Acute mountain sickness (AMS) occurs in non-acclimatized people after an acute ascent to an altitude of 2,500 m or higher. The aim of this study was to examine the incidence and severity of AMS and ...associated symptoms in children.
The prospective observational study included 197 healthy, non-acclimatized 11 and 12-year-old children trekking the round-trip from the trailhead to the summit of Xue Mountain, Taiwan (2,179 m to 3,886 m) over 3 days. AMS was evaluated at Qika Hut (2,460 m) on Day 1, at Sanliujiu Hut on Day 2 (3,100 m), and at the same altitude (3,100 m) after reaching the summit on Day 3. We used the Lake Louise Score (LLS) to diagnose AMS and record daily AMS-associated symptoms. We gave acetazolamide to children with mild to moderate AMS. Dexamethasone was reserved for individuals suffering from severe AMS. Acetaminophen was administrated to children with headache, and metoclopramide for those with nausea or vomiting.
There were 197 subjects eligible for analysis. The overall incidence of AMS was 40.6%, which was higher in males and in subjects with a higher body mass index (BMI) (p < 0.05). The prevalence of AMS on Day 1 was 5.6%, which was significantly lower than that on Day 2 (29.4%) and Day 3 (23.4%). The mean LLS of all subjects was 1.77 ± 2.08. The overall incidence of severe AMS (LLS ≥ 5) was 12.5%. The mean LLS of the AMS group (3.02 ± 2.46) was significantly higher than that of the non-AMS group (0.92 ± 1.16, p < 0.001). Among the AMS group, the mean LLS was 1.00 ± 1.55 on Day 1, 4.09 ± 1.97 on Day 2, and 3.98 ± 2.42 on Day 3. The most common symptom was sleep disturbance followed by dizziness, and headache. The prevalence of headache was 46.2% on Day 2 at 3,100 m, and 31.3% on Day 3 at the same altitude after climbing the summit (3,886 m). Males experienced significantly more headache and fatigue than females (p < 0.05). The LLS and prevalence of all AMS symptoms were significantly higher in the AMS than the non-AMS group (p < 0.05).
The AMS incidence among children trekking to Xue Mountain was 40.6%. AMS is common and mostly manifests as mild symptoms. Gender (male) and a higher BMI could be considered two independent risk factors of higher AMS incidence. Sleep disturbance is the most common symptom, and the lower prevalence of headache on Day 3 may be due to the effects of medication and/or acclimatization.

The integration of III-V and Si multi-junction solar cells as photovoltaic devices has been studied in order to achieve high photovoltaic conversion efficiency. However, large differences in the ...coefficients of thermal expansion and the lattice parameters of GaAs, Si, and InGaAs have made it difficult to obtain high-efficiency solar cells grown as epilayers on Si and InP substrates. In this paper, two types of devices, including GaInP/GaAs stacked on Si (GaInP/GaAs//Si) and GaInP/GaAs stacked on InGaAs (GaInP/GaAs//InGaAs), are fabricated via mechanical stacking and wire bonding technologies. Mechanically stacked GaInP/GaAs//Si and GaInP/GaAs//InGaAs triple-junction solar cells are prepared via glue bonding. Current-voltage measurements of the two samples are made at room temperature. The short-circuit current densities of the GaInP/GaAs//Si and GaInP/GaAs//InGaAs solar cells are 13.37 and 13.66 mA/cm
, while the open-circuit voltages of these two samples are measured to be 2.71 and 2.52 V, respectively. After bonding the GaInP/GaAs dual-junction with the Si and InGaAs solar cells, the conversion efficiency is relatively improved by 32.6% and 30.9%, respectively, compared to the efficiency of the GaInP/GaAs dual-junction solar cell alone. This study demonstrates the high potential of combining mechanical stacked with wire bonding and ITO films to achieve high conversion efficiency in solar cells with three or more junctions.

Although three-dimensional finite-element analysis has been introduced to foundation design for years, simplified computer-based calculations based on the spring analogy of soils and structural ...elements are also frequently used in many engineering projects. Using coefficients of subgrade reactions and pile stiffness is a straightforward way to design and analyze a piled raft foundation. This paper discusses the determination of the coefficients of pile reactions for embedded piled raft foundations in soft clays subjected to vertically uniform loads through a series of finite-element analyses. The geologic property was assumed to be composed of homogeneous soft clays with the shear-wave velocities varying at 100–140 m/s. The foundations with a square raft ranging from 16 to 36 m and embedment depths of 8 and 12 m were studied. Load intensity approximating 1.5 times the excavated soil pressures was considered where the maximum uniform load was applied to the foundation. The foundation settlements and reactions of the piles were obtained first from a three-dimensional finite-element analysis. For design convenience, the dimensionless pile displacements, pile reactions, and coefficients of pile reactions were developed and presented in plots. The embedment depth-to-raft size ratio, pile slenderness ratio, and pile-to-pile spacing distance-to-pile diameter ratio were examined to find their optimized correlation. Finally, a design example was given to verify the proposed suggestions. We conclude that the proposed method can be an alternative to the piled raft foundation design.
Climate change gives rise to rapid degradation of rural soils in sloping subtropical and tropical areas and might further threaten environmental sustainability. In this study, we conducted an ...integrated evaluation of the effects of wood biochar (WB) application mixed with a green waste dreg compost (GWC) on runoff quality, soil losses, and agricultural productivity for a highly weathered tropical soil. A conventional agriculture method, in which soils are treated with anionic polyacrylamide (PAM), was also conducted for comparison. The amounts of runoff and soil loss, and nutrient retention were evaluated a year after WB application. Soil fertility was also investigated through a year pot experiment with rape (Brassica campestris L.) cultivation. Our results showed that the WB application not only effectively increased soil pH, soil organic carbon (SOC) and exchangeable K+ but also increased the production of rape plants. Significant reduction of runoff and the increases of inorganic nitrogen (IN) and total phosphorus (TP) were found in the WB-treated soil. Compared to the control, the co-application of WB and GWC, particularly for the WB at 4%, decreased runoff by 16.8%, soil loss by 25%, and IN loss (via runoff) by 41.8%. Meanwhile, compared to the control and PAM treatments, the co-application of WB and GWC improved soil acidity and the contents of SOC, IN, TP, and exchangeable K+. The co-application of WB and GWC could be an alternative agricultural strategy to obtain benefits to agricultural productivity and environmental sustainability.

