AbstractBackgroundElectrocardiogram (ECG) lead misplacement can adversely affect ECG diagnosis and subsequent clinical decisions. V1 and V2 are commonly placed superior of their correct position. The ...aim of the current study was to use machine learning approaches to detect V1 and V2 lead misplacement to enhance ECG data quality. MethodECGs for 453 patients, (normal n = 151, Left Ventricular Hypertrophy (LVH) n = 151, Myocardial Infarction n = 151) were extracted from body surface potential maps. These were used to extract both the correct and incorrectly placed V1 and V2 leads. The prevalence for correct and incorrect leads were 50%. Sixteen features were extracted in three different domains: time-based, statistical and time-frequency features using a wavelet transform. A hybrid feature selection approach was applied to select an optimal set of features. To ensure optimal model selection, five classifiers were used and compared. The aforementioned feature selection approach and classifiers were applied for V1 and V2 misplacement in three different positions: first, second and third intercostal spaces (ICS). ResultsThe accuracy for V1 misplacement detection was 93.9%, 89.3%, 72.8% in the first, second and third ICS respectively. In V2, the accuracy was 93.6%, 86.6% and 68.1% in the first, second and third ICS respectively. There is a noticeable decline in accuracy when detecting misplacement in the third ICS which is expected.

Latest Sandbian to early Katian sequences across Laurentia's epicontinental sea exhibit a transition from lithologies characterized as ‘warm-water’ carbonates to those characterized as ...‘cool-water'carbonates. This shift occurs across the regionally recognized M4/M5 sequence stratigraphic boundary and has been attributed to climatic cooling and glaciation, basin reorganization and upwelling of open ocean water, and/or increased water turbidity and terrigenous input associated with the Taconic tectophase. Documentation of oxygen isotopic trends across the M4/M5 and through bracketing strata provides a potential means of distinguishing among these alternative scenarios; however, oxygen isotopic records generated to date have failed to settle the debate. This lack of resolution is because δ18O records are open to multiple interpretations and potentially confounding factors related to local environmental conditions have not been tested by examining the critical interval in multiple areas and different depositional settings. To begin to address this shortcoming, we present new species-specific and mixed assemblage conodont δ18O values in samples spanning the M4/M5 boundary from the Upper Mississippi Valley, Alabama, and Virginia. The new results are combined with previous studies, providing a record of δ18O variability across SE Laurentia. The combined dataset allows us to test for regional trends at a resolution not previously available. Our results document a ~1.5‰ decrease in values across Laurentia instead of increasing δ18O values across the M4/M5 as predicted in various ‘cool-water’ scenarios. In short, these results do not support a shift to ‘cool-water’ conditions as an explanation for changes in early Katian carbonates across the M4/M5.
Coronary artery vasospasm is the sudden narrowing of an artery caused by rapid prolonged contraction. It reduces blood supply to the heart and can present with typical cardiac chest pain symptoms. ...Vasospasm can lead to fatal arrhythmic complications such as ventricular fibrillation. Our case report describes an example of this occurring in a 53-year-old female, and the management plan that ensued. We look at the importance of accurate and prompt diagnosis of vasospasm and how this can have implications for treatment options. One of the available treatments for vasospasm is placement of an implantable cardioverter defibrillator (ICD). This delivers a shock in the event of future life-threatening arrhythmia, with the aim of preventing cardiac arrest. ICD placement, however, is not always a suitable option. This case report discusses the various challenges that arose while making the decision for ICD placement and gives insight into the best available treatment options for coronary artery vasospasm. We also highlight early warning signs that predict life-threatening vasospastic events and how this can be diagnosed and treated appropriately.

