BACKGROUND Systemic inflammatory response and nutritional status are important to the prognosis of patients with colorectal cancer (CRC). This study aimed to investigate the prognostic value of the ...combination of preoperative hemoglobin, lymphocyte, albumin, and neutrophil (HLAN) in patients with locally advanced CRC (LACRC). MATERIAL AND METHODS We performed a retrospective analysis in 536 LACRC patients undergoing radical surgery. The value of HLAN was defined as follow: HLAN=Hemoglobin (g/L)×Lymphocyte (/L)×Albumin (g/L)/Neutrophil (/L)/100. The X-tile program was used to determine the optimal cut-point of HLAN, and the prognostic value of HLAN for overall survival (OS) was evaluated with the Cox proportional hazard model. RESULTS The cut-point of HLAN was set at 19.5. Compared with the high-HLAN group, the low-HLAN group had a 1.50-fold (95% confidence interval 1.09-2.05) increased risk of death and a significantly lower OS rate (P<0.001). Furthermore, the risk stratification model based on HLAN (AUC=0.72) displayed better accuracy in OS prediction than the TNM system (AUC=0.61). CONCLUSIONS HLAN is a valuable prognostic marker for patients with LACRC.
Mountain road tunnels are prone to water leakage and lining corrosion under the complex geological conditions and corrosive environments, which will reduce the strength of the lining structure until ...it loses its load-bearing capacity; eventually, the definitive lining will need to be replaced. In this paper, a highway tunnel in a mountainous area in Southwest China is taken as an example. Field investigation found that the tunnel was seriously corroded by sulfate, the strength of the definitive lining decreased, and large-scale cracks and spalling appeared on the surface, so the operator decided to replace the definitive lining by the method of interval replacement. Based on the data obtained from drilling and coring, a numerical model of long-distance replacement of the definitive lining of the damaged tunnel is established. First, the back analysis of the calculation parameters is carried out, and the modified calculation results are compared with the field monitoring results for verification. Then, the deformation trend of the tunnel and the development of the plastic zone during the process of long-distance replacement of the definitive lining are studied. Finally, the construction scheme is optimized. Numerical analysis results show that the replacement of the definitive lining of the tunnel mainly leads to the settlement of the arch crown and the uplift of the inverted arch. The deformation of the tunnel shows two rapid growth stages and two stable stages during the replacement process; after replacement, the deformation of the arch crown and the inverted arch is divided into two buffer zones and one stable zone. In the progress of the replacement of the definitive lining, the plastic zone does not change. Regarding the reinforcement measures, with the increase in the grouting range, the grouting efficiency decreases, and the effect of the temporary steel arch on controlling the overall deformation is not obvious. The length of the replacement of the single section should be determined according to the geological conditions of the replacement section and the monitoring data during construction. The research results can provide a reference for similar projects for the replacement of the definitive lining.

Purpose
The present study was done to establish an objective, sensitive prognostic scoring system and to determine the applicability of this model in predicting the 3-month mortality of patients with ...acute-on-chronic liver failure in hepatitis B (ACLFB).
Methods
We developed a novel prognostic scoring system, calculated from six clinical indices including serum total bilirubin, prothrombin activity, serum creatinine, hepatic encephalopathy, infections, and the depth of ascites from 499 patients with ACLFB. Differences in the sensitivity, specificity, and practicality of a Novel prognostic scoring system and the model of end-stage liver disease (MELD) were analyzed.
Results
The areas under the receiver operating characteristic curve (ROC) for the Novel scoring systems and MELD scoring systems were 0.967 (95% CI, 0.956–0.977) and 0.900 (95% CI, 0.878–0.922), respectively. The analysis of the ROC curve indicated that the Novel scoring systems were an exact, pertinent, and objective prognostic model with greater accuracy than the MELD. In the Novel scoring systems, the survival rate of these patients whose scores ranged from 2 to 6 was 98.80%, while for those whose scores point at 7 and 15, the mortality rates were 8.70% (2/23) and 95.45% (21/22), respectively, and the mortality rate of these patients whose scores were 16 and above was 100.00%. However, in the MELD prognostic scoring systems, there were no score ranges with 100.00% survival rate.
Conclusions
We developed an objective, pertinent, and sensitive prognostic scoring system that predicted the 3-month mortality of patients with ACLFB with greater accuracy than the MELD.

Rubidium (Rb) deposits mostly occur in the South China and Central Asia orogenic belts and are often closely associated with highly differentiated granites. This study investigates a newly‐discovered ...giant Rb deposit at Gariatong in the Central Lhasa terrane in Tibet. Detailed field studies and logging data revealed that the Rb mineralization mainly occurs in monzogranite and is related to greisenization. LA‐ICP‐MS U‐Pb dating of zircon yielded ages of 19.1 ± 0.2 Ma and 19.0 ± 0.2 Ma for greisenized monzogranite and fresh monzogranite, respectively. The monzogranites are characterized as strongly peraluminous, with high contents of SiO2, Al2O3, K2O and Na2O as well as a high differentiation index. They are enriched in light rare earth and large ion lithophile elements with significant negative Eu anomalies and depleted high field‐strength elements. Petrological and geochemical features of these ore‐related monzogranites suggest that they are highly fractionated S‐type granites, derived from remelting of crustal materials in a post‐collisional setting. The geochemistry of zircon and apatite points to a low oxygen fugacity of the ore‐related monzogranite during the magma's evolution. The discovery of the Gariatong Rb deposit suggests that the Central Lhasa terrane may be an important region for rare metal mineralization.

