Aims.Ground-based observations at thermal infrared wavelengths suffer from large background radiation due to the sky, telescope and warm surfaces in the instrument. This significantly limits the ...sensitivity of ground-based observations at wavelengths longer than ~3 μm. The main purpose of this work is to analyse this background emission in infrared high-contrast imaging data as illustrative of the problem, show how it can be modelled and subtracted and demonstrate that it can improve the detection of faint sources, such as exoplanets. Methods. We used principal component analysis (PCA) to model and subtract the thermal background emission in three archival high-contrast angular differential imaging datasets in the M′ and L′ filter. We used an M′ dataset of β Pic to describe in detail how the algorithm works and explain how it can be applied. The results of the background subtraction are compared to the results from a conventional mean background subtraction scheme applied to the same dataset. Finally, both methods for background subtraction are compared by performing complete data reductions. We analysed the results from the M′ dataset of HD 100546 only qualitatively. For the M′ band dataset of β Pic and the L′ band dataset of HD 169142, which was obtained with an angular groove phase mask vortex vector coronagraph, we also calculated and analysed the achieved signal-to-noise ratio (S/N). Results. We show that applying PCA is an effective way to remove spatially and temporarily varying thermal background emission down to close to the background limit. The procedure also proves to be very successful at reconstructing the background that is hidden behind the point spread function. In the complete data reductions, we find at least qualitative improvements for HD 100546 and HD 169142, however, we fail to find a significant increase in S/N of β Pic b. We discuss these findings and argue that in particular datasets with strongly varying observing conditions or infrequently sampled sky background will benefit from the new approach.

ABSTRACT
Relative homogenization methods assume that measurements of nearby stations experience similar climate signals and rely therefore on dense station networks with high‐temporal correlations. ...In developing countries such as Peru, however, networks often suffer from low‐station density. The aim of this study is to quantify the influence of network density on homogenization. To this end, the homogenization method HOMER was applied to an artificially thinned Swiss network.
Four homogenization experiments, reflecting different homogenization approaches, were examined. Such approaches include diverse levels of interaction of the homogenization operators with HOMER, and different application of metadata. To evaluate the performance of HOMER in the sparse networks, a reference series was built by applying HOMER under the best possible conditions.
Applied in completely automatic mode, HOMER decreases the reliability of temperature records. Therefore, automatic use of HOMER is not recommended. If HOMER is applied in interactive mode, the reliability of temperature and precipitation data may be increased in sparse networks. However, breakpoints must be inserted conservatively. Information from metadata should be used only to determine the exact timing of statistically detected breaks. Insertion of additional breakpoints based solely on metadata may lead to harmful corrections due to the high noise in sparse networks.
The study shows that data homogenization increases the reliability of temperature and precipitation data in sparse station networks, particularly if the data is used for trend analyses. However, the performance of homogenization is strongly decreased in sparse compared to dense station networks. To avoid erroneous corrections in sparse networks, breakpoints should be inserted conservatively based on clear statistical detection and metadata.

Objective
Distress screening programs aim to ensure appropriate psychooncological support for cancer patients, but many eligible patients do not use these services. To improve distress management, we ...need to better understand patients' supportive care needs. In this paper, we report the first key finding from a longitudinal study that focused on patients' intentions to use psychooncological support and its association with distress and uptake of the psychooncology service.
Methods
We conducted a prospective, observational study in an Oncology Outpatient Clinic and assessed distress, intention to use psychooncological support, and uptake of the psychooncology service by using the Distress Thermometer, a semistructured interview, and hospital records. We analyzed data with a mixed‐methods approach.
Results
Of 333 patients (mean age 61 years; 55% male; 54% Distress Thermometer ≥ 5), 25% intended to use the psychooncology service (yes), 33% were ambivalent (maybe), and 42% reported no intention (no). Overall, 23% had attended the psychooncology service 4 months later. Ambivalent patients reported higher distress than patients with no intention (odds ratio = 1.18, 95% confidence interval 1.06‐1.32) but showed significantly lower uptake behavior than patients with an intention (odds ratio = 14.04, 95% confidence interval 6.74‐29.24). Qualitative analyses revealed that ambivalent patients (maybe) emphasized fears and uncertainties, while patients with clear intentions (yes/no) emphasized knowledge, attitudes, and coping concepts.
Conclusions
We identified a vulnerable group of ambivalent patients with high distress levels and low uptake behavior. To optimize distress screening programs, we suggest addressing and discussing patients' supportive care needs in routine clinical practice.

