Although the ideal technique is still controversial, mitral valve reconstruction for mitral insufficiency usually includes an annuloplasty. From August 1985 to June 1993, 126 cases of pure, acquired ...mitral insufficiency were repaired by means of a posterior annuloplasty. Whatever the etiology, all types of mitral insufficiency allowing a mitral reconstruction were included. The annuloplasty, performed with a flexible linear reducer, was associated with valvular (62%) and subvalvular (11%) repairs. An associated surgical procedure was necessary in 62% of the patients. Operative mortality was 2.4%, and follow-up (mean 29 months) was complete for all survivors. Five-year survival was 90%. Five-year complication-free rate for emboli was 91%. Only one patient at 12 months underwent reoperation to treat recurrent mitral insufficiency. Ninety-seven patients were in New York Heart Association Class I or II. Follow-up echocardiographic studies on 75% of eligible patients showed a free rate for significant regurgitation of 99%. Mean valve area was estimated at 2.88 +/- 0.85 cm2. These findings suggest that the flexible linear reducer seems to be a reliable device and a valid alternative for annuloplasty.
We examined changes in the haematological, metabolic, immunological, hormonal, and psychological fields using selected variables in 20 professional soccer players, over the course of a competitive ...season. The team performance was assessed by computing the winning percentage. A symptom checklist was used to assess the severity of upper respiratory tract infections. A high-intensity training programme induced a significant increase in cortisol and uric acid concentrations. Despite lower glutamine concentrations than the normal range throughout the study, infection occurred only in two of the soccer players. Moreover, the levels of immunological factors IgA, IgG, and IgM, and the haematological parameters were unaltered. Subsequent decreased performance coincided with changes in specific mood states of the team. Our results show some alterations on the metabolic, hormonal, and psychological variables over the five fields studied, suggesting that combined psychological and physiological changes during training are of primary interest to monitor the training stress in relation to performance in team sport.

In this study, we examined the effect of rapidly increased training volume and intensity on hormonal responses (salivary cortisol C and urinary dehydroepiandrosterone sulphate DHEA-S) and ...recovery-stress state perceived by 12 female cyclists. Over the 4-day experimental period, there was an average increase in training load of approximately 122% compared with that during the previous 12 days. Scores on subscales of the Recovery Stress Questionnaire for Athletes increased for the somatic component of stress (Fatigue, Emotional Stress and Social Stress; P < 0.05) and decreased the factor indicating recovery (General Well Being; P < 0.05) after the heavy training period. The training programme increased resting concentrations of salivary cortisol (P < 0.05) and decreased the DHEA-S/C ratio (P < 0.05). The increase in training load of cyclists was correlated with this hormonal ratio (r = −0.48, P < 0.05). Changes in resting cortisol concentration as a result of heavy training stress were positively related to the change in Physical Complaints (r = 0.69, P < 0.01). Negative relationships were also found between changes in the DHEA-S/C ratio and changes in the somatic component of stress. The present results suggest that there is a dose-response relationship between increased training load, resting DHEA-S/C ratio and subjective assessment of stress and recovery, implying that this ratio could be used as an indicator of training status in female athletes.

Par ailleurs, les troubles de l’image corporelle et des conduites alimentaires constituent aujourd’hui de véritables enjeux de santé publique. We report a case of IgA-kappa multiple myeloma in a 68-year-old woman that was revealed by concomitant pleural and pericardial effusion. These effusions were found to be caused by myeloma and were ...verified by cytological examination of the pleural fluid and pericardial biopsy. The patient had neither osteolytic lesions nor Bence-Jones proteinuria. After a pericardiocentesis, her condition improved with a melphalan and prednisolone treatment. As far as we know, such a phenomenon is rare and has never been reported yet as a way of diagnosing multiple myeloma.