The killer immunoglobulin-like receptors (KIRs), found predominantly on the surface of natural killer (NK) cells and some T-cells, are a collection of highly polymorphic activating and inhibitory ...receptors with variable specificity for class I human leukocyte antigen (HLA) ligands. Fifteen KIR genes are inherited in haplotypes of diverse gene content across the human population, and the repertoire of independently inherited KIR and HLA alleles is known to alter risk for immune-mediated and infectious disease by shifting the threshold of lymphocyte activation. We have conducted the largest disease-association study of KIR-HLA epistasis to date, enabled by the imputation of KIR gene and HLA allele dosages from genotype data for 12,214 healthy controls and 8,107 individuals with the HLA-B*27-associated immune-mediated arthritis, ankylosing spondylitis (AS). We identified epistatic interactions between KIR genes and their ligands (at both HLA subtype and allele resolution) that increase risk of disease, replicating analyses in a semi-independent cohort of 3,497 cases and 14,844 controls. We further confirmed that the strong AS-association with a pathogenic variant in the endoplasmic reticulum aminopeptidase gene ERAP1, known to alter the HLA-B*27 presented peptidome, is not modified by carriage of the canonical HLA-B receptor KIR3DL1/S1. Overall, our data suggests that AS risk is modified by the complement of KIRs and HLA ligands inherited, beyond the influence of HLA-B*27 alone, which collectively alter the proinflammatory capacity of KIR-expressing lymphocytes to contribute to disease immunopathogenesis.

The human MHC represents the strongest susceptibility locus for autoimmune diseases. However, the identification of the true predisposing gene(s) has been handicapped by the strong linkage ...disequilibrium across the region. Furthermore, most studies to date have been limited to the examination of a subset of the HLA and non-HLA genes with a marker density and sample size insufficient for mapping all independent association signals. We genotyped a panel of 1,472 SNPs to capture the common genomic variation across the 3.44 megabase (Mb) classic MHC region in 10,576 DNA samples derived from patients with systemic lupus erythematosus, Crohn's disease, ulcerative colitis, rheumatoid arthritis, myasthenia gravis, selective IgA deficiency, multiple sclerosis, and appropriate control samples. We identified the primary association signals for each disease and performed conditional regression to identify independent secondary signals. The data demonstrate that MHC associations with autoimmune diseases result from complex, multilocus effects that span the entire region.

