Les métastases hypophysaires et la sarcoïdose sont deux causes connues de l’épaississement de la tige pituitaire. Leur association est décrite depuis 3 décennies. Nous rapportons dans ce cadre, un ...cas rare de panhypopituitarisme révélant une métastase pituitaire d’un carcinome pulmonaire à petites cellules associé à une sarcoïdose.
Il s’agit d’un patient âgé de 49 ans tabagique et diabétique type 2, hospitalisé dans un tableau d’insuffisance surrénalienne aiguë avec un syndrome polyuro-polydipsique et un syndrome tumoral hypophysaire. L’exploration hormonale a confirmé l’insuffisance antéhypophysaire globale et le test de restriction hydrique a été en faveur d’un diabète insipide central. L’IRM hypothalamo-hypophysaire a montré un processus expansif intrasellaire de 1 cm avec épaississement nodulaire de la tige. La radio thorax a montré une opacité basale gauche hétérogène mal limitée. Le scanner thoraco-abdominal a objectivé la présence, au niveau du champ pulmonaire gauche, d’une masse tumorale très suspecte de malignité avec multiples adénopathies médiastinales, de multiples nodules hépatiques et une hypertrophie de la surrénale gauche. Une fibroscopie bronchique, réalisée à trois reprises, a montré une infiltration sténosante de l’arbre bronchique gauche mais l’étude anatomopathologique des biopsies bronchiques a été toujours non concluante. Une ponction biopsie du foie sous échographie a été alors réalisée et l’étude anatomopathologique a trouvé l’aspect d’une hépatite granulomateuse chronique compatible avec une sarcoïdose.
Le diagnostic de sarcoïdose n’a pu être retenu comme cause de l’atteinte pulmonaire surtout devant l’altération de l’état général du patient. Le contrôle radiologique a montré une évolution lésionnelle importante et l’étude anatomopathologique d’une biopsie trans-thoracique sous scanner a mis en évidence la présence d’un carcinome à petites cellules.
Chez l’homme, le carcinome à petites cellules est le cancer qui se complique le plus fréquemment de métastases hypophysaires. L’association sarcoïdose - néoplasie reste controversée. Le lien entre les deux est jusque là non prouvé avec risque élevé d’erreur de classification.
Pituitary metastasis and sarcoidosis are two causes of pituitary stalk thickening. Their association has been described ago three decades. In this setting, we report a case of panhypopituitarism revealing pituitary metastasis from a small-cell lung carcinoma associated with sarcoidosis.
A 49 year-old smoking patient with type 2 diabetes was admitted for acute adrenal failure with polyuria polydipsia syndrome and a pituitary tumor syndrome. Hormone explorations confirmed anterior pituitary insufficiency. Water restriction revealed central diabetes insipidus. The hypothalamic-pituitary MRI revealed a 1-cm sellar mass with nodular thickening of the stalk. The chest radiograph showed a heterogeneous opacity in the left lung. The thoraco-abdominal scan demonstrated a mass in the left lung highly suggestive of malignancy and many enlarged mediastinal nodes, hepatic nodules, and hypertrophy of the left adrenal.
Bronchoscopy was performed three times and showed infiltration of the left bronchial tree but histological examination of the bronchial biopsies was negative for all samples. Ultrasound-guided biopsy of the liver was achieved and histology demonstrated sarcoidosis.
The diagnosis of sarcoidosis was incompatible with the deterioration of the patient's general status. Subsequent radiographic explorations showed an increase in the size of the tumor mass and histological evaluation of a scan-guided trans-thoracic biopsy demonstrated small-cell carcinoma.
Small-cell lung carcinoma is the most common cancer with pituitary metastasis. The proposed link between sarcoidosis and malignancy has remained controversial but has not been proven false.

