Cette deuxième partie est une mise au point qui s’intéresse aux manifestations auto-immunes satellites d’une pathologie tumorale, pouvant la révéler, en émailler le cours évolutif ou bien compliquer ...son traitement. La survenue de pathologies auto-immunes est classique au cours des syndromes lymphoprolifératifs. Les manifestations observées sont variées mais il s’agit surtout de cytopénies, notamment d’anémie hémolytique auto-immune, le plus souvent au cours d’une leucémie lymphoïde chronique. La relation entre cancers et myopathies inflammatoires n’est pas contestée. Le risque relatif de cancer au cours de la dermatomyosite est de 3,4 à 4,4. La stratégie optimale à adopter pour la recherche d’un cancer occulte n’est pas définie mais la recherche d’anticorps anti-p155 et la réalisation d’un TEP-scanner semblent être particulièrement intéressantes dans cette indication. Un tableau de vascularite cutanée ou systémique peut révéler une pathologie tumorale. Il s’agit le plus souvent d’une hémopathie (leucémie à tricholeucocytes) mais parfois d’un cancer solide (cancer pulmonaire ou digestif notamment). L’âge élevé et la corticorésistance doivent être des éléments d’alerte. Une polyarthrite cliniquement comparable à l’atteinte articulaire de la polyarthrite rhumatoïde peut révéler un cancer, le plus souvent bronchique. La polyarthrite est le plus souvent séronégative, chez un sujet masculin âgé et dans un contexte d’altération de l’état général. Les syndromes myélodysplasiques s’accompagnent de manifestations auto-immunes dans 10 à 30 % des cas. Il s’agit le plus souvent d’une vascularite ou d’une polychondrite atrophiante. Quant aux complications auto-immunes des traitements anti-tumoraux, il s’agit avant tout de cytopénies auto-immunes liées à la fludarabine et à l’alemtuzumab et aux thyroïdites liées à l’interféron alpha et à la radiothérapie cervicale.
Autoimmune diseases may reveal or occur during the course of a neoplasia or its treatment. Autoimmune cytopenia, especially haemolytic anaemia, is common in lymphoproliferative disorders such as chronic lymphoid leukemia. The link between cancer and myositis is well established. Dermatomyositis is associated with an increased relative risk of cancer of 3.4 to 4.4. A combination of detection of antibodies against p155 and TEP-computed tomography may be the best approach to ascertain the presence of occult malignancy in patients with dermatomyositis. A cutaneous or a systemic vascularitis may reveal a cancer, most often a haematological malignancy such as hairy cell leukemia. Paraneoplastic polyarthritis have been described in particular with adenocardinoma of the lungs. Underlying neoplasia should be considered in male smokers patients with new onset polyarthritis and poor health status. The prevalence of autoimmune conditions in myelodysplastic syndromes is 10 to 30%. Vasculitis and relapsing polychondritis are the most commonly reported manifestations. Immune manifestations can also be related to treatment. The most common treatment complications are autoimmune haemolytic anaemia with fludarabine and thyroiditis related to interferon and cervical radiotherapy.

Le lien entre maladie auto-immune et cancer n’est pas fortuit. Une pathologie auto-immune peut ainsi représenter le starter pour le développement ultérieur d’une pathologie lymphomateuse. Le chef de ...file de cette relation est le syndrome de Gougerot-Sjögren, avec un risque relatif de lymphome évalué jusqu’à 44. Il existe également un risque de lymphome au cours du lupus (risque relatif de 4,5) et de la polyarthrite rhumatoïde, mais qui apparaît moindre, avec un risque relatif d’environ de 2 à 3 dans les études les plus récentes. Il est actuellement bien démontré que c’est avant tout l’activité de la maladie et non les traitements immunosuppresseurs qui est l’élément déterminant de ce sur-risque. La survenue d’une tumeur solide au cours de l’évolution d’une maladie auto-immune est nettement moins documentée et concerne surtout la sclérodermie, associée notamment aux tumeurs bronchiques. Quant au risque de néoplasie induite par les immunosuppresseurs et les biothérapies, les données sont plutôt rassurantes. Il n’y a pas de risque avéré sous anti-TNFα hormis pour les carcinomes cutanés et peut-être les mélanomes, ni sous tocilizumab ou abatacept mais un recul évolutif plus important est nécessaire pour ces 2 molécules.
The link between systemic disease and cancer is not fortuitous. An autoimmune disease can represent the starter for developing a non-Hodgkin lymphoma. This is particularly true for Sjögren's syndrome that is associated with the highest risk of lymphoma (odds ratio up to 44). Other systemic autoimmune diseases concerned are systemic lupus with an odds ratio of 4.5 and rheumatoid arthritis with an odds ratio of 2 to 3. It is now well established that high inflammatory activity, rather than immunosuppressive treatment, is the major risk determinant. The association between solid cancer and autoimmune systemic disease is uncommon and concerns in particular scleroderma and lung cancer. Concerning biotherapy-induced cancers, there is no demonstrated increased risk with anti-TNFα (except for cutaneous carcinoma and maybe melanoma) or with tocilizumab and abatacept even if studies with longer follow-up are needed at least for these two last drugs.