There is no consensus and a limited evidence base for choice of contrast agents (CA) in angiography. This study evaluated the impact of iohexol and iodixanol CA on fibrinolytic factors (tissue ...plasminogen activator t-PA and plasminogen activator inhibitor-1 PAI-1), as well as platelet-monocyte conjugates in cardiac patients undergoing elective angiography in a double-blind, randomised parallel group study.
Patients (men, 50-70 years old; n = 12) were randomised to receive either iohexol (Omnipaque; n = 6) or iodixanol (Visipaque; n = 6) during elective angiography at Raigmore Hospital, Inverness, UK. Arterial and venous blood samples were drawn prior to CA delivery and following angiography. Assessment of platelet-monocyte conjugation, t-PA and PAI-1 antigen and activity was conducted in samples pre- and post-angiography.
Plasma t-PA antigen was depressed equally in the study groups after angiography, but there was a greater reduction in PAI-1 antigen in the group receiving iodixanol. These findings corresponded to a substantial reduction in t-PA activity in patients receiving iohexol, with no change in those receiving iodixanol (P = 0.023 between the CA groups). Both CAs caused a reduction in platelet-monocyte conjugation, with no difference between the groups. No adverse events were reported during the trial.
Avoiding reduced plasma t-PA activity might be an important consideration in choosing iodixanol over iohexol in patients at risk of thrombosis following angiography. The trial is registered on the ISRCTN register (ISRCTN51509735) and funded by the Coronary Thrombosis Trust and National Health Service (Highland) R&D Endowments. Effective self-care is an important factor in the successful management of patients with heart failure (HF). Despite the importance of self-care, most patients with HF are not adequately taught the ...wide range of skills required to become proficient in self-care. Digital health technology (DHT) may provide a novel solution to support patients at home in effective self-care, with the view to enhancing the quality of life and ultimately improving patient outcomes. However, many of the solutions developed to date have failed to consider users' perspectives at the point of design, resulting in poor effectiveness. Leveraging a human-centered design (HCD) approach to the development of DHTs may lead to the successful promotion of self-care behaviors in patients with HF.
This study aimed to outline the HCD, development, and evaluation process of a DHT designed to promote effective self-care in patients with HF.
A design thinking approach within the HCD framework was undertaken, as described in the International Organization for Standardization 9241-210:2019 regulations, using a 5-step process: empathize, ideate, design, develop, and test. Patients with HF were involved throughout the design and evaluation of the system. The designed system was grounded in behavior change theory using the Theoretical Domains Framework and included behavior change techniques. Mixed methods were used to evaluate the DHT during the testing phase.
Steps 1 to 3 of the process resulted in a set of evidence- and user-informed design requirements that were carried forward into the iterative development of a version 1 system. A cross-platform (iOS and Android) mobile app integrated with Fitbit activity trackers and smart scales was developed. A 2-week user testing phase highlighted the ease of use of the system, with patients demonstrating excellent adherence. Qualitative analysis of semistructured interviews identified the early potential for the system to positively influence self-care. Specifically, users perceived that the system increased their confidence and motivation to engage in key self-care behaviors, provided them with skills and knowledge that made them more aware of the importance of self-care behaviors, and might facilitate timely help seeking.
The use of an HCD methodology in this research has resulted in the development of a DHT that may engage patients with HF and potentially affect their self-care behaviors. This comprehensive work lays the groundwork for further development and evaluation of this solution before its implementation in health care systems. A detailed description of the HCD process used in this research will help guide the development and evaluation of future DHTs across a range of disease use cases.
Background ECG data quality can be affected by lead misplacement which can affect clinical decisions. VI and V2 are commonly misplaced in the superior direction from their correct position, which can ...mimic or conceal abnormalities. The aim of the current study is to use artificial intelligence (AI) in the form of deep learning to detect VI and V2 lead misplacement to enhance ECG data quality and to make the black box decisions of AI systems more transparent by providing AI attention maps. Methods VI and V2 signals were collected from 453 patients (normal n = 151, Left Ventricular Hypertrophy (LVH) n = 151, Myocardial Infarction n = 151) and extracted using a high-resolution body surface potential maps (BSPM) and converted into RGB images. A deep convolutional neural network (CNN) with 68 layers was developed and trained to classify the ECG images of VI and V2 into correct and incorrect placement. An attention map was generated and analyzed for each ECG image in the last convolution layer to show the most important features (see Fig. 1) that have been selected by the CNN. CNN has been trained on 67% of the data and tested on 33%. Results Using CNN with 68 layers, the accuracy of detecting lead misplacement was 92.6% (TN = 291/300, TP = 265/300, FP = 9/ 300, FN = 35/300). Based on attention maps, P waves (56%), T waves (55%) and R (48%) waves contributed the most to the predicted classes correct and incorrect. The S wave was not considered important in most cases in detecting correct VI and V2 placement. The other features, including the PR interval, Q, wave and J point contributed 29%, 17% and 27% respectively to the predicted classes correct and incorrect. Conclusions Deep CNN achieved a high accuracy (92.6%) to detect VI and V2 lead misplacement, whilst increasing the transparency of the algorithmic decision making. Attention maps demonstrate what the algorithm 'looked at' prior to making it's decision, which also elucidate areas of the ECG that are most important in detecting lead misplacement. Physicians can use the attention map to calibrate their trust with the machine and to consider the machine's attention (a proxy for machine rationale). According to the generated attention maps, the P waves, T waves and R waves were considered the most important features, while the S wave was considered as the least important feature. Whilst the other features PR interval, Qwave and J point are considered as mid-level features.

Implantable cardioverter defibrillators (ICDs) and cardiac resynchronisation therapy with defibrillators (CRT-D) reduce mortality in certain cardiac patient populations. However, inappropriate shocks ...pose a problem, having both adverse physical and psychological effects on the patient. The advances in device technology now allow remote monitoring (RM) of devices to replace clinic follow up appointments. This allows real time data to be analysed and actioned and this may improve patient care.
To determine if RM in patients with an ICD is associated with fewer inappropriate shocks and reduced time to medical assessment.
This was a single centre, retrospective observational study, involving 156 patients implanted with an ICD or CRT-D, followed up for 2 years post implant. Both appropriate and inappropriate shocks were recorded along with cause for inappropriate shocks and time to medical assessment.
RM was associated with fewer inappropriate shocks (13.6% clinic
