To compare the clinical outcome and restenosis incidence of patients who underwent carotid endarterectomy with patch closure (CEAP) on one side and carotid eversion endarterectomy (CEE) on the other.
...Although a few investigators have compared the results of CEAP versus CEE, no reports have compared the outcome of CEAP versus CEE in the same patient.
Eighty-six patients were randomly selected for sequential surgical treatment involving either CEAP/CEE or CEE/CEAP. All patients underwent postoperative duplex ultrasound study and clinical follow-up at 1, 6, and 12 months and every year thereafter. Various factors were analyzed to ascertain any association with restenosis, and Kaplan-Meier analysis was used to estimate the risk of restenosis.
Demographic and clinical data were similar in the CEAP and CEE groups. The selective shunting rate was statistically higher in the CEAP group. There were no perioperative deaths. Although the incidence of perioperative ipsilateral stroke was not significant, CEAP patients had a rate of combined transient ischemic attacks and strokes that approached statistical significance. The mean follow-up was 40 months. CEAP patients had a significantly higher incidence of restenosis and combined occlusive events and restenoses. Kaplan-Meier analysis showed that CEE had a significantly better cumulative patency rate than CEAP and that freedom from restenoses at 24 and 36 months was 87% and 83% for CEAP and 98% and 98% for CEE, respectively.
CEE is less likely to cause perioperative neurologic complications and restenoses than CEAP. The significantly higher rate of unilateral recurrence suggests that local factors play a more important role than systemic factors in the occurrence of restenosis.

Clinical Case
We report the case of a 84–year–old female patient suffering from post–infarct heart disease with severe systolic dysfunction, wearing a dual–chamber defibrillator in secondary ...prevention because she had previously presented an episode of monomorphic ventricular tachycardia. After the implant, the patient was enrolled in the remote monitoring service of the device and constantly followed by our team. After a few months, at the remote control we found repeated episodes of very short duration of an alarm recognized by the device as ventricular arrhythmia in the VF window. Contrary to the arrhythmic interpretation of the device, the analysis of the EGM by the medical and nursing staff highlighted an electromagnetic interference defined by the type of signal, the presence of the same in all channels and by the lack of correlation with the underlying heart rhythm. The sensing of the ventricular lead was 5–6 mV, at the lower limits but stable over time, the other parameters were optimal. The patient was promptly contacted and questioned about the circumstances and symptoms: she was asymptomatic and we found that the interference came in conjunction with the use of a cordless telephone. The alarm was related to the electromagnetic field generated by the phone and the woman was asked to stop using the device with subsequent absence of device alarms. Electromagnetic interference in defibrillators can be a cause of potentially serious inappropriate therapies (ATP, pacing and shock), the analysis and correct interpretation of the EGM by the clinician is essential for the prevention of the same. Thanks to the efficiency of the remote monitoring service, the risk of serious consequences for our patient was avoided. Subsequently the lead was repositioned due to a worsening of the sensing, but even before the re–intervention there was no longer any false alarm thanks to the absence of the environmental source of interference. The remote monitoring service applied to implantable defibrillation systems is a valuable tool for identifying lead problems or, as in this case, environmental problems, and toprevent, thanks to rapid intervention, further potentially serious complications.
Abdominal aortic aneurysm (AAA) is an age related disease, so the aging of the population has meant to more elderly people undergoing AAA repair. The authors conducted a systematic review of the ...literature to analyze the perioperative mortality and complication rates and long-term survival of elderly people after AAA repair. The literature was searched using the Embase, Cochrane library and Medline databases as at May 2008. All studies reporting on the perioperative and long-term outcomes of patients aged 80 years or more undergoing elective open (OAR) or endovascular AAA repair (EVAR) were considered. The risk of perioperative mortality and morbidity were calculated using the odds ratio (OR), with 95% confidence intervals (CIs), and the chi(2) test. Thirty-five studies on OAR, five on EVAR and four on both were considered. In the OAR group, the mortality rate (38 studies, 1793 patients) was 5.6% (95% CI, 4.5 to 6.7) and the morbidity rate (18 studies, 725 patients) was 26.9% (95% CI, 23.7 to 30.1). Twenty studies reported a median 5-year survival rate of 60% (range, 14% to 86%). In the EVAR group, the mortality rate (9 studies, 1159 patients) was 4.5% (95% CI, 3.3 to 5.7) and the morbidity rate (8 studies, 1078 patients) was 16.5% (95% CI, 14.3 to 18.7). The follow-up data covered less than 5 years in five studies. Although the perioperative death rate was higher after OAR than after EVAR, the difference was not statistically significant (P=0.170; 95% CI, 0.90 to 1.78). The major systemic morbidity rate was significantly higher after OAR (P<0.001; 95% CI, 1.47 to 2.34). Although the perioperative mortality rate was comparable between the two surgical procedures, the marked selection bias cannot be ignored and may well mean that the mortality rates are actually higher for both procedures. Although the mid- and long-term survival rates after OAR and EVAR could seem acceptable, more information is needed on the long-term outcome after EVAR in larger samples in order to assess the durability of this less invasive procedure.