Objective
The pathophysiology of idiopathic spinal cord herniation remains unknown. However, several different factors have been postulated, such as congenital causes (ventral dura mater duplication, ...preexisting pseudomeningocele, or other congenital dural defects), inflammation, remote spinal trauma, or thoracic disc herniation. Herein, the diagnosis and surgical treatment of a patient with spinal cord herniation caused by an intraspinal bone spur is presented along with a relevant literature review.
Case presentation
A 56-year-old male patient presented with a non-traumatic Brown-Sequard syndrome persisting for over 1 year. A magnetic resonance imaging of the spinal axis revealed a ventral spinal cord displacement in the level of T 6/7. A supplementary thin-sliced computed tomography of the spine revealed a bone spur at the same level. For neurosurgical intervention, T 6 and T 7 laminectomy was performed. The cranial and caudal end of the right paramedian ventral dural defect was visualized and enlarged. Following extradural spinal cord mobilization by denticulate ligament transection, the spinal cord was finally released. The spinal cord was rotated and the ventral closure of the dural defect was performed by continuous suture. The patient recovered from surgery without additional deficits. The patient’s postoperative gait, sensory, and motor function deficits improved, and further neurological deterioration was prevented.
Conclusion
Since the first description of spinal cord herniation by Wortzman et al. in 1974, approximately 260 cases have been reported in the literature. In addition to other causes, intraspinal bone spur is a possible cause of spinal cord herniation.
A detailed overview of the knowledge gaps in our understanding of the heliospheric interaction with the largely unexplored Very Local Interstellar Medium (VLISM) are provided along with predictions ...of with the scientific discoveries that await. The new measurements required to make progress in this expanding frontier of space physics are discussed and include in-situ plasma and pick-up ion measurements throughout the heliosheath, direct sampling of the VLISM properties such as elemental and isotopic composition, densities, flows, and temperatures of neutral gas, dust and plasma, and remote energetic neutral atom (ENA) and Lyman-alpha (LYA) imaging from vantage points that can uniquely discern the heliospheric shape and bring new information on the interaction with interstellar hydrogen. The implementation of a pragmatic Interstellar Probe mission with a nominal design life to reach 375 Astronomical Units (au) with likely operation out to 550 au are reported as a result of a 4-year NASA funded mission study.

Aims.
RefPlanets is a guaranteed time observation programme that uses the Zurich IMaging POLarimeter (ZIMPOL) of Spectro-Polarimetric High-contrast Exoplanet REsearch instrument at the Very Large ...Telescope to perform a blind search for exoplanets in wavelengths from 600 to 900 nm. The goals of this study are the characterisation of the unprecedented high polarimetic contrast and polarimetric precision capabilities of ZIMPOL for bright targets, the search for polarised reflected light around some of the closest bright stars to the Sun, and potentially the direct detection of an evolved cold exoplanet for the first time.
Methods.

Ceti we used the polarimetricdifferential imaging (PDI) mode of ZIMPOL which removes the speckle noise down to the photon noise limit for angular separations ≿0.6′′. We describe some of the instrumental effects that dominate the noise for smaller separations and explain how to remove these additional noise effects in post-processing. We then combine PDI with angular differential imaging as a final layer of post-processing to further improve the contrast limits of our data at these separations.
Results.
For good observing conditions we achieve polarimetric contrast limits of 15.0–16.3 mag at the effective inner working angle of ~0.13′′, 16.3–18.3 mag at 0.5′′, and 18.8–20.4 mag at 1.5′′. The contrast limits closer in (≾0.6′′) display a significant dependence on observing conditions, while in the photon-noise-dominated regime (≿0.6′′) the limits mainly depend on the brightness of the star and the total integration time. We compare our results with contrast limits from other surveys and review the exoplanet detection limits obtained with different detection methods. For all our targets we achieve unprecedented contrast limits. Despite the high polarimetric contrasts we are not able to find any additional companions or extended polarised light sources in the data obtained so far.

