Abstract Few studies have examined the association between perioperative blood transfusion and postoperative delirium (POD) in aged patients undergoing total hip replacement surgery. In this ...prospective study, 186 patients older than 65 years undergoing elective unilateral total hip replacement surgery were enrolled. Of those, 94 patients were randomly assigned to the restrictive strategy transfusion strategy group, in which red blood cells were transfused in order to maintain 10.0 g/dL > hemoglobin ≧ 8.0 g/dL. Ninety-two patients were randomly assigned to the liberal transfusion strategy group, in which red blood cells were transfused in order to maintain hemoglobin ≧ 10.0 g/dL. POD was diagnosed by confusion assessment method. The baseline characteristics of patients, the length of hospital stay, the incidence of POD, myocardial infarction, stroke, wound infection, pulmonary embolism, and the transfusion volume were recorded. No difference was observed in the baseline characteristics, the length of hospital stay, and the incidence of POD, myocardial infarction, stroke, wound infection, and pulmonary embolism between the two groups ( P > 0.05). The proportion of patients transfused with red blood cell and frozen plasma was decreased in the restrictive transfusion group compared with the liberal transfusion group ( P < 0.05). In conclusion, restrictive transfusion does not influence the incidence of POD but reduces blood transfusion. Thus, restrictive transfusion may serve as an effective and safe strategy for aged patients following total hip replacement.

To determine the predictability of diagnosing diabetic nephropathy (DN) versus non-diabetic renal disease (NDRD) from clinical and laboratory data in Chinese patients with type 2 diabetes mellitus ...(T2DM) manifesting heavy proteinuria.
We retrospectively analyzed the clinical and laboratory data of patients with T2DM manifesting heavy proteinuria who underwent renal biopsy from January 2014 to December 2017.
According to renal biopsy, 220 patients were finally enrolled, including 109 cases diagnosed with DN alone (49.55%), 94 with NDRD alone (42.73%) and 17 with DN plus superimposed NDRD (7.73%). Multivariate analysis showed the significant risk factors for DN alone were age, duration of diabetes, presence of retinopathy, 24-h proteinuria, serum albumin and SBP. Presence of retinopathy achieved the highest overall diagnostic efficiency with the area under the curve of 0.852, sensitivity of 78.9% and specificity of 91.5%. The combined diagnosis with four indicators (duration of diabetes, retinopathy, SBP, and serum albumin) showed the area under the curve of 0.938, sensitivity of 88.1% and specificity of 87.2%.
The prevalence of DN is high in patients with T2DM manifesting heavy proteinuria. Renal biopsy should be performed in diabetics in the atypical clinical scenario.

Materials with strongly coupled magnetic and electronic degrees of freedom provide new possibilities for practical applications.In this paper,we have investigated the structure,magnetic property,and ...magnetodielectric(MD)effect in Ho2Cu2O5 and Yb2Cu2O5 polycrystalline samples,which possess a non-centrosymmetric polar structure with space group Pna21.In Ho2Cu2O5,Ho3+and Cu2+sublattices order simultaneously,exhibiting a typical paramagnetic to antiferro-magnetic transition at 13.1 K.While for Yb2Cu2O5,two magnetic transitions which originate from the orderings of Yb3+(7.8 K)and Cu2+(13.5 K)sublattices are observed.A magnetic field induced metamagnetic transition is obtained in these two cuprates below Néel temperature(TN).By means of dielectric measurement,distinct MD effect is demonstrated by the dielectric anomaly at TN.Meanwhile,the MD effect is found to be directly related to the metamagnetic transition.Due to the specific spin configuration and different spin evolution in the magnetic field,a positive MD effect is formed in Ho2Cu2O5,and a negative one is observed in Yb2Cu205.The spontaneous dielectric anomaly at TN is regarded as arising from the shifts in optical phonon frequencies,and the magnetoelectric coupling is used to interpret the magnetic field in-duced MD effect.Moreover,an H-T phase diagram is constructed for Ho2Cu2O5 and Yb2Cu2O5 based on the results of isothermal magnetic and dielectric hysteresis loops.

Phyi Dbang-Dung Kar is one of the most important archaeological sites in western Tibet which lasted from the Bronze Age to the 16th century. Nomadic swords here provide new evidence of interaction outside the western Himalayas. These copper-base and gold artifacts were analyzed for their elemental, metallographic, and lead isotopic features. A comparative study suggests that the raw materials used changed over time and show few connections with Nepal or southwest China. However, similar copper-base and gold techniques were seen in Nepal and India. Even though direct trading is not confirmed, interaction regarding techniques in the western Himalayas is a possibility.

To choose an appropriate kernel function is one major task for SVM. Different kernel functions will produce different SVMs and may result in different performances. Combined kernel function shows ...more stable and higher performance than single kernel function, so there is a need to optimize the combined kernel function to enhance the generalization capability of SVM. This paper proposes to optimize the combined kernel function by particle swarm optimization (PSO) based on large margin learning theory of SVM. The comparison of the performance between GA and PSO algorithm on this optimization problem is provided. The simulation results show that the PSO is another feasible solution for optimization of combined kernel function, which normally leads to SVM with better generalization capability and stability.

By choosing proper element type and material modle, the designed box grider is modeled by the nonlinear analysis software ANSYS. The shear lag effect of the concrete huge section box girder is studied by applying four load conditions including prestressed load, the upper and lower live load and double load. Parameter analysis are done, which include the load type and width-span ratio.And corresponding conclusions are drawn.