The effects of biogeographical separation and parent material differences in soil bacterial structure and diversity in offshore islands remain poorly understood. In the current study, we used ...next-generation sequencing to characterize the differences in soil bacterial communities in five offshore subtropical granite islands (Matsu Islets, MI) of mainland China and two offshore tropical andesite islands (Orchid OI and Green Islands GI) of Taiwan. The soils of OI and GI were more acidic and had higher organic carbon and total nitrogen content than MI soils. The bacterial communities were dominated by Acidobacteria and Proteobacteria but had different relative abundance because soils were derived from different parent material and because of geographic distance. Non-metric multi-dimensional scaling revealed that the communities formed different clusters among different parent material and geographically distributed soils. The alpha-diversity in bacterial communities was higher in tropical than subtropical soils. Mantel test and redundancy analysis indicated that bacterial diversity and compositions of OI and GI soils, respectively, were positively correlated with soil pH, organic carbon, total nitrogen, microbial biomass carbon and nitrogen. These results suggest that variations in soil properties of offshore islands could result from differences in soil parent material. Distinct soils derived from different parent material and geographic distance could in turn alter the bacterial communities.

The pathological identification of lymph node (LN) metastasis is demanding and tedious. Although convolutional neural networks (CNNs) possess considerable potential in improving the process, the ...ultrahigh-resolution of whole slide images hinders the development of a clinically applicable solution. We design an artificial-intelligence-assisted LN assessment workflow to facilitate the routine counting of metastatic LNs. Unlike previous patch-based approaches, our proposed method trains CNNs by using 5-gigapixel images, obviating the need for lesion-level annotations. Trained on 5907 LN images, our algorithm identifies metastatic LNs in gastric cancer with a slide-level area under the receiver operating characteristic curve (AUC) of 0.9936. Clinical experiments reveal that the workflow significantly improves the sensitivity of micrometastasis identification (81.94% to 95.83%, P < .001) and isolated tumor cells (67.95% to 96.15%, P < .001) in a significantly shorter review time (-31.5%, P < .001). Cross-site evaluation indicates that the algorithm is highly robust (AUC = 0.9829).
Because of its hallucinogenic and dissociative effects, ketamine is often abused for recreational purposes. Thus, the seizure of ketamine manufacturing units is crucial for preventing drug abuse. The ...precursors popularly used for ketamine synthesis include 1-(2-chlorophenyl)(methylimino)methylcyclopentanol hydrochloride and 2-(2-chlorophenyl)-2-nitrocyclohexanone (2-CPNCH). Herein, we report a case of the seizure of a ketamine manufacturing unit by law enforcement officers. The seized materials were sent to our laboratory for confirmation. We found that 2-CPNCH was used as the precursor. Using zinc powder and formic acid, 2-CPNCH was reduced to norketamine. Through the Eschweiler–Clarke reaction, norketamine was reacted with formaldehyde and formic acid to synthesize ketamine; the advantages of this process are a short duration of reaction and the requirement of small amounts of chemicals. We further identified an impurity (N-methyl ketamine), which was used as a marker to validate this new process of ketamine synthesis. To the best of our knowledge, this study is the first to report illegal ketamine synthesis through the Eschweiler–Clarke reaction when using 2-CPNCH as the precursor. Our findings inform law enforcement officers and forensic practitioners about this new process of ketamine synthesis.

Antibodies have been developed as therapeutic agents for the treatment of cancer, infection, and inflammation. To identify the optimal glycan structures for individual antibodies with desired activity, we have developed an effective method to modify the Fc-glycan structures to a homogeneous glycoform. In this study, it was found that the biantennary N-glycan structure with two terminal alpha-2,6-linked sialic acids is a common and optimized structure for the enhancement of antibody-dependent cell-mediated cytotoxicity, complement-dependent cytotoxicity, and antiinflammatory activities.

Fast Radio Bursts (FRBs) are bright millisecond-duration radio transients that appear about 1000 times per day, all-sky, for a fluence threshold 5 Jy ms at 600 MHz. The FRB radio-emission ...physics and the compact objects involved in these events are subjects of intense and active debate. To better constrain source models, the Bustling Universe Radio Survey Telescope in Taiwan (BURSTT) is optimized to discover and localize a large sample of rare, high-fluence, and nearby FRBs. This population is the most amenable to multi-messenger and multi-wavelength follow-up, which allows a deeper understanding of source mechanisms. BURSTT will provide horizon-to-horizon sky coverage with a half power field-of-view (FoV) of ∼10

, a 400 MHz effective bandwidth between 300 and 800 MHz, and subarcsecond localization, which is made possible using outrigger stations that are hundreds to thousands of km from the main array. Initially, BURSTT will employ 256 antennas. After tests of various antenna designs and optimizing the system’s performance, we plan to expand to 2048 antennas. We estimate that BURSTT-256 will detect and localize ∼100 bright (≥100 Jy ms) FRBs per year. Another advantage of BURSTT’s large FoV and continuous operation will be its greatly enhanced monitoring of FRBs for repetition. The current lack of sensitive all-sky observations likely means that many repeating FRBs are currently cataloged as single-event FRBs.