L’atteinte de la tige pituitaire est fréquemment rencontrée depuis le développement de l’imagerie hypophysaire. La tige peut être la cible de diverses pathologies, infectieuses, infiltratives et ...tumorales. Dans les cas où l’enquête étiologique s’avère négative, des anomalies génétiques ont été incriminées particulièrement lorsqu’il existe des malformations cérébrales associées. À travers 11 observations, nous montrons la variabilité des affections pouvant atteindre la tige pituitaire ainsi que leurs aspects cliniques et radiologiques. Il s’agit d’une étude rétrospective à propos de 7 hommes (67 %) et 4 femmes (33 %) dont l’âge moyen était de 28 ans (15-53 ans). Ils avaient eu une IRM hypophysaire pour une insuffisance antéhypophysaire dissociée, un diabète insipide central ou une hyperprolactinémie. Trois patients avaient eu une IRM cérébrale pour cause extra-hypophysaire. L’IRM hypophysaire avait montré une interruption de la tige pituitaire dans 3 cas (27 %), son atrophie dans 1 cas (9 %) et son épaississement dans 7 cas (67 %). L’atteinte de la tige était associée à une hyerprolactinémie dans 3 cas (27 %), un diabète insipide central dans 4 cas (36 %), une insuffisance somatotrope dans 4 cas (36 %), une insuffisance corticotrope dans 5 cas (45 %), une insuffisance gonadotrope dans 5 cas (45 %) et une insuffisance thyréotrope dans 1 cas (9 %). L’enquête étiologique a permis de retenir le diagnostic de métastase sellaire dans 2 cas (18 %), d’histiocytose langerhansienne, de tuberculose et d’hypophysite auto-immune à chaque fois dans 1 cas (9 %). Enfin l’enquête étiologique était négative dans les 6 cas restants (54 %). Devant la diversité des pathologies en cause, on insiste sur l’importance d’une enquête étiologique minutieuse, de la surveillance clinique et radiologique à la recherche d’éléments orientant vers l’origine de l’atteinte de la tige pituitaire classée initialement comme idiopathique.
Newer techniques of magnetic resonance imaging (MRI) describe more accurately pituitary stalk abnormalities such as infections, infiltrative lesions and tumors. In absence of all the above mentioned etiological factors, genetics defects are suspected, mainly when other malformations are equally present. We attempt to show through 11 observations the variability of pathologies involving the pituitary stalk with their respective clinical and radiological features and associated endocrine abnormalities. This is a retrospective study of 7 men (67%) and 4 women (33%), mean age of 28 year (range: 15 to 53) in whom pituitary MRI was performed for hypopituitarism, diabetes insipidus or hyperprolactinemia. Three patients had brain MRI for an extra-pituitary condition. The pituitary MRI showed a stalk section in 3 cases (27%), atrophy in 1 case and thickening in 7 cases (67%). The pituitary stalk anomaly was associated with hyperprolactinemia in 3 cases (27%), central diabetes insipidus in 4 cases (36%), growth hormone deficiency in 4 cases (36%), adrenal insufficiency in 5 cases (45%), hypogonadism in 5 cases (45%) and hypothyroidism in one case (9%). Established diagnoses were: sellar metastasis in 2 cases (18%), Langerhans’ histocytosis, tuberculosis and autoimmune hypophysitis respectively in 3 cases (9%). In 6 cases (54%), no clear etiology was found. Given the multitude of pituitary stalk pathologies, a detailed etiologic inquiry must be performed in order to detect elements able to reclassify an initially idiopathic disorder.

Context: LHX4 is a LIM homeodomain transcription factor involved in pituitary ontogenesis. Only a few heterozygous LHX4 mutations have been reported to be responsible for congenital pituitary hormone ...deficiency.
Subjects and Methods: A total of 136 patients with congenital hypopituitarism associated with malformations of brain structures, pituitary stalk, or posterior pituitary gland was screened for LHX4 mutations.
Results: Three novel allelic variants that cause predicted changes in the protein sequence of LHX4 (2.3%) were found (p.Thr99fs, p.Thr90Met, and p.Gly370Ser). On the basis of functional studies, p.Thr99fs mutation was responsible for the patients’ phenotype, whereas p.Thr90Met and p.Gly370Ser were likely polymorphisms. Patients bearing the heterozygous p.Thr99fs mutation had variable phenotypes: two brothers presented somato-lactotroph and thyrotroph deficiencies, with pituitary hypoplasia and poorly developed sella turcica; the youngest brother (propositus) also had corpus callosum hypoplasia and ectopic neurohypophysis; their father only had somatotroph deficiency and delayed puberty with pituitary hyperplasia. Functional studies showed that the mutation induced a complete loss of transcriptional activity on POU1F1 promoter and a lack of DNA binding. Cotransfection of p.Thr99fs mutant and wild-type LHX4 failed to evidence any dominant negative effect, suggesting a mechanism of haploinsufficiency. We also identified prolactin and GH promoters as potential target genes of LHX4 and found that the p.Thr99fs mutant was also unable to transactivate these promoters.
Conclusions: The present report describes three new exonic LHX4 allelic variants with at least one being responsible for congenital hypopituitarism. It also extends the phenotypical heterogeneity associated with LHX4 mutations, which includes variable anterior pituitary hormone deficits, as well as pituitary and extrapituitary abnormalities.
L’ectopie thyroïdienne est une pathologie rare (1/4000 à 1/8000 des patients atteints d’hypothyroïdie) dont la pathogénie reste mal élucidée. Elle peut être asymptomatique ou se manifester par une ...hypothyroïdie clinique ou biologique. Nous rapportons deux observations d’ectopie thyroïdienne diagnostiquées à un âge avancé. La première est celle d’une patiente âgée de 20 ans connue hypothyroïdienne depuis l’âge de 13 ans et qui a été hospitalisée suite à l’arrêt de son traitement hormonal substitutif. À l’examen, la thyroïde n’était pas palpable et l’examen de l’arrière cavité buccale a montré une formation linguale postérieure. À la scintigraphie thyroïdienne, cette formation correspond à une thyroïde ectopique en position sublinguale. La deuxième observation est celle d’une patiente âgée de 32 ans, hospitalisée pour exploration d’une hyperprolactinémie. À l’examen clinique, il y avait des signes d’hypothyroïdie et la thyroïde n’était pas palpable. L’échographie cervicale a montré une loge thyroïdienne vide mais a mi en évidence une formation sublinguale rappelant une thyroïde ectopique. La scintigraphie thyroïdienne, la tomodensitométrie et l’imagerie par résonnance magnétique cervicales ont confirmé le diagnostic d’ectopie thyroïdienne en position sublinguale. Les deux patientes ont été mises sous traitement hormonal substitutif.
Nous discutons dans cet article les particularités cliniques et radiologiques de nos observations, ainsi que les bases génétiques de cette pathologie ; et nous insistons sur la nécessité d’évoquer ce diagnostic devant toute hypothyroïdie associée à l’absence de palpation d’une glande thyroïde en position normale même chez l’adulte.
Ectopic thyroid is a rare condition (1/4000 to 1/8000 among patients with hypothyroidism). The underlying etiological pathogenic mechanisms remain unknown. Diagnosis is established on the basis of imaging findings. We report two cases of hypothyroidism in adult females who had ectopic sublingual thyroid glands. The first patient was a 20-year-old woman who had been treated for hypothyroidism since the age of 13 years before the diagnosis of ectopic thyroid 7 years later. In both patients, the thyroid gland was palpable. In the first patient the physical examination revealed an ectopic sublingual gland. Scintigraphy confirmed the diagnosis in both patients. The CT-scan and MRI were positive in the second patient. Hormonal substitution therapy using L-thyroixine was given.

