Bronchoalveolar lavage (BAL) is frequently performed in patients with suspected ocular sarcoidosis. This study describes the immunogenetical, immunological, radiological, and functional features of a ...subclinical alveolar lymphocytosis unrelated to sarcoidosis in patients with chronic uveitis. Two hundred and ten patients with chronic uveitis of unknown origin, who underwent fiber-optic bronchoscopy with BAL as part of a prospective protocol over a three-year period, were evaluable for retrospective analysis. Sixty-five patients had alveolar lymphocytosis: Sarcoidosis was diagnosed in 13 (6%) patients, whereas alveolar lymphocytosis was considered unrelated to sarcoidosis in 52 (25%). Alveolar lymphocytosis unrelated to sarcoidosis was not associated with radiologically detectable interstitial lung disease or pulmonary function impairment. CD4/CD8 lymphocyte ratio was 3.7 +/- 3.0. Total cell count, total lymphocyte, and CD4 lymphocyte percentage were significantly lower when compared with sarcoidosis-related alveolitis (129,000 +/- 80,000 vs. 218,000 +/- 117,000, p <0.05; 33.1% +/- 13.2 vs. 39.7% +/- 13.2, p <0.05; and 54.3% +/- 18.2 vs. 65.4% +/- 10.1, p <0.05, respectively). Patients with alveolar lymphocytosis unrelated to sarcoidosis were older (47.8 +/- 17.7 years vs. 42.7 +/- 14.2 years, p <0.05) and more likely to carry the HLA-B51 allele (19.7% vs. 7.1%, p <0.01) than patients with chronic uveitis without alveolar lymphocytosis. They did not appear to be at risk of developing clinically apparent interstitial lung disease on followup. We conclude that alveolar lymphocytosis is frequently observed in patients with chronic uveitis. It is generally unrelated to sarcoidosis and may then be associated with a distinctive immunogenetic phenotype.

Ragweed in France is a rather recent pollen which causes strong allergies on the populations concerned. This pollen, mainly located in the area Rhone-Alpes, extend more and more on totality of ...France. Indeed, the pollinic data highlight the presence of ragweed on all the territory with very low, as for Brittany, to several hundreds grains in the area of Rhone-Alpes. So four towns of this area have followed closely the extend of this pollen during the year 2002 through five pollen-traps (2 for the town of Lyon). The trap of Lyon I (Gerland) made it possible to follow temporal the extend of ragweed from 1987 to 2002 and the increase of the number of grains as that of the number of days with a significant allergic risk. The study shows also the differences in quantity of pollens, the differences between the daily allergic risks of the five sites, as well as the differences, between the cities, of the number of days when the allergic risk is significant. It is necessary that pollen of ragweed can be followed closely because of its strong allergenic capacity, in the area of Rhone-Alpes, where the plant prevails mainly, but also on the totality of the own territory, this to supervise its evolution and the speed of proliferation of the plant.

Primary intrapericardial thymoma is an unusual localization. We report a case of a patient with an isolated and primary pericardial thymoma. This 72-year-old woman presented with dyspnea, dysphony ...and myalgia. The radiological evaluation revealed an intrapericardial mass. Surgical exploration showed a hemorrhagic and infiltrative tumor in the pericardial sac, while the mediastinum was free of tumor. Surgical biopsies and, later, an ablation of pericardial mass were done. The tumor was a thymoma, composed of large epithelial cells and immature T lymphocytes and was classified B2 according to the World Health Organization classification (1999). Clinically, a myasthenia gravis was revealed. We discuss the few cases reported in the literature.

We estimate the incidence of multiply-imaged AGNs among the optical counterparts of X-ray selected point-like sources in the XXL field. We also derive the expected statistical properties of this ...sample, such as the redshift distribution of the lensed sources and of the deflectors that lead to the formation of multiple images, modelling the deflectors using both spherical (SIS) and ellipsoidal (SIE) singular isothermal mass distributions. We further assume that the XXL survey sample has the same overall properties as the smaller XMM-COSMOS sample restricted to the same flux limits and taking into account the detection probability of the XXL survey. Among the X-ray sources with a flux in the 0.5-2 keV band larger than 3.0x10\(^{-15}\) erg cm\(^{-2}\) s\(^{-1}\) and with optical counterparts brighter than an r-band magnitude of 25, we expect ~20 multiply-imaged sources. Out of these, ~16 should be detected if the search is made among the seeing-limited images of the X-ray AGN optical counterparts and only one of them should be composed of more than two lensed images. Finally, we study the impact of the cosmological model on the expected fraction of lensed sources.
This work is part of a series of studies focusing on the environment and the properties of the X-ray selected active galactic nuclei (AGN) population from the XXL survey. The present survey, given ...its large area, continuity, extensive multiwavelength coverage, and large-scale structure information, is ideal for this kind of study. Here, we focus on the XXL-South (XXL-S) field. Our main aim is to study the environment of the various types of X-ray selected AGN and investigate its possible role in AGN triggering and evolution. We studied the large-scale (>1 Mpc) environment up to redshift z=1 using the nearest neighbour distance method to compare various pairs of AGN types. We also investigated the small-scale environment (<0.4 Mpc) by calculating the local overdensities of optical galaxies. In addition, we built a catalogue of AGN concentrations with two or more members using the hierarchical clustering method and we correlated them with the X-ray galaxy clusters detected in the XXL survey. It is found that radio detected X-ray sources are more obscured than non-radio ones, though not all radio sources are obscured AGN. We did not find any significant differences in the large-scale clustering between luminous and faint X-ray AGN, or between obscured and unobscured ones, or between radio and non-radio sources. At local scales (<0.4 Mpc), AGN typically reside in overdense regions, compared to non-AGN; however, no differences were found between the various types of AGN. A majority of AGN concentrations with two or more members are found in the neighbourhood of X-ray galaxy clusters within <25-45 Mpc. Our results suggest that X-ray AGN are typically located in supercluster filaments, but they are also found in over- and underdense regions.
A 46-year-old man, without remarkable past medical history, had a perianal ulcer that appeared spontaneously two months before presentation. At admission, the ulcer was painless, measuring 4 to 5 cm ...in diameter and showing detachment and a slightly papillomatous aspect at the edge but without induration of the base. Microscopic examination revealed cutaneous ulceration with a well-developed inflammatory response, a few small vessels with intraluminal thrombosis or necrotizing walls, and isolated microscopic granulomata. No infectious disease was detected. The diagnosis of Wegener's granulomatosis was made six months later, when the disease was clinically evident in three principal sites: upper airways, lung, and kidney. At that time, serum antineutrophil cytoplasmic autoantibodies were detected with indirect immunofluorescence microscopy. There has been an excellent response to immunosuppressive therapy. Review of the literature revealed no similar reports.
In HIV-infected patients, acute respiratory failure is usually due to infectious pneumonia. In this report, we describe two cases of acute respiratory failure in HIV patients with clinical ...presentation suggesting infectious pneumonia. In both cases, the clinical condition deteriorated and death occurred after several days despite therapy. In both cases bronchial biopsies confirmed bronchogenic carcinoma responsible for carcinomatous lymphangitis.

We present a catalogue containing the redshifts of 3,660 X-ray selected targets in the XXL southern field. The redshifts were obtained with the AAOmega spectrograph and 2dF fibre positioner on the ...Anglo-Australian Telescope. The catalogue contains 1,515 broad line AGN, 528 stars, and redshifts for 41 out of the 49 brightest X-ray selected clusters in the XXL southern field.