Context.
Young stars with debris disks are the most promising targets for an exoplanet search because debris indicate a successful formation of planetary bodies. Debris disks can be shaped by planets ...into ring structures that give valuable indications on the presence and location of planets in the disk.
Aims.
We performed observations of the Sco-Cen F star HD 117214 to search for planetary companions and to characterize the debris disk structure.
Methods.
HD 117214 was observed with the SPHERE subsystems IRDIS, IFS, and ZIMPOL at optical and near-IR wavelengths using angular and polarimetric differential imaging techniques. This provided the first images of scattered light from the debris disk with the highest spatial resolution of 25 mas and an inner working angle <0.1″. With the observations with IRDIS and IFS we derived detection limits for substellar companions. The geometrical parameters of the detected disk were constrained by fitting 3D models for the scattering of an optically thin dust disk. Investigating the possible origin of the disk gap, we introduced putative planets therein and modeled the planet–disk and planet–planet dynamical interactions. The obtained planetary architectures were compared with the detection limit curves.
Results.
The debris disk has an axisymmetric ring structure with a radius of 0.42(±0.01)″ or ~45 au and an inclination of 71(±2.5)° and exhibits a 0.4″ (~40 au) wide inner cavity. From the polarimetric data, we derive a polarized flux contrast for the disk of (

Conclusions.
The fractional scattered polarized flux of the disk is eight times lower than the fractional IR flux excess. This ratio is similar to the one obtained for the debris disk HIP 79977, indicating that dust radiation properties are similar for these two disks. Inside the disk cavity we achieve high-sensitivity limits on planetary companions with a mass down to ~4
M
J
at projected radial separations between 0.2″ and 0.4″. We can exclude stellar companions at a radial separation larger than 75 mas from the star.