Les anévrismes infectieux de l’aorte sont rares, représentant environ 2 % des anévrismes aortiques. La majorité des cas décrits dans la littérature proviennent de séries chirurgicales et concernent ...l’aorte abdominale. Dans ce travail rétrospectif, nous avons analysé les caractéristiques cliniques, paracliniques et évolutives de patients présentant un anévrisme infectieux de l’aorte thoracique.
Le diagnostic a été porté sur l’association d’une imagerie thoracique évocatrice et d’une documentation microbiologique ou de l’évolution favorable sous traitement anti-infectieux. Étude rétrospective de cas.
Il s’agissait de 6 hommes et une femme, d’âge moyen 66 ans. Tous présentaient au moins un facteur de risque cardiovasculaire ou de l’athérome. La fièvre (71 %) et les douleurs thoraciques (42 %) étaient les signes cliniques les plus fréquents. Les germes identifiés étaient : Salmonella enteritidis (n=3), Staphylococcus aureus (n=1), et Candida albicans (n=1). L’angioscanner montrait un aspect inflammatoire de l’aorte (n=4), un anévrisme supérieur à 50 mm (n=5) ou augmentant rapidement de diamètre (n=5). La prise en charge était médicale et interventionnelle : chirurgie (n=3) ou prise en charge endovasculaire (n=4). L’évolution était favorable pour 6 patients ; le dernier décédait de complications liées à l’anévrisme.
La présentation clinique d’un anévrisme infectieux de l’aorte thoracique est peu spécifique. Il convient d’évoquer ce diagnostic devant un anévrisme volumineux ou augmentant rapidement de diamètre, associé à un syndrome inflammatoire. Le traitement endovasculaire semble une bonne alternative à la chirurgie. La place du TEP-scanner dans le diagnostic et le suivi reste à définir.
Infectious aortic aneurysms are rare conditions, being responsible of 2% of aortic aneurysms. Most published results are surgical case series concerning infected abdominal aorta. In this retrospective study, we assessed clinical features and outcome of patients presenting infectious thoracic aortic aneurysms.
Diagnosis was based upon a combination of imaging evidence for thoracic aorta aneurysm and evidence for an infective aetiology including a culture of a causative pathogen, or a favourable outcome with anti-infective therapy. Retrospective case series.
Six men and one woman were included, with a mean age of 66 years. All the patient presented at least one cardiovascular risk factor or atherosclerosis localisation. Fever (71%) and chest pain (42%) were the most common clinical presenting manifestations. The causative pathogens were: Staphylococcus aureus (N=1), Salmonella enteritidis (N=3) and Candida albicans (N=1). The contrast-enhanced computed-tomography disclosed an aneurysm whose diameter reached more than 50 mm (N=5), that increased rapidly in size (N=5), or presented an inflammatory aspect of the aortic wall (N=4). Management was both medical and interventional: surgery (N=3) or endoluminal repair (N=4). Outcome was favourable in six patients; one patient died from aneurysm-related complications.
Clinical manifestations revealing an infectious thoracic aneurysm are variable. Diagnosis should be considered in patients presenting a rapidly-growing aneurysm, especially in the presence of elevated acute phase reactants. Endoluminal repair constitutes a treatment option. The role of FDG-PET for diagnosis and follow-up remains to be defined.

Le purpura thrombotique thrombocytopénique (PTT) est une microangiopathie thrombotique de pronostic sévère. Parmi les formes acquises de PTT, certaines sont secondaires à l’utilisation de ...médicaments. Nous rapportons une observation de PTT liée au clopidogrel.
Nous rapportons l’observation d’un PTT chez un homme de 50
ans traité par clopidogrel pour un infarctus aigu du myocarde. L’imputabilité du clopidogrel est formelle, car le test de réintroduction était positif. Les signes de microangiopathie thrombotique apparaissaient une dizaine de jours après le début du traitement par clopidogrel et les signes neurologiques étaient prédominants. Les dosages de l’activité ADAMTS 13, métalloprotéase du facteur von Willebrand, étaient effondrés et la recherche d’un inhibiteur plasmatique était positive lors des deux épisodes. Malgré 24 plasmaphérèses, l’évolution initiale n’était pas favorable et seule l’introduction de la corticothérapie permettait la rémission complète.
Bien que rares, les formes secondaires à la prise de médicaments doivent être systématiquement recherchées chez tout patient hospitalisé pour un PTT. Le mécanisme reste mal connu, mais notre observation plaide en faveur d’une étiologie immunologique. Les cliniciens doivent être informés de cet effet indésirable car la précocité du traitement est synonyme de succès thérapeutique.
Thrombotic thrombocytopenic purpura (TTP) is a life-threatening disorder. Drug-induced TTP is uncommon and we report a TTP associated with the use of clopidogrel.
We report a 50-year-old man who presented with acute myocardial infarction and received clopidogrel therapy. He developed acute TTP ten days after clopidogrel onset. Imputability of the drug was demonstrated during a reintroduction test. Deficiency of ADAMTS 13 was confirmed and autoantibodies against ADAMTS 13 were detected. Complete remission was obtained after 24 plasma exchange sessions and adjunction of corticosteroids.
Drug-induced TTP are probably immunologic, as was demonstrated in our patient. Clinicians should be aware of this possible uncommon adverse effect of clopidogrel because prompt therapy is imperative for life saving.

