox anticoagulation.
Aims: This study aims to assess how the use of HMS allows to reduction of protamine administration while restoring the basal activated clotting time (ACT) at the end of cardiac surgery.
Settings and Design: A retrospective observational study in a tertiary care university hospital.
Subjects and Methods: We analyzed data from 42 consecutive patients undergoing cardiopulmonary bypass (CPB) for cardiac surgery. For all patients HMS tests were performed before and after CPB, to determine how much heparin was needed to reach target ACT, and how much protamine was needed to reverse it.
Results: At the end of cardiopulmonary bypass, 2.2 ± 0.5 mg/kg of protamine was sufficient to reverse heparin effects. The protamine-to-heparin ratio was 0.56:1 over heparin total dose (a 44% reduction) and 0.84:1 over heparin initial dose (a 16% reduction).
Conclusion: A lower dose of protamine was sufficient to revert heparin effects after cardiopulmonary bypass. While larger studies are needed to confirm these findings and detect differences in clinically relevant outcomes, the administration of a lower protamine dose is endorsed by current guidelines and may help to avoid the detrimental effects of protamine overdose, including paradox bleeding.

Nowadays, the necessity of having a cardioplegia circuit capable of being adapted in order to administer different types of cardioplegia is strategically fundamental, both for the perfusionist and ...for the cardiac surgeon. This allows to avoid cutting tubes, guarantees sterility and, most of all, limits the number of cardioplegia circuits for the different strategies of cardiac arrest. The novel “ReverseTWO cardioplegia circuit system” is the development of the precedent “Reverse system” where mainly the 4:1 and crystalloid cardioplegia were used, It has the advantage of allowing immediate change of cardioplegia set-up versus four types of cardioplegia technique, when the strategy is unexpectedly changed before the beginning of cardiopulmonary bypass (CPB), is safe and enables the perfusionist to use one single custom pack of cardioplegia. Two pediatric roller pumps are usually used in our centre for cardioplegia administration; they have a standardized calibration (the leading with ¼ inch and the follower with 1/8 inch) and the circuit consequently has two different tube diameters for the two different pumps. The presence in the circuit of two different shunts coupled with two different coloured clamps allows the immediate set-up for different cardioplegia administration techniques utilizing a colour-coding mechanism The aim of this manuscript is to present the new ReverseTWO Circuit. This novel system allows to administer four different cardioplegic solutions (4:1, 1:4, crystalloid, ematic) based on multiple tubes, which can be selectively clamped, identified through a color-coding method. This work provides the first continuous measurements of carbonaceous aerosol at the Global Atmosphere Watch (GAW) Monte Curcio regional station, within the southern Mediterranean basin. We ...specifically analyzed elemental carbon (EC) and organic carbon (OC) concentrations in particulate matter (PM) samples, collected from April to December during the two years of 2016 and 2017. The purpose of the study is to understand the behavior of both PM and carbonaceous species, in their fine and coarse size fraction, along with their seasonal variability. Based on 18 months of observations, we obtained a dataset that resulted in a vast range of variability. We found the maximum values in summer, mainly related to the enhanced formation of secondary pollutants owing to intense solar radiation, also due to the high frequency of wildfires in the surrounding areas, as well as to the reduced precipitation and aerosol-wet removal. We otherwise observed the lowest levels during fall, coinciding with well-ventilated conditions, low photochemical activity, higher precipitation amounts, and less frequency of Saharan dust episodes. We employed the HYSPLIT model to identify long-range transport from Saharan desert. We found that the Saharan dust events caused higher concentrations of PM and OC in the coarser size fraction whereas the wildfire events likely influenced the highest PM, OC, and EC concentrations we recorded for the finer fraction.

Mercury is a toxic pollutant that poses risks for the human population, mainly by eating contaminated fish. Mercury is released into the atmosphere from a variety of anthropogenic activities, with levels of emissions and under policy controls that largely vary across the world, leading thus to different relative contributions to the environmental matrices. Establishing the exact sources of this contaminant in the environment is crucial to optimising the policies aimed at mitigating the exposure risks for specific populations or ecosystems. In this study, we modelled, for the first time, the fingerprint of mercury anthropogenic emissions, jointly released by source-sectors (11) and source-regions (13), on the deposition over (19) FAO fishery zones, and on the FAO official fishery productions worldwide over the 2012–2021 decade. Using mercury anthropogenic emissions for 2012 from EDGAR, East Asia and ”Artisanal and Small scale Gold Mining” result the source-region and the source-sector, respectively, that contribute the most to the mercury deposition over all the FAO fishery zones. The only exception applies for the FAO fishery zone 37, the Mediterranean Sea, where the ”Industrial Combustion” from the closest Europe is the pair region-sector whose joint contribution is the greatest. When normalised to the overall fishery production worldwide, representing the global fish consumption, the anthropogenic mercury fingerprint showed a similar general pattern, however with notable differences, amplifying the relative contributions of all source-sectors from East Asia and attenuating the relative contributions of the regions in the Southern Hemisphere. This fingerprint further changes when the fish consumption in countries, classified by the World Bank as having different incomes, is considered. These results demonstrate that the same anthropogenic mercury deposited on any fishery zone actually affects in a different way the different population segments worldwide. This study aims to urge the science community as well as the policy makers to use a measure that better represents the mercury hazard for human health. Further, we hope that this study, using nomenclatures that are largely used on final shelf-product, could increase the people’s awareness regarding the products they consume.