The expression of amino acid transporters in small intestine epithelia of human newborns has not been studied yet. It is further not known whether the maturation of imino acid (proline) transport is ...delayed as in the kidney proximal tubule. The possibility to obtain small intestinal tissue from patients undergoing surgery for jejunal or ileal atresia during their first days after birth was used to address these questions. As control, adult terminal ileum tissue was sampled during routine endoscopies. Gene expression of luminal imino and amino acid transporter SIT1 (SLC6A20) was approximately threefold lower in newborns versus adults. mRNA levels of all other luminal and basolateral amino acid transporters and accessory proteins tested were similar in newborn mucosa compared with adults. At the protein level, the major luminal neutral amino acid transporter B
AT1 (SLC6A19) and its accessory protein angiotensin-converting enzyme 2 were shown by immunofluorescence to be expressed similarly in newborns and in adults. SIT1 protein was not detectable in the small intestine of human newborns, in contrast to adults. The morphology of newborn intestinal mucosa proximal and distal to the obstruction was generally normal, but a decreased proliferation rate was visualized distally of the atresia by lower levels of the mitosis marker K
-67. The mRNA level of the 13 tested amino acid transporters and accessory proteins was nonetheless similar, suggesting that the intestinal obstruction and interruption of amniotic fluid passage through the small intestinal lumen did not affect amino acid transporter expression. NEW & NOTEWORTHY System IMINO transporter SIT1 is not expressed in the small intestine of human newborns. This new finding resembles the situation in the proximal kidney tubule leading to iminoglycinuria. Lack of amniotic fluid passage in small intestinal atresia does not affect amino acid transporter expression distal to intestinal occlusion.
Context. In recent years, our understanding of giant planet formation progressed substantially. There have even been detections of a few young protoplanet candidates still embedded in the ...circumstellar disks of their host stars. The exact physics that describes the accretion of material from the circumstellar disk onto the suspected circumplanetary disk and eventually onto the young, forming planet is still an open question. Aims. We seek to detect and quantify observables related to accretion processes occurring locally in circumstellar disks, which could be attributed to young forming planets. We focus on objects known to host protoplanet candidates and/or disk structures thought to be the result of interactions with planets. Methods. We analyzed observations of six young stars (age 3.5–10 Myr) and their surrounding environments with the SPHERE/ZIMPOL instrument on the Very Large Telescope (VLT) in the Hα filter (656 nm) and a nearby continuum filter (644.9 nm). We applied several point spread function (PSF) subtraction techniques to reach the highest possible contrast near the primary star, specifically investigating regions where forming companions were claimed or have been suggested based on observed disk morphology. Results. We redetect the known accreting M-star companion HD142527 B with the highest published signal to noise to date in both Hα and the continuum. We derive new astrometry ( r=62.8−2.7+2.1 $r{\,=\,}62.8^{+2.1}_{-2.7}$ r = 62.8−2.7+2.1 mas and PA=(98.7±1.8)° $\mathrm{PA}{\,=\,}(98.7{\,\pm\,}1.8)^{\circ}$PA = (98.7 ± 1.8)° ) and photometry (ΔN_Ha = 6.3−0.3+0.2 $6.3^{+0.2}_{-0.3}$6.3−0.3+0.2 mag, ΔB_Ha = 6.7 ± 0.2 mag and ΔCnt_Ha = 7.3−0.2+0.3 $7.3^{+0.3}_{-0.2}$7.3−0.2+0.3 mag) for the companion in agreement with previous studies, and estimate its mass accretion rate (Ṁ ≈ 1−2 × 10−10 M⊙yr−1). A faint point-like source around HD135344 B (SAO206462) is also investigated, but a second deeper observation is required to reveal its nature. No other companions are detected. In the framework of our assumptions we estimate detection limits at the locations of companion candidates around HD100546, HD169142, and MWC 758 and calculate that processes involving Hα fluxes larger than ~ 8 × 10−14–10−15 erg s−1 cm−2 (Ṁ > 10−10−10−12 M⊙yr−1) can be excluded. Furthermore, flux upper limits of ~10−14−10−15 erg s−1 cm−2 (Ṁ < 10−11–10−12 M⊙yr−1) are estimated within the gaps identified in the disks surrounding HD135344 B and TW Hya. The derived luminosity limits exclude Hα signatures at levels similar to those previously detected for the accreting planet candidate LkCa15 b.

Currently there are few tools available for clinicians to predict outcomes in cardiac arrest survivors. Our objective was to determine if the combination of simple clinical parameters (initial blood ...lactate and vasopressor use) can predict outcome in post-cardiac arrest patients.
The design was a retrospective medical record review. The study was carried on in two urban, tertiary-care, university teaching hospitals. As for patients, inclusion criteria were: 1) age ≥18 years; 2) non-traumatic out-of-hospital cardiac arrest with return of spontaneous circulation; 3) lactic acid measured within one hour of return of circulation. No interventions was performed.
Patients were divided into groups based on two variables: 1) vasopressor status (receipt of vasopressors vs. no vasopressors); and 2) initial blood lactate (categories defined as lactate <5 mmol/L, lactate 5 to 10 mmol/L, lactate ≥10 mmol/L); 128 out-of-hospital cardiac arrest patients met study inclusion criteria. Overall mortality was 71% (95%CI 63-79%). Patients who received vasopressors had significantly higher mortality rates compared to patients who did not receive vasopressors (80% vs. 52%; P=0.002). A stepwise increase in mortality is associated with increasing lactate levels (39% lactate <5 mmol/L, 67% lactate 5 mmol/L to10 mmol/L, and 92% lactate ≥10 mmol/L; P<0.001). The AUC for our model was 0.82.
The combination of two clinical parameters, vasopressor need and lactic acid levels, is an accurate severity of illness classification system and can predict mortality in patients following out-of-hospital cardiac arrest. Prospective validation of these variables in post-cardiac arrest is needed.
