To identify risk factors present at admission in adult patients hospitalised due to influenza virus infection during the 2009/10 and 2010/11 seasons--including whether infection was from pandemic or ...seasonal influenza A infections--that were associated with the likelihood of developing severe pneumonia with multilobar involvement and shock.
Prospective cohort study. Patients hospitalised due to influenza virus infection were recruited. We collected information on sociodemographic characteristics, pre-existing medical conditions, vaccinations, toxic habits, previous medications, exposure to social environments, and EuroQoL-5D (EQ-5D). Severe pneumonia with multilobar involvement and/or shock (SPAS) was the primary outcome of interest. We constructed two multivariate logistic regression models to explain the likelihood of developing SPAS and to create a clinical prediction rule for developing SPAS that includes clinically relevant variables.
Laboratory-confirmed A(H1N1)pdm09, EQ-5D utility score 7 days before admission, more than one comorbidity, altered mental status, dyspnoea on arrival, days from onset of symptoms, and influenza season were associated with SPAS. In addition, not being vaccinated against seasonal influenza in the previous year, anaemia, altered mental status, fever and dyspnoea on arrival at hospital, difficulties in performing activities of daily living in the previous 7 days, and days from onset of symptoms to arrival at hospital were related to the likelihood of SPAS (area under the curve value of 0.75; Hosmer-Lemeshow p value of 0.84).
These variables should be taken into account by physicians evaluating a patient affected by influenza as additional information to that provided by the usual risk scores.
The understanding of planet formation has changed recently, embracing the new idea of pebble accretion. This means that the influx of pebbles from the outer regions of planet-forming disks to their ...inner zones could determine the composition of planets and their atmospheres. The solid and molecular components delivered to the planet-forming region can be best characterized by mid-infrared spectroscopy. With Spitzer low-resolution (R=100, 600) spectroscopy, this approach was limited to the detection of abundant molecules such as H2O, C2H2, HCN and CO2. This contribution will present first results of the MINDS (MIRI mid-IR Disk Survey, PI: Th. Henning) project. Due do the sensitivity and spectral resolution (R~1500-3500) provided by JWST we now have a unique tool to obtain the full inventory of chemistry in the inner disks of solar-types stars and brown dwarfs, including also less abundant hydrocarbons and isotopologues. The Integral Field Unit (IFU) capabilities enable at the same time spatial studies of the continuum and line emission in extended sources such as debris disks, the flying saucer and also the search for mid-IR signatures of forming planets in systems such as PDS70. These JWST observations are complementary to ALMA and NOEMA observations of the outer disk chemistry; together these datasets provide an integral view of the processes occurring during the planet formation phase.
Combined liver and kidney transplantation (CLKT) is an exceptional therapeutic procedure limited to a few diseases with advanced compromise of these organs. Hyperoxaluria type I and polycystic ...disease are the most frequent indications. The aim of this article was to report our indications and results of CLKT in a multicenter transplantation program in Chile.
Our Excel database was reviewed to select patients who were treated with CLKT between 1993 and July 2004.
Among 242 liver transplantations (LT) and 48 kidney transplantations (KT), 7 were CLKT, representing 2.8% of LT and 14.5% of KT. Four patients were women and 3 were male of average age 46.8 years. One patient was a child. Most frequent indications were chronic renal failure associated with terminal liver disease and polycystic disease. One patient needed liver retransplantation due to hepatic vein thrombosis. One patient had a biliary fistula and another had a urinary fistula, treated conservatively. Acute liver rejection took place in 3 cases, 1 of which required antibodies. Two patients died, 1 due to aspergillosis and the other due to vascular complications in the transplanted liver. Actuarial survival rates were 71.4% at 1 and 5 years. Chronic renal failure is not a contraindication to LT.
CLKT is an acceptable option for these patients.

A retrospective study was performed in patients diagnosed with primary lung cancer, and admitted to the Instituto Nacional de Enfermedades Respiratorias between 1984 to 1992. One thousand and ...nineteen patients were studied, 636 males and 383 females. We found a higher incidence in the group among 61–70 years of age in both sexes. The highest percentage of tumors were from the adenocarcinoma variety, followed by the epidermoid carcinoma, while the small-cell carcinoma presented a lower incidence. Smoking has been associated with the development of lung cancer. Many of our patients were smokers. The highest frequency was in smokers with a smoking habit of more than 30 years and a rate of more than 10 cigarettes per day. However, an important part of the patients with cancer did not have a smoking habit, which led us to believe that there are other etiological possibility factors (genetic or environmental) that could be involved. Most of the cases of cancer were staged as stage IIIb and stage IV and the patients had an ECOG of 1–2. The main symptoms were coughing, sputum, dyspnea and thoracic pain. This data shows an increasing frequency of lung cancer in Mexico city as well as other countries, it also shows that it is going to be a serious health problem in the future. We consider that in order to improve the prognosis, it is necessary to increase the educational and orientation campaigns among the adult population with or withou a smoking history.