Whether surgically correcting symptomatic carotid elongation with coiling or kinking in the absence of an atherosclerotic lesion of the carotid bifurcation (isolated elongation) is effective in ...preventing stroke remains a controversial issue. The hypothesis behind this study was that surgical correction of symptomatic isolated carotid elongation with coiling or kinking could yield better results, in terms of stroke prevention and freedom from late stroke or carotid occlusion, than medical treatment.
We conducted a prospective clinical study randomly assigning symptomatic patients with isolated carotid elongation to undergo either elective surgery or medical treatment, with surgery reserved for any new onset or worsening of symptoms. The follow-up ranged from 1 month to 10 years (median, 5.9; mean, 6.2 years) and was obtained for all patients. The study end points were perioperative (30-day) stroke and mortality, late stroke, and stroke-related death and late carotid occlusions.
Ninety-two patients were randomly assigned for surgery and 90 for medical treatment. Overall, 139 carotid surgical corrections were performed in 129 patients. All 92 patients in the surgical arm had an elective operation; 10 of these patients later developed symptoms on the opposite side (7 hemispheric and 3 retinal transient ischemic attacks) and had contralateral internal carotid artery surgery. An additional 37 patients (41.1%) randomly assigned to medical treatment crossed over to the surgical group within a mean of 16.8 months after randomization due to new hemispheric symptoms or worsening nonhemispheric complaints. There were no perioperative strokes or deaths. The incidence of late hemispheric and retinal transient ischemic attacks was significantly lower in the surgical than in the medical group, respectively, 7.6% (7 of 92) vs 21.1% (19 of 90) (P = .01) and 3.2% (3 of 92) vs 12.2% (11 of 90) (P = .03). Late strokes, 2 (2.2%) of which were fatal, occurred only in the medical group (6 of 90, 6.6%; P = .01). Late carotid occlusions also developed only in the medical group (5 of 90, 5.5%; P = .02). All surgically treated carotid elongations were analyzed histologically and 78 (56.%) of 139 showed atypical and typical patterns of fibromuscular dysplasia.
The overall results of this trial indicate that surgical correction of symptomatic isolated carotid elongations with coiling or kinking is better for stroke prevention than medical treatment.

Background: The aim of this article was to analyze the perioperative mortality and stroke risk rates and late benefits of carotid endarterectomy (CE) contralateral to an occluded internal carotid ...artery (ICA), on the basis of our surgical experience from July 1990 to June 1996.
Methods: In 57 (14.7%) of 336 patients undergoing 388 CEs, the contralateral ICA was occluded (group I). All operations were performed under general anesthesia with selective shunting based on electroencephalographic criteria. Shunting was used in 36 (63.1%) of 57 revascularizations in group I and 47 (14.2%) of 331 operations performed on the remaining 279 patients with patent contralateral ICAs (group II) (
p < 0.001).
Results: Perioperative strokes occurred in two patients (3.5%) in group I and three patients (1%) in group II (difference not significant). The only perioperative death, which occurred in one patient (1.7%) in group I, was the result of a perioperative stroke; two patients (0.7%) in group II died within 30 days of operation (difference not significant). Life-table cumulative stroke-free rates at 1, 3, and 5 years were 95%, 95%, and 95% in group I and 98.8%, 98.2%, and 98.2% in group II, respectively (
p = 0.272). Life-table cumulative survival rates at 1, 3, and 5 years were 97.5%, 94.2%, and 78.1% in group I and 99.2%, 94.8%, and 71.7% in group II, respectively (
p = 0.306).
Conclusions: The results of this analysis indicate that CE contralateral to an occluded ICA can be performed with acceptable perioperative mortality and stroke risk rates and late stroke-free and survival rates comparable to those seen in patients without contralateral ICA occlusion who have undergone operation. Nevertheless, we think it is misleading to imply that the risks of operating on the two groups are the same. Moreover, because no late stroke-related death occurred in patients with contralateral ICA occlusion, it would appear that superior late stroke-free rates did not translate into a prolonged Survival Advantage.