Mass balance of contaminants can provide useful information on the processes that influence their concentrations in various environmental compartments. The most important sources, sinks and the ...equilibrium or non-equilibrium state of the contaminant in individual environmental compartments can also be identified. Using the latest mercury speciation data, the results of numerical models and the results of recent studies on mercury transport and transformation processes in the marine environment, we have re-evaluated the total mercury (HgT) mass balance in the Mediterranean Sea. New calculations have been performed employing three distinct marine layers: the surface layer, the thermocline and the deep sea. New transport mechanisms, deep water formation and density-driven sinking and upwelling, were included in the mass balance calculations. The most recent data have even enabled the calculation of an approximate methylmercury (MeHg) mass balance. HgT is well balanced in the entire Mediterranean, and the discrepancies between inputs and outputs in individual layers do not exceed 20 %. The MeHg balance shows larger discrepancies between gains and losses due to measurement uncertainties and gaps in our knowledge of Hg species transformation processes. Nonetheless, the main sources and sinks of HgT (deposition and evasion) and MeHg (fluxes from sediment, outflow through the Gibraltar Strait) are in accordance with previous studies on mercury in the Mediterranean Basin. Mercury in the Mediterranean fish harvest is the second largest MeHg sink; about 300 kg of this toxic substance is consumed annually with sea food.
Atmospheric mercury emission scenarios from artisanal and small-scale gold mining for 56 tropical and subtropical countries have been elaborated and assessed for their comparative significance. A ...multi-step quantitative method that yields narrow and robust confidence intervals for mercury emission estimates was employed. Firstly, data on gold production for different years, the ratio of mercury used in the different amalgamation processes, and ancillary input parameters were retrieved from official and unofficial sources, and their potential for emission reduction examined. Then, a Monte Carlo method to combine the data and generate mercury emission samples was used. These samples were processed by a non-parametric re-sampling method (bootstrap) to obtain robust estimates of mercury emissions, and their 95% confidence intervals, both for the current state and for the emission scenarios designed in this study. The artisanal and small-scale gold mining mercury emission (to the atmosphere) estimates agree with those reported in the Global Mercury Assessment 2018; however, the overall uncertainty is reduced from roughly 100% in the Global Mercury Assessment (779.59 tons/y; uncertainty range: 361.07–1197.97) to 27% (1091.93 tons/y; confidence interval at 95% level of confidence: 964.54–1219.77) in this study. This is a substantial outcome since the narrowing of the confidence intervals permits a more meaningful evaluation of the different emission scenarios investigated, which otherwise, given the broad uncertainty of other estimates, would have led only to vague conclusions in a study of this nature.
Gastroresistant microparticles for oral administration of hesperidin (Hd) were produced by spray-drying using cellulose acetate phthalate (CAP) as enteric polymer in different polymer/Hd weight ratio ...(1:1, 3:1, and 5:1), and a series of enhancers of the dissolution rate, such as sodium carboxymethylcellulose crosslinked (CMC), sodium dodecylbenzene sulfonate (SDBS), or Tween85. The raw materials and the microparticles were investigated by differential-scanning calorimetry, X-ray diffraction, infrared spectroscopy and imaged using scanning electron and fluorescence microscopy.
In vitro
dissolution tests were conducted using a pH-change method to investigate the influence of formulative parameters on the dissolution/release properties of the drug. CAP/Hd microparticles showed a good gastro-resistance but incomplete drug dissolution in the simulated intestinal fluid (SIF). The presence of the enhancers in the formulation produced well-formed microparticles with different size and morphology, containing the drug well coated by the polymer. All the enhancers were able to increase the dissolution rate of Hd in the simulated intestinal environment without altering CAP ability to protect Hd in the acidic fluid. The spray-drying technique and process conditions selected were effective in microencapsulating and stabilizing the flavonoid giving satisfactory encapsulation efficiency, product yield, and microparticles morphology, and a complete drug release in the intestine.

Background: Recently, evidence of cardio-protection and reduction in mortality due to the use of volatile agents during cardiac surgery led to an increase in their use during cardiopulmonary bypass ...(CPB). These findings seem to be enhanced when the volatile agents are used during all the surgical procedure, including the CPB period. Aims: Since the administration of volatile agents through CPB can be beneficial to the patients, we decided to review the use of volatile agents vaporized in the CPB circuit and to summarize some tricks and tips of this technique using our 10-year experience of Brazilian and Italian centers with a large volume of cardiac surgeries. Study Setting: Hospital. Methods: A literature review. Results: During the use of the volatile agents in CPB, it is very important to analyze all gases that come in and go out of the membrane oxygenators. The proper monitoring of inhaled and exhaled fraction of the gas allows not only monitoring of anesthesia level, but also the detection of possible leakage in the circuit. Any volatile agent in the membrane oxygenator is supposed to pollute the operating theater. This is the major reason why proper scavenging systems are always necessary when this technique is used. Conclusion: While waiting for industry upgrades, we recommend that volatile agents should be used during CPB only by skilled perfusionists and physicians with the aim to reduce postoperative morbidity and mortality.