Plusieurs cas de thrombose ont été décrits dans la maladie cœliaque. Dans ce travail, nous avons analysé les caractéristiques cliniques et les facteurs de thrombophilie de sept patients adultes ayant ...présenté une thrombose associée à une maladie cœliaque.
Il s’agit d’une série rétrospective de sept observations de thrombose parmi une cohorte de 87 patients ayant une maladie cœliaque révélée à l’âge adulte. Les facteurs de thrombophilie ont été répertoriés et une recherche d’anticorps antiphospholipides a été faite de manière rétrospective sur le sérum des sept patients.
La prévalence globale des thromboses était de 8 %, celle des thromboses spontanées de 5,7 %, avec des thromboses veineuses (
n

5) ou artérielles (
n
=
1) ou l’association des deux (
n
=
2). Il s’agissait de six femmes et d’un homme d’âge moyen 44,8
ans au moment de la thrombose. Les manifestations thrombotiques précédaient le diagnostic de maladie cœliaque dans quatre cas. Dans trois cas, la thrombose était de siège inhabituel : veine porte (

2), veine splénique (
n
=
1). Des facteurs de risque de thrombose étaient identifiés dans six cas et pouvaient tous être reliés à la maladie cœliaque : hyperhomocystéinémie (

1), déficit en protéine C et S (
n
=
3) liés à une carence en vitamine K et anticorps antiphospholipides (
n
=
2).
Des facteurs de risque de thrombose peuvent être acquis au cours de la maladie cœliaque par carence vitaminique ou anticorps antiphospholipides. Il faut rechercher ces facteurs, les corriger, voire mettre en place une prophylaxie thromboembolique.
Thrombotic events may occur in celiac disease. In this study, we analyzed clinical features and risk factors for thrombosis in seven patients who had celiac disease and thrombosis.
We retrospectively studied 87 patients with adult celiac disease and identified seven cases of thrombosis. We searched if risk factors for thrombosis were identified and tested retrospectively antiphospholipid antibodies on the serum.
In our study, the global prevalence of thrombosis was 8 %, and 5.7 % for spontaneous thrombosis, with venous thrombosis (
n
=
5) or arterial thrombosis (
n
=
1) or both (
n
=
2). The seven patients consisted in six women and one man with a mean age of 44.8
years at time of thrombosis. Thrombotic events occurred before the diagnosis of celiac disease in four cases. In three cases, venous thrombosis was in unusual sites: portal (
n
=
2), splenic vein thrombosis (
n
=
1). In six cases, we identified risk factors for thrombosis, which could be linked to celiac disease: hyperhomocysteinemia (
n
=
1), protein C and S deficiency due to vitamin K deficiency (
n
=
3) and antiphospholipid antibodies (
n
=
2).
Such risk factors for thrombosis should be identified in patients in adult celiac disease in order to correct them and add a thromboembolic prophylaxis.

Summary
Background
Patients with thrombophilia and/or a history of venous thromboembolism (VTE) are at risk of thrombosis during pregnancy. A risk score for pregnancies with an increased risk of VTE was previously described by our group (Lyon VTE score).
Objectives
The aim of this prospective study was to assess the efficacy and safety of our score‐based prophylaxis strategy in 542 pregnancies managed between 2005 and 2015 in Lyon University Hospitals.
Patients/Methods
Of 445 patients included in the study, 36 had several pregnancies during the study period. Among these 445 patients, 279 had a personal history of VTE (62.7%), 299 patients (67.2%) had a thrombophilia marker, and 131 (29.4%) thrombophilic women had a personal history of VTE. During pregnancy, patients were assigned to one of three prophylaxis strategies according to the risk scoring system.
Results
In the antepartum period, low molecular weight heparin (LMWH) prophylaxis was prescribed to 64.5% of patients at high risk of VTE. Among them, 34.4% were treated in the third trimester only, and 30.1% were treated throughout pregnancy. During the postpartum period, all patients received LMWH for at least 6 weeks. Two antepartum‐related VTEs (0.37%; one with a score of < 3 and the other with a score of > 6) and four postpartum‐related VTEs (0.73%; three with scores of 3–5 and one with a score of > 6) occurred. No case of pulmonary embolism was observed during the study period. The rate of bleeding was 0.37%. No serious bleeding requiring transfusions or surgery occurred during the study period.
Conclusion
The use of a risk score may provide a rational decision process to implement safe and effective antepartum thromboprophylaxis in pregnant women at high risk of VTE.
