Health effects of air pollution, especially particulate matter (PM), have been widely investigated. However, most of the studies rely on few monitors located in urban areas for short-term ...assessments, or land use/dispersion modelling for long-term evaluations, again mostly in cities. Recently, the availability of finely resolved satellite data provides an opportunity to estimate daily concentrations of air pollutants over wide spatio-temporal domains. Italy lacks a robust and validated high resolution spatio-temporally resolved model of particulate matter. The complex topography and the air mixture from both natural and anthropogenic sources are great challenges difficult to be addressed. We combined finely resolved data on Aerosol Optical Depth (AOD) from the Multi-Angle Implementation of Atmospheric Correction (MAIAC) algorithm, ground-level PM10 measurements, land-use variables and meteorological parameters into a four-stage mixed model framework to derive estimates of daily PM10 concentrations at 1-km2 grid over Italy, for the years 2006–2012. We checked performance of our models by applying 10-fold cross-validation (CV) for each year. Our models displayed good fitting, with mean CV-R2=0.65 and little bias (average slope of predicted VS observed PM10=0.99). Out-of-sample predictions were more accurate in Northern Italy (Po valley) and large conurbations (e.g. Rome), for background monitoring stations, and in the winter season. Resulting concentration maps showed highest average PM10 levels in specific areas (Po river valley, main industrial and metropolitan areas) with decreasing trends over time. Our daily predictions of PM10 concentrations across the whole Italy will allow, for the first time, estimation of long-term and short-term effects of air pollution nationwide, even in areas lacking monitoring data.

Abstract Background As ablation and electroanatomical mapping technology evolve, workflows change to maximize the impact on procedural efficacy, safety, and efficiency. In a magnetic-primary ...navigation mode, optimization of the magnetic-impedance field links (VoXels) must be considered to provide accurate environments for anatomic modeling, mapping, and lesion placement. To achieve this accuracy and optimize speed of data collection, many operators have adopted the strategy of collecting VoXels using an ablation catheter prior to introduction of a multipolar mapping catheter. However, the impact of this workflow on procedural metrics has not yet been quantified in a multicenter, international cohort. Objective Assess the impact of optimizing the magnetic navigation field using an ablation catheter prior to collecting left atrial (LA) data using a high-density diagnostic tool on acute outcomes in de novo atrial fibrillation (AF) ablation procedures. Methods Acute observational data from 199 de novo pulmonary vein isolation (PVI) cases was prospectively collected from 56 operators at 42 centers in Europe, the US, and Australia. Procedural metrics, modeling workflow, ablation technology used, and acute outcomes were collected. Results Of 199 de novo AF (62.3% PAF) ablation procedures, VoXels were collected with an ablation catheter prior to LA map collection with a multipolar catheter in 65/199 (32.7%) cases; the remainder collected with multipolar catheter first. An irrigated, contact force sensing RF ablation catheter was used in all procedures. Mean procedure duration was similar in procedures in which VoXels were collected prior to map collection (VC) compared to those that did not (nVC) (107.0±46.8 min and 115.4±59.2 min, respectively (p=0.28)). Mean fluoroscopy time was significantly reduced in VC cases compared to nVC cases (3.0±4.5 min and 5.3±5.6 min, respectively (p=0.002)). Total RF duration was significantly reduced in VC cases compared to nVC cases (904±444s and 1063±632s, respectively (p=0.04)). There was also a significant reduction in fluid delivered through the ablation catheter in VC cases, 369±159mL, compared to nVC cases, 520±243mL (p<0.001). Acute success rates were similar between groups: 98.5% for VC cases and 99.2% for nVC cases (p=0.64). Conclusion In this international cohort, cases where VoXels were collected prior to map and model collection had significantly reduced fluoroscopy times, RF durations, and fluid volumes delivered through the ablation catheter, potentially as a result of improved model quality. Despite the introduction of an additional