Objective
There are currently no studies on the role of vitamin D receptor (VDR) levels as a cause of or risk factor for sepsis. We aimed to establish the association between VDR levels and 28-day ...mortality in critically ill patients with sepsis.
Methods
This prospective cross-sectional observational study included 148 patients diagnosed with sepsis who were treated in the intensive care unit. We measured VDR levels, laboratory characteristics, and health scores and related them to survival.
Results
The 148 patients included 96 survivors and 52 non-survivors, with VDR levels of 1.92 and 1.36 ng/mL, respectively. Baseline VDR was a significant predictor of 28-day mortality, with an area under the curve of 0.778. A low VDR level was significantly associated with lower overall survival in patients with sepsis according to Kaplan–Meier curve analysis. VDR levels were also negatively correlated with lactate, C-reactive protein, acute physiological and clinical health evaluation (APACHE) II and sequential organ failure assessment (SOFA) scores, and disease severity.
Conclusions
VDR levels were associated with high 28-day mortality and negatively correlated with lactate, C-reactive protein, APACHE II and SOFA scores, and disease severity in patients with sepsis. VDR levels can predict poor outcomes in patients with sepsis.
The lactate/albumin (L/A) ratio correlates with Acute Physiology and Chronic Health Evaluation 2 (APACHE-2) and Sequential Organ Failure Assessment (SOFA) scores. This study examined whether the L/A ...ratio has prognostic value in a larger group of critically ill (adult) patients admitted to an intensive care unit (ICU) due to pneumosepsis.This retrospective study analyzed the data of 273 patients with pneumosepsis admitted to the Internal Medicine ICU of Adana City Training and Research Hospital between 2018 and 2020. Patients diagnosed with pneumosepsis were included in the study. The data were obtained from the hospital system. Patients who had cancer, who were pregnant, and patients whose necessary data could not obtain for the study were excluded from the study.The L/A ratio was superior to lactate or albumin alone as a predictor of mortality. Furthermore, this result was valid for patients with kidney and hepatic dysfunction. A correlation occurred between the L/A ratio and APACHE-2 and SOFA scores in patients with pneumosepsis.The L/A ratio can be an independent predictor of mortality in patients with pneumosepsis and patients with pneumosepsis with renal and hepatic dysfunction. The L/A ratio correlated positively with lactate levels and APACHE-2 and SOFA scores but negatively with albumin levels.
Purpose – In this study, it is aimed to investigate and detect fraudulent acts on the basis of fraud auditing and / or investigative accountancy. Design/methodology/approach – Within the framework of ...fraud auditing / investigative accountancy, which is one of the sub-dimensions of the forensic accounting profession, a case analysis was carried out to investigate a fraud case in an enterprise, to identify the process and to suggest solutions. Findings – In the analyzed fraud case, the procurement commission formed within the company exists on paper, all purchasing transaction authority belongs to the purchasing manager, in this context, the principle of separation of duties is violated, there are no ethical warning lines in the enterprise and this situation prevents the transmission of suspicious events to the management, cost analysis is not effective. It has been concluded that the purchasing manager and head chef of the enterprise agreed with a supplier enterprise where the enterprise works and purchased goods with high unit prices in order to gain personal gain in return and left the enterprise faced with significant losses. Discussion – As a result of the emergence of forensic accounting expertise as a popular and needed expertise, many countries have carried out the necessary legal regulation and certification activities and made the necessary updates in their higher education curricula. Subject in terms Turkey is a fact that in our country there is no legislation on forensic accounting and forensic accounting expertise. Considering the profession's contribution to the field of accounting and auditing, it will be important to carry out the necessary work as soon as possible and to strengthen it with legal regulations.

The mortality rate of patients diagnosed with sepsis is high. To date, many markers in sepsis patients have been studied to diagnose, determine their prognosis, and contribute to treatment. These ...studies were conducted in both experimental and clinical settings, but clinical trials remain limited. Therefore, more well-planned clinical studies are needed in patients with sepsis.The current study aimed to examine the prognostic role of signal peptide-CUB-epidermal growth factor-like domain-containing protein 1 (SCUBE-1) in sepsis and sepsis-related mortality. We also wanted to study its relationship with inflammatory markers and scoring systems.This prospective, cross-sectional, observational study included a total of 187 sepsis cases treated in the intensive care unit. Venous samples were obtained after diagnosis. The patients were separated into 2 groups: (1) the survivor group who were discharged or transferred within 28 days of the first diagnosis and (2) the nonsurvivor group who died within 28 days of the first diagnosis.The SCUBE-1, C-reactive protein, procalcitonin, creatinine, lactate values, acute physiology and chronic health evaluation 2, sequential organ failure assessment scores were significantly higher in the survivor group, and platelets were higher in the survivor group. In addition, SCUBE-1 positively correlated with the inflammatory markers C-reactive protein, lactate, sequential organ failure assessment, and acute physiology and chronic health evaluation 2. Additionally, the SCUBE-1 value predicts 28-day mortality, and the optimal cutoff value for predicting mortality is 4,73 pg/mL.Sepsis is a disease with high mortality. SCUBE-1 can be used as a new prognostic factor for sepsis patients.
Determining the smoking rates and nicotine dependency levels of soldiers can be a guide in smoking cessation interventions and developing a policy for smoking prevention for soldiers serving in ...military units. The cross-sectional study was conducted with a total of 313 soldiers stationed in gendarmerie units in a city center in the Central Anatolian Region of Turkey. Data were collected between 01.06.2021 and 31.07.2021 by face-to-face interview technique using Personal Information Form and Fagerström Nicotine Addiction Test. It was determined that 54.6% of the participants smoked, 87.1% had low-moderate nicotine addictions, and 94.9% were smokers in their friends' circle. It was revealed that the prevalence of smoking among the participants was high, and the nicotine addiction levels of the participants working in the "Prison" unit were higher.