In recent years, many text summarization models based on pre-training methods have achieved very good results. However, in these text summarization models, semantic deviations are easy to occur ...between the original input representation and the representation that passed multi-layer encoder, which may result in inconsistencies between the generated summary and the source text content. The Bidirectional Encoder Representations from Transformers (BERT) improves the performance of many tasks in Natural Language Processing (NLP). Although BERT has a strong capability to encode context, it lacks the fine-grained semantic representation. To solve these two problems, we proposed a semantic supervision method based on Capsule Network. Firstly, we extracted the fine-grained semantic representation of the input and encoded result in BERT by Capsule Network. Secondly, we used the fine-grained semantic representation of the input to supervise the fine-grained semantic representation of the encoded result. Then we evaluated our model on a popular Chinese social media dataset (LCSTS), and the result showed that our model achieved higher ROUGE scores (including R-1, R-2), and our model outperformed baseline systems. Finally, we conducted a comparative study on the stability of the model, and the experimental results showed that our model was more stable.

An excision biopsy of the mass showed lung squamous cell carcinoma.
We performed radiofrequency ablation.
The patient underwent 3.5 and 10 months of follow-up, with a partial response and complete remission, respectively.
CT-guided radiofrequency ablation is a safe and an effective minimally invasive treatment option. Radiofrequency appears to be a valuable alternative to surgery for inoperable patients presenting with a single-lung NSCLC.

Host defense and inflammation are regulated by the NF-kB essential modulator (NEMO), a scaffolding protein with a broad immune cell and tissue expression profile. Hypomorphic mutations in inhibitor ...of NF-kB kinase regulatory subunit gamma (IKBKG) encoding NEMO typically present with immunodeficiency. Here, we characterized a pediatric autoinflammatory syndrome in 3 unrelated male patients with distinct X-linked IKBKG germline mutations that led to overexpression of a NEMO protein isoform lacking the domain encoded by exon 5 (NEMO- \ex5). This isoform failed to associate with TANK binding kinase 1 (TBK1), and dermal fibroblasts from affected patients activated NF-kB in response to TNF but not TLR3 or RIG-I-like receptor (RLR) stimulation when isoform levels were high. By contrast, T cells, monocytes, and macrophages that expressed NEMO-Aex5 exhibited increased NF-kB activation and IFN production, and blood cells from these patients expressed a strong IFN and NF-kB transcriptional signature. Immune cells and TNF-stimulated dermal fibroblasts upregulated the inducible IKK protein (IKKi) that was stabilized by NEMO-Aex5, promoting type I IFN induction and antiviral responses. These data revealed how IKBKG mutations that lead to alternative splicing of skipping exon 5 cause a clinical phenotype we have named NEMO deleted exon 5 autoinflammatory syndrome (NDAS), distinct from the immune deficiency syndrome resulting from loss-offunction IKBKG mutations.
AIM: To demonstrate that administering heparanase inhibitor PI-88 at 160 mg/d is safe and promising in reducing hepatocellular carcinoma(HCC) recurrence for up to 3 year following curative resection. ...METHODS: A total of 143 patients(83.1% of the 172 participants in the phase Ⅱ study) participated in the follow-up study. Of these patients, 50 had received no treatment, 48 had received 160 mg/d PI-88, and 45 had received 250 mg/d PI-88 during the phase Ⅱ trial. Safety parameters and the following efficacy endpoints were investigated:(1) time to recurrence;(2) diseasefree survival; and(3) overall survival. RESULTS: PI-88 at 160 mg/d delayed the onset and frequency of HCC recurrence, and provided a clinically significant survival advantage for up to 3 years after treatment compared with those of the control group:(1) the recurrence-free rate increased from 50% to 63%, and(2) time to recurrence at the 36 th percentile was postponed by 78%. The efficacy of administering PI-88 at 250 mg/d was confounded by a high dropout rate(11 out of 54 patients). Additionally, subgroup analyses of patients with(1) multiple tumors or a single tumor ≥ 2 cm; and(2) hepatitis B or C revealed that administering PI-88 at 160 mg/d conferred the most significant survival advantage(56.8% improvement in disease-free survival, P = 0.045) for patients with both risk factors for recurrence. CONCLUSION: Administering PI-88 at 160 mg/d is a safe and well-tolerated dosage that may confer significant clinical benefits for patients with HCC.