step to the workflow, there was a trend toward shorter procedures when VoXels were collected prior to map and model collection, although it did not achieve statistical significance. Increasing evidence suggests that iron-mediated oxidative stress might underlie the development of neurodegeneration in Friedreich's ataxia (FRDA), an autosomal recessive ataxia caused by decreased ...expression of frataxin, a protein implicated in iron metabolism. In this study, we demonstrate that, in fibroblasts of patients with FRDA, the cellular redox equilibrium is shifted toward more protein-bound glutathione. Furthermore, we found that actin is glutathionylated, probably as a result of the accumulation of reactive oxygen species, generated by iron overload in the disease. Indeed, high-pressure liquid chromatography analysis of control fibroblasts in vivo treated with FeSO4 showed a significant increase in the protein-bound/free GSH ratio, and Western blot analysis indicated a relevant rise in glutathionylation. Actin glutathionylation contributes to impaired microfilament organization in FRDA fibroblasts. Rhodamine phalloidin staining revealed a disarray of actin filaments and a reduced signal of F-actin fluorescence. The same hematoxylin/eosin-stained cells showed abnormalities in size and shape. When we treated FRDA fibroblasts with reduced glutathione, we obtained a complete rescue of cytoskeletal abnormalities and cell viability. Thus, we conclude that oxidative stress may induce actin glutathionylation and impairment of cytoskeletal functions in FRDA fibroblasts.

Premenopausal women who are HCV positive (HCV+) have failing ovarian function, which is likely to impact their fertility. Thus, we investigated the reproductive history, risk of infertility, and pregnancy outcomes in women of childbearing age who were HCV+.
Three different groups were studied: (1) Clinical cohort: 100 women who were HCV+ and also had chronic liver disease (CLD), age matched with 50 women who were HBV+ with CLD and with 100 healthy women; all women were consecutively observed in three gastroenterology units in hospitals in Italy; (2) 1,998 women who were HCV+ and enrolled in the Italian Platform for the Study of Viral Hepatitis Therapies (PITER); (3) 6,085 women, who were mono-infected with HCV, and 20,415 women, who were HCV−, from a large de-identified insurance database from the USA. Measurements: total fertility rate (TFR) defined as the average number of children that a woman would bear during her lifetime. To define the reproductive stage of each participant, levels of anti-Müllerian hormone (AMH) and 17β-estradiol were measured.
Clinical cohort: women who were either HCV+ or HBV+ had similar CLD severity and age at first pregnancy. Based on a multivariate analysis, women who were HCV+ had a higher risk of miscarriage than those who were HBV+ (odds ratio OR 6,905; 95% CI 1.771–26.926). Among women who were HCV+, incidence of miscarriage was correlated with median AMH level (1.0 ng/ml). Achieving a sustained virologic response (SVR) after antiviral treatment reduced the risk of miscarriage (OR 0.255; 95% CI 0.090–0.723). In the PITER-HCV cohort, miscarriage occurred in 42.0% of women (44.6% had multiple miscarriages). TFR for women who were HCV+ and between 15 and 49 years of age was 0.7 vs. 1.37 of Italian population of the same age range. In the US cohort: compared with women who were HCV−, women who were HCV+ positive were significantly more likely to have infertility (OR 2.439; 95% CI 2.130–2.794), premature birth (OR 1.34; 95% CI 1.060–1.690), gestational diabetes (OR 1.24; 95% CI 1.020–1.510), and pre-eclampsia (OR 1.206; 95% CI 0.935–1.556), and were less likely to have a live birth (OR 0.754; 95% CI 0.622–0.913).
Ovarian senescence in women of childbearing age who are HCV+ is associated with a lower chance of live birth, greater risk of infertility, gestational diabetes, pre-eclampsia and miscarriage. Such risks could be positively influenced by successful HCV cure.
Most new cases of HCV infection are among people who inject drugs, many of whom are young women in their childbearing years. Women of reproductive age who are HCV+ display markers of ovarian senescence. This is associated with an increased burden in terms of infertility and adverse pregnancy outcomes, including stillbirth, miscarriage, fewer live births, and gestational diabetes. Early viral suppression with therapy is likely to mitigate these risks.