Abstract Objective Our objective was to study the effects of physical training combined with dietary measures in obese adults. In a second step, we sought to compare two training protocols and ...establish the additional contribution of strength training. Methods We performed a randomized, prospective survey from July 2004 to November 2007. Included patients were randomized into three groups: a control group (G1), a group (G2) performing dietary measures and a programme of treadmill training at 60% of each individual's maximum heart rate (HRmax ) and a group (G3) who followed the G2 programme supplemented with strength training. All patients underwent an initial and final assessment of anthropometric & cardiovascular parameters, muscle strength, dyspnoea during activities of daily living, metabolic disorders, psychological status and quality of life. Results The greatest weight loss (7.24%) was observed in G3. Reduction in waistline measurement (WL) of 4.3% and 10.26% were noted in G2 and G3, respectively ( p < 0.001). The percentage fat body mass fell by 10.4% in G3 ( p < 0.001) and 8.6% in G2 ( p = 0.03). We particularly noted an improvement in physical condition in groups 2 and 3, with lower HR and blood pressure values at rest and at maximum effort. The overall improvement in both arm and leg muscle strength was greater for G3 than for G2. Likewise, we noted an improvement in the metabolic parameters and depression & anxiety scores for the trained groups (G2, G3), relative to the control group (G1). We also noted improvements in the total impact of weight on quality of life (IWQOL) lite score of 15.2% in G2 and 18% in G3. Conclusion Our survey demonstrated the beneficial effect of combining dietary measures and physical training in obese patients. In addition to weight loss, the programme enabled a reduction in the patients’ body fat mass and abdominal obesity, a correction of metabolic disorders and an improvement in aerobic capacity. The improvement in all these parameters also enhanced the patients’ psychological status and quality of life. The addition of strength training produced notable improvements in weight loss, arm muscle strength and abdominal obesity.