Detection of the fluctuations in a 21 cm line emission from neutral hydrogen during the Epoch of Reionization in thousand hour integrations poses stringent requirements on calibration and image ...quality, both of which necessitate accurate primary beam models. The Murchison Widefield Array (MWA) uses phased‐array antenna elements which maximize collecting area at the cost of complexity. To quantify their performance, we have developed a novel beam measurement system using the 137 MHz ORBCOMM satellite constellation and a reference dipole antenna. Using power ratio measurements, we measure the in situ beampattern of the MWA antenna tile relative to that of the reference antenna, canceling the variation of satellite flux or polarization with time. We employ angular averaging to mitigate multipath effects (ground scattering) and assess environmental systematics with a null experiment in which the MWA tile is replaced with a second‐reference dipole. We achieve beam measurements over 30 dB dynamic range in beam sensitivity over a large field of view (65% of the visible sky), far wider and deeper than drift scans through astronomical sources allow. We verify an analytic model of the MWA tile at this frequency within a few percent statistical scatter within the full width at half maximum. Toward the edges of the main lobe and in the sidelobes, we measure tens of percent systematic deviations. We compare these errors with those expected from known beamforming errors.

Aims. We explore the capabilities of CARMENES for characterising hot-Jupiter atmospheres by targeting multiple water bands, in particular, those at 1.15 and 1.4 μm. Hubble Space Telescope ...observations suggest that this wavelength region is relevant for distinguishing between hazy and/or cloudy and clear atmospheres. Methods. We observed one transit of the hot Jupiter HD 189733 b with CARMENES. Telluric and stellar absorption lines were removed using SYSREM, which performs a principal component analysis including proper error propagation. The residual spectra were analysed for water absorption with cross-correlation techniques using synthetic atmospheric absorption models. Results. We report a cross-correlation peak at a signal-to-noise ratio (S/N) of 6.6, revealing the presence of water in the transmission spectrum of HD 189733 b. The absorption signal appeared slightly blueshifted at –3.9 ± 1.3 km s−1. We measured the individual cross-correlation signals of the water bands at 1.15 and 1.4 μm, finding cross-correlation peaks at S/N of 4.9 and 4.4, respectively. The 1.4 μm feature is consistent with that observed with the Hubble Space Telescope. Conclusions. The water bands studied in this work have been mainly observed in a handful of planets from space. Being able also to detect them individually from the ground at higher spectral resolution can provide insightful information to constrain the properties of exoplanet atmospheres. Although the current multi-band detections can not yet constrain atmospheric haze models for HD 189733 b, future observations at higher S/N could provide an alternative way to achieve this aim.

Current attempts to measure the 21cm Power Spectrum of neutral hydrogen during the Epoch of Reionization are limited by systematics which produce measured upper limits above both the thermal noise ...and the expected cosmological signal. These systematics arise from a combination of observational, instrumental, and analysis effects. In order to further understand and mitigate these effects, it is instructive to explore different aspects of existing datasets. One such aspect is the choice of observing field. To date, MWA EoR observations have largely focused on the EoR0 field. In this work, we present a new detailed analysis of the EoR1 field. The EoR1 field is one of the coldest regions of the Southern radio sky, but contains the very bright radio galaxy Fornax-A. The presence of this bright extended source in the primary beam of the interferometer makes the calibration and analysis of EoR1 particularly challenging. We demonstrate the effectiveness of a recently developed shapelet model of Fornax-A in improving the results from this field. We also describe and apply a series of data quality metrics which identify and remove systematically contaminated data. With substantially improved source models, upgraded analysis algorithms and enhanced data quality metrics, we determine EoR power spectrum upper limits based on analysis of the best \(\sim\)14-hours data observed during 2015 and 2014 at redshifts 6.5, 6.8 and 7.1, with the lowest \(2\sigma\) upper limit at z=6.5 of \(\Delta^2 \leq (73.78 ~\mathrm{mK)^2}\) at \(k=0.13~\mathrm{h~ Mpc^{-1}}\), improving on previous EoR1 measurement results.
