While methadone maintenance treatment (MMT) has been demonstrated to be an effective treatment for opiate dependence, its impact on the treatment outcome of other illicit drug abuse is not as clear. ...Using the initial urine drug screen (UDS) and follow-up UDS at 1, 6, 12, and 24 months, 167 patients consecutively admitted to MMT were evaluated for opiate, sedative (predominantly benzodiazepine), and stimulant (predominantly cocaine) use. Retention for the opiate only group was 97.32 days longer on average than for patients using opiates along with stimulants, sedatives, or both stimulants and sedatives. Patients abusing opiates only had the greatest decrease in drug use; however, MMT was also associated with decreases in cocaine and sedative use over the 24 month follow-up period. There was no evidence that patients "switched" their drugs of abuse with time in treatment. The negative impact of non-opiate drug use on outcome in MMT and its implications for treatment planning are discussed.

In the primary care setting, use of the BATHE (Background, Affect, Trouble, Handling, and Empathy) method of interviewing has been shown to increase patient satisfaction. This technique is a brief ...psychotherapeutic method used to address patients' physical and psychosocial problems. The BATHE technique has not been evaluated in the perioperative setting as a way of improving patient satisfaction. In this study, we sought to determine whether satisfaction could be enhanced by use of the BATHE technique during the preoperative evaluation by anesthesiologists.
Fifty cardiac and 50 general surgery patients were interviewed in the preanesthesia clinic (PAC) of an academic hospital. They were randomly enrolled in the BATHE group or the control group and asked to complete an anonymous satisfaction survey after their visit. This survey was modified from current studies and not validated elsewhere. The relative influence of the BATHE condition was examined as it pertained to interview duration, patient satisfaction, and patient report of the BATHE items being asked.
Ninety-two percent of patients approached by the study group voluntarily enrolled. Patients interviewed using the BATHE method reported being asked about all BATHE questions significantly more often than control patients: t(98)=19.10, P=0.001 (95% confidence interval CI=2.59, 3.20). Patients in the BATHE group were more satisfied with their visit to the PAC than those in the control group: t(98)=5.37, P=0.001 (95% CI=0.19, 0.41). The use of the BATHE method did not significantly increase the amount of time physicians spent evaluating patients: t(98)=0.110, P=0.912 (95% CI=-1.519, 1.359).
Use of the BATHE method in an academic medical center's cardiac and general PAC showed promising results in this preliminary study. A validated and fully developed survey instrument is needed before we can convincingly conclude that the BATHE method is an effective way of improving patient satisfaction.
Correct characterization of heavy metal availability is a prerequisite for the management of polluted soils. Our objective was to describe zinc (Zn) availability in polluted soils by measuring the ...isotopic exchangeability of Zn in soil/solution (E value) and in soil/plant systems (L value), by assessing the transfer of Zn and ⁶⁵Zn in the fractions of a six-step selective sequential extraction (SSE) in incubated soils and by identifying Zn forms in soils by means of extended X-ray absorption fine structure (EXAFS) spectroscopy. We distinguished three pools of exchangeable Zn: the pool of Zn exchangeable within 1 minute, which is observed in all soils, Zn exchangeable in the medium term, and the slowly and not exchangeable Zn. The amount of Zn present in the first two pools was similar to the L value measured with Thlaspi caerulescens. The first three steps of the SSE solubilized the first pool and a fraction of the second pool. Most of the second pool and a fraction of the third pool were extracted in the fourth step of the SSE, while the rest of the third pool was extracted in the final steps of the SSE. The EXAFS study conducted on two soils showed that more than half of the Zn was present in species weakly bound to organic compounds and/or outer sphere inorganic and organic complexes. Other species included strongly sorbed Zn species and Zn species in crystalline minerals. The EXAFS study of selected SSE residues showed that the specificity and the efficiency of the extractions depended on the properties of the soil studied.

ABSTRACT
Two meteorite pieces have been recovered in Italy, near the town of Cavezzo (Modena), on 2020 January 4th. The associated fireball was observed on the evening of New Year’s Day 2020 by eight ...all-sky cameras of the PRISMA fireball network, a partner of FRIPON. The computed trajectory had an inclination angle of approximately 68° and a velocity at infinity of 12.8 km s−1. Together with the relatively low terminal height, estimated as 21.5 km, those values were indicating the significant possibility of a meteorite dropping event, as additionally confirmed by the non-zero residual total mass. The strewn-field was computed taking into account the presence of two bright light flashes, revealing that the meteoroid had been very likely subject to fragmentation. Three days after the event, two samples, weighing 3.1 and 52.2 g, were collected as a result of a dedicated field search and thanks to the involvement of the local people. The two pieces were immediately recognized as freshly fallen fragments of meteorite. The computed orbital elements, compared with the ones of known Near-Earth Asteroids from the NEODyS database, are compatible with one asteroid only; 2013 VC10. The estimated original mass of the meteoroid, 3.5 kg, and size, approximately 13 cm, is so far the smallest among the current 35 cases in which meteorites were recovered from precise strewn-field computation thanks to observational data. This result demonstrates the effectiveness of accurate processing of fireball network data even on challenging events generated by small size meteoroids.
Background: Methadone has been effectively used in the treatment of opiate dependence. Adequate dose and blood level have correlated with success in treatment. A number of factors including the ...regular use of alcohol, medications, and urinary pH can influence blood level and thereby effectiveness. Fluvoxamine has been shown to increase methadone blood levels.
Methods: Single case report.
Results: A patient unable to maintain an effective methadone blood level despite a dose of 200 mg per day was administered fluvoxamine with subsequent increase in her methadone blood level and reduction of opiate withdrawal symptoms.
Conclusions: In patients unable to maintain an effective methadone blood level throughout the dosing interval, fluvoxamine can help increase the methadone blood level and alleviate opiate withdrawal symptoms.

Two polymorphisms of the human mu opioid receptor gene are described. A non‐coding region polymorphism (G to T) occurs at nucleotide 175 preceding the initiation of translation. A coding polymorphism ...in exon 1 (C to T) at nucleotide 229 changes an alanine residue to a valine residue. Frequencies of these polymorphisms were examined in groups of cocaine and/or opioid dependent individuals and matched controls. There were no significant differences between groups, although a trend (p= 0.05) towards a higher frequency of the 229 valine allele was observed in the substance abuse group, suggesting a need for large, well‐controlled studies of this polymorphism in severe substance abusers.