Le pneumomédiastin est une pathologie rare dont l’incidence est estimée à 1 cas/33 000. Il constitue une complication rare de l’acidocétose diabétique. A travers deux observations personnelles de ...patients diabétiques et d’une revue de la littérature, nous analyserons la relation entre l’acidocétose diabétique et cette affection.
Il s’agit d’une jeune fille et d’un jeune garçon âgés respectivement de 21 et 18 ans, diabétiques type 1, hospitalisés pour une décompensation acidocétosique. La symptomatologie clinique associait, outre les signes cardinaux de diabète, un syndrome grippal à une dyspnée et à une douleur thoracique. A l’examen, l’état général de nos patients était altéré, avec tachycardie et polypnée. L’examen physique notait une tuméfaction diffuse douloureuse du cou avec un emphysème sous-cutané. Les explorations biologiques montraient des glycémies élevées avec présence d’acétone dans les urines et une acidose métabolique sévère. La radiographie thoracique confirmait l’emphysème sous-cutané avec présence d’air au niveau du médiastin antérieur. Le scanner thoracique, réalisé chez le 2
e patient, montrait une hyperclarté latéro-médiastinale entourant la silhouette cardiaque. Le traitement a consisté en un repos strict avec oxygénothérapie, réhydratation, insulinothérapie et héparinothérapie. L’évolution était marquée par une résolution du pneumomédiastin en 3 jours en moyenne, dans les deux cas.
La causalité de l’acidocétose diabétique dans la survenue du pneumomédiastin chez nos deux patients a été retenue après avoir éliminé les autres causes, à savoir un traumatisme thoracique, une crise d’asthme.
Pneumomediastinum is a rare condition with an incidence of 1/33,000. It can be a rare complication of diabetic acidoketosis. We présent the cases of two diabetic patients and review the literature, focusing our analysis on the interrelationships between these two diseases.
Both patients were young subjects, a 21-year-old woman and an 18-year-old man with type 1 diabetes who were admitted for acidoketosis. Clinically, the patients presented the cardinal signs of diabetes and a flu-like syndrome associated with dyspnea and chest pain. Physical examination revealed a poor general health status, tachycardia and polymnea, as well as a painful diffuse tumefaction of the neck with subcutaneous emphysema. Blood tests disclosed elevated glycemia and urine was positive for acetone. The diagnosis of severe metabolic acidosis was retained. The chest x-ray demonstrated the subcutaneous emphysema and air in the anterior mediastinum. On the computed tomography scan obtained in the second patient, the heart was silhouetted with a hyperlucent zone laterally. Treatment consisted in strict bed rest with oxygen therapy, fluid replacement, insulin and heparin. The pneumomediastinum resolved in both patients within three days on average.
The causal effect of diabetic acidoketosis in the development of pneumomediastinum in our two patients was retained after ruling out all other potential causes, including chest trauma and asthma.

35 associée au moins à un des facteurs de risque, ayant des gonalgies mécaniques avec des signes radiologiques évidents de gonarthrose. Tous les patients ont bénéficié d’une évaluation avant et après prise en charge, de la perte de poids, des paramètres de l’impédancemétrie (Tanita
® TBF-300), de la douleur, de la distance parcourue lors du test de six minutes, des paramètres cardiovasculaires (une épreuve d’effort, selon le protocole de Bruce modifié) et de la force musculaire. L’évaluation du retentissement fonctionnel a été réalisée par les indices de Womac et de Lequesne. Tous les patients ont été randomisés en quatre groupes, un groupe témoin (G1), un groupe exercice physique seul (G2), un groupe exercice physique associé au régime (G3) et un groupe régime seul (G4).
Les quatres groupes étaient comparables pour tous les paramètres. Une amélioration de la fonction (selon le Womac) a été constatée dans les groupes ayant bénéficié d’exercice physique seul (G2) (26 %), d’exercice associé au régime (G3) (37,89 %) et dans le groupe régime seul (G4) (18,34 %). De même on a constaté une amélioration de la douleur aussi bien dans le G2 (

Une amélioration significative des paramètres cardiovasculaires a été retrouvée dans le G2 et le G3. La perte de poids, la diminution de l’indice de masse corporelle ainsi que du tour de taille étaient plus importants dans les groupes ayant bénéficié de l’exercice physique associé au régime (G3).
Notre étude souligne l’intérêt d’une association de l’exercice physique et du régime dans la prise en charge de la gonarthrose modérée chez l’obèse. Ce type de prise en charge permet, d’une part, une diminution des contraintes mécaniques et, d’autre part, une amélioration de la capacité physique et de la force musculaire permettant ainsi une amélioration du retentissement algofonctionel de la gonarthrose chez les obèses. De même notre étude souligne l’intérêt particulier de l’exercice physique utilisé seul dans la prise en charge de ces obèses souffrant de gonarthrose.
Our objective was to determine whether exercise and weight loss are more effective either separately or in combination, in improving pain and physical function in obese adults with moderate knee osteoarthritis (OA).

35 associated to at least one cardiovascular risk factor, suffering from knee pain with evident radiographic signs of knee OA, were involved in our study. All patients were evaluated at baseline and at the end of the study. The assessment parameters were weight loss, the bioelectric impedance analysis, pain, six-minute walk distance, cardiovascular parameters, and muscular strength. The physical function was measured with the Womac and the Lequesne indexes. Patients were randomized into four groups, a control group (G1), exercise only group (G2), diet plus exercise group (G3) and diet only group (G4).
There was no difference between the four groups at baseline. Significant improvement of function (Womac) was noticed in groups performing exercise only (G2) (26 %), diet plus exercise (G3) (37,89 %) and diet only (G4) (18,34 %). We also noticed an improvement in pain in G2 (