In the last couple of years, more than 3.6 million Syrian refugees have been hosted under the “temporary protection” scheme in Turkey. Despite these high numbers Turkey did not have a centralized ...refugee settlement and integration policy. As a result, various stakeholders including local governments have played critical roles in providing refugee assistance services. This research looks at the role of local governments in delivering services evolving from emergency response to local integration. This article argues that this role with respect to the United Nations’ Global Compact on Refugees (GCR) has to be further strengthened. The data for this research were collected through a comprehensive study based on interviews and surveys carried out in İstanbul with local authorities, İstanbul Metropolitan Municipality, and its 39 district municipalities in 2016‐2017. Although this research has its focus on İstanbul as the selected case study, the findings can reveal conclusions relevant to global implications and perspectives.

Cytotoxic activity-guided isolation studies on the underground parts of Valeriana sisymbriifolia Vahl. led to the isolation of 12 secondary metabolites including two undescribed iridoids, ...sisymbriifolivaltrate and sisymbriifolioside, and two unreported sesquiterpene lactones, sisymbriifolins A and B. Chemical structures of the isolates were established by extensive 1D and 2D NMR analyses as well as HR-ESI-MS. The in vitro cytotoxic activities of the extract, sub-fractions and isolates on lung (A549), breast (MCF7), gastric (HGC27) and prostate (PC3) cancer cell lines were evaluated by MTS assay. Sisymbriifolivaltrate, didrovaltrate, valtrate, 7-homovaltrate and 1-α-acevaltrate exhibited promising cytotoxic activity on MCF7 cell line with IC50 values ranging from 2.5 to 12.3 μM, while valtrate demonstrated the best cytotoxicity against A549 cells with the IC50 value of 7.5 μM. Valtrate and 7-homovaltrate were found to exert noteworthy cytotoxicity towards HGC27 cell line (IC50 values: 2.3 and 3.7 μM, respectively), whereas valtrate, 7-homovaltrate and 1-α-acevaltrate (IC50 values: 2.3–9.7 μM) were found to be potent cytotoxic against PC3 cells. Among the tested compounds, particularly valepotriate-type iridoids were found to be the main cytotoxic principles of V. sisymbriifolia.

Cystic fibrosis (CF) is one of the most common autosomal recessive and multisystemic diseases. CF affects many systems. One of these systems is the endocrine and exocrine functions of the pancreas, ...causing cystic fibrosis‐related diabetes, which is extremely complex and has unique pathogenesis. Maturity‐onset diabetes of the young (MODY) is a rare type of diabetes with autosomal dominant inheritance and is not expected in patients with CF. In this study, we present MODY due to a novel glucokinase gene mutation, which is an unexpected form of diabetes in patients with CF. This is previously unreported in the literature.

59 male army officers made up the research team. A follow-up study of strength endurance (push-up and sit-up) test was conducted in the gym. The exam took two minutes to complete, and each application was tested separately. In both 2004 and 2019, persons with genotype ID had the best mean sit-up and push-up outcomes, followed by participants with genotype DD, and finally participants with genotype II (

0.05). Compared to the original rates in 2004, all genotype groups showed a significant reduction in push-up and sit-up scores in the test.
Conclusion
The findings of this study may reveal if strength and lifestyle choices affect the metabolic implications of the genetic polymorphism in the body. Particular varieties actuated by genes, on either hand, don’t result in significant improvements without any changes in individuals’ practices or ways of living, as per the conclusions.

In this paper, heat transfer coefficients and critical heat fluxes of the R134a fluid in two-phase flow was studied in a 27-channel, 421 μm hydraulic diameter and 40 mm long microchannel experimental ...set-up. Data were collected at constant mass flux of 1000 kg m−2 s−1, saturation inlet temperature range of 20–28 °C and at inlet vapor quality range of 0.01–0.20. The heat flux was initiated at 250 kW m−2 and gradually increased until reaching the critical heat flux (CHF). The effect of inlet vapor quality, saturation inlet temperature and heat flux on heat transfer coefficient and critical heat flux were investigated in detail and presented graphically. It can be observed that the CHF decreases with the increasing of saturation inlet temperature and inlet vapor quality. However, the heat transfer coefficient at the CHF point increased with the increasing of saturation temperature and the heat transfer coefficient at the CHF point was decreased with the increasing of vapor quality. The test results from the correlations, suggested in well-known articles, were compared with the experimental data, where the Kosaz et al. proposed correlation was showing well fit with the experimental data having absolute error of 9.86%. In this investigation, the new correlations for the heat transfer coefficient for the heat transfer coefficient and the critical heat flux have been proposed by taking into account the effect of the vapor quality.
