The diagnosis, staging and therapeutic monitoring of lung cancer were amongst the first applications for which the utility of FDG PET was documented and FDG PET/CT is now a routine diagnostic tool ...for clinical decision-making. As well as having high sensitivity for detection of disease sites, which provides critical information about stage, the intensity of uptake provides deeper biological characterization, while the burden of disease also has potential clinical significance. These disease characteristics can easily be quantified on delayed whole-body imaging as the maximum standardized uptake value (SUVmax) and metabolic tumor volume (MTV), respectively. There have been significant efforts to harmonize the measurement of these features, particularly within the context of clinical trials. Nevertheless, however calculated, in general, a high SUVmax and large MTV have been shown to have an adverse prognostic significance. Nevertheless, the use of these parameters in the interpretation and reporting of clinical scans remains inconsistent and somewhat controversial. This review details the current status of semi-quantitative FDG PET/CT in the evaluation of lung cancer.

Recent advances in imaging, use of prognostic indices, and molecular profiling techniques have the potential to improve disease characterization and outcomes in lymphoma. International trials are ...under way to test image-based response–adapted treatment guided by early interim positron emission tomography (PET)–computed tomography (CT). Progress in imaging is influencing trial design and affecting clinical practice. In particular, a five-point scale to grade response using PET-CT, which can be adapted to suit requirements for early- and late-response assessment with good interobserver agreement, is becoming widely used both in practice- and response-adapted trials. A workshop held at the 11th International Conference on Malignant Lymphomas (ICML) in 2011 concluded that revision to current staging and response criteria was timely.
An imaging working group composed of representatives from major international cooperative groups was asked to review the literature, share knowledge about research in progress, and identify key areas for research pertaining to imaging and lymphoma.
A working paper was circulated for comment and presented at the Fourth International Workshop on PET in Lymphoma in Menton, France, and the 12th ICML in Lugano, Switzerland, to update the International Harmonisation Project guidance regarding PET. Recommendations were made to optimize the use of PET-CT in staging and response assessment of lymphoma, including qualitative and quantitative methods.
This article comprises the consensus reached to update guidance on the use of PET-CT for staging and response assessment for 18Ffluorodeoxyglucose-avid lymphomas in clinical practice and late-phase trials.
This paper follows the immunotherapy symposium held during the European Association of Nuclear Medicine (EANM) 2017 Annual Congress. The biological basis of the immune checkpoint inhibitors and the ...drugs most frequently used for the treatment of solid tumours are reviewed. The issues of pseudoprogression (frequency, timeline), hyperprogression and immune-related side effects are discussed, as well as their implications for patient management. A review of the recent literature on the use of FDG PET for assessment of immunotherapy is presented, and recommendations are provided for assessing tumour response and reporting immune-related side effects with FDG PET based on published data and experts’ experience. Representative clinical cases are also discussed.

"Total-body" and ultra-extended field-of-view PET/CT scanners are now available commercially with great enthusiasm for their potential in both streamlining clinical practice and providing unique ...research opportunities. Accordingly, many groups are rushing to implement this technology. For early adopters, the challenges of these systems compared with more standard PET/CT systems have been significant. In this guide, aspects that need to be considered in planning installation of one of these scanners are discussed. These include financing, space, structural engineering, power supply, chilled water and environmental controls to manage heat loads, IT infrastructure and data storage, radiation safety and radiopharmaceutical procurement, staffing levels, patient handling logistics and imaging protocol redesign to leverage the superior sensitivity of these scanners, and marketing. It is a daunting but worthwhile endeavor in the author's opinion but needs a great team and the ability to bring in the appropriate expertise at the appropriate time.
Purpose
Diverse radionuclide imaging techniques are available for the diagnosis, staging, and follow-up of phaeochromocytoma and paraganglioma (PPGL). Beyond their ability to detect and localise the ...disease, these imaging approaches variably characterise these tumours at the cellular and molecular levels and can guide therapy. Here we present updated guidelines jointly approved by the EANM and SNMMI for assisting nuclear medicine practitioners in not only the selection and performance of currently available single-photon emission computed tomography and positron emission tomography procedures, but also the interpretation and reporting of the results.
Methods
Guidelines from related fields and relevant literature have been considered in consultation with leading experts involved in the management of PPGL. The provided information should be applied according to local laws and regulations as well as the availability of various radiopharmaceuticals.

It also has limited utility in selecting the optimal type of immunotherapy. As knowledge about immune contexture increases, new targets that may be amenable to imaging are being defined. These exciting advances, coupled with increasingly sophisticated methods for generating radiopharmaceuticals, provide the potential for either replacing or complementing
F-FDG PET/CT in the selection and monitoring of immunotherapy. Approaches include imaging specific characteristics of immune cell infiltrates or aspects of the tumor microenvironment that are known to be associated with suppression of the innate and adaptive immune response. Following a large body of preclinical work, promising agents that are entering into early clinical evaluation are discussed. We suggest a speculative algorithm as to how these might be used in routine practice, subject to validation in clinical trials.
Abstract
Purpose
Treatment options for unresectable paraganglioma (PGL)/pheochromocytoma (PCC), especially with uncontrolled secondary hypertension (HTN), are limited. Preliminary studies with ...peptide receptor radionuclide therapy (PRRT) suggest efficacy, but data on HTN control and survival are lacking. We assessed PRRT outcomes in such patients from two referral centers.
Methods
Twenty consecutive patients (13 men; age range, 21 to 77 years) with high somatostatin receptor (SSTR) expression treated with 177Lu-DOTA-octreotate, nine with radiosensitizing chemotherapy, were retrospectively reviewed. Median cumulative activity was 22 GBq (median 4 cycles). Fourteen patients were treated for uncontrolled HTN and six for progressive or symptomatic metastatic disease or local recurrence.
Results
Three months after PRRT, 8 of 14 patients treated for HTN required reduced medication doses, 5 had no change in anti-HTN doses, and 1 was lost to follow-up. Eighty-six percent had serum chromogranin-A reduction. Of the entire cohort, 36% had disease regression (29% partial and 7% minor response) on computed tomography, with stable findings in 50%. Three other patients had bony disease evaluable only on SSTR imaging (2 partial response and 1 stable). Median progression-free survival was 39 months; median overall survival was not reached (5 deaths; median follow-up, 28 months). Four patients had grade 3 lymphopenia; 2 had grade 3 thrombocytopenia. Renal impairment in 2 patients was attributed to underlying disease processes.
Conclusions
PRRT achieves worthwhile clinical and biochemical responses with low toxicity and encouraging survival in PGL/PCC. Because PRRT has logistic and radiation-safety advantages compared to 131I-MIBG therapy, further prospective evaluation is warranted.
This study showed favorable disease control, clinical effectiveness, overall survival, and low toxicity with PRRT in patients with functional metastatic PCC/PGL from two referral centers.
