Authors Elkbuli et al. describe the decline in the twitter engagement of the Eastern Association for the Surgery of Trauma (EAST) Annual Scientific Assembly conference-specific hashtag ...#EAST2016-2020. 1 However, a decreased social media engagement in #EAST2019-2020 cannot be entirely attributed to social media/Twitter fatigue. 1 The authors have attempted to capture the engagement of the conference by evaluating a static hashtag in a dynamic digital platform with changing algorithms. While there are a few nano influencers in the surgical scientific community (those with 1000 to 10 000 followers), social media is becoming a studied strategic endeavor that requires platform knowledge to optimize results. Dissemination of scientific messages is more successful when connected accounts are actively involved in social media activity, and social media posts constitute the right combination of components. 4 Multivariable logistic regression analysis, using multimedia, URL, or hashtags, and mentioning other Twitter account(s) were independently associated with retweet success.

This commentary discusses the social impact of bowel management programs (BMPs) on children with colorectal diseases, including anorectal malformations (ARM), Hirschsprung disease (HD), functional ...constipation (FC), and spina bifida. Previous studies focused on functional outcomes, but this study bridges the gap to daily life experiences.
The study examined children's experiences in BMPs, focusing on school participation, vacation ability, and overall patient experience. Cleanliness, defined as fewer than one stool soiling episode per week, was achieved by 70% of participants.
Positive patient experiences were linked to achieving stool cleanliness, regardless of the management method. Invasive methods like enemas did not negatively affect experiences if cleanliness was maintained. Validated patient-reported experience measures (PREMs) and patient-reported outcomes measures (PROMs) were used, though the median age of 8.9 years posed limitations.
The commentary highlights the significance of stool cleanliness in improving patient experiences and supports the effectiveness of various BMP methods. Future research should include longitudinal follow-ups to assess BMP durability and gather data from older children.

To provide a high-fidelity, animal tissue-based model for the advanced surgical simulation of a Posterior Sagittal Anorectoplasty (PSARP) for rectovestibular fistula in anorectal malformation (ARM).
...A chicken cadaver was used to assess the feasibility of simulating a PSARP for rectovestibular fistula in ARM. No modification was required to implement the surgical simulation.
A detailed description of the high-fidelity surgical simulation model is provided. The PSARP can be simulated while providing realistic anatomy (e.g. common wall between rectovestibular fistula and vagina), adequate rectal size, location and placement of the rectovestibular fistula, and proximity to the vagina. Haptic conditions of the tissue resemble human tissue and operative conditions as well.
Concerning the decreased exposure of index cases of pediatric surgical trainees and pediatric surgeons in practice, simulation-based training can provide means to acquire or maintain the necessary skills to perform complex surgical procedures 1–5
Surgical simulation models for ARM are limited. Few low-cost trainers are available with predominant artificial and mostly unrealistic tissue 6–8 Animal models have the advantage of realistic multilayer tissue haptic feedback 6.
We provide a low-cost, high-fidelity model for correcting a rectovestibular fistula in a child with ARM, a complex operative procedure with low incidence but high-stake outcomes. The described tissue model utilizing the chicken cloaca anatomy provides a high-fidelity model for operative correction of rectovestibular ARM. For simulation purposes in the treatment of ARM, this model appears to be promising in terms of providing realistic pathology and haptic feedback in pediatric dimensions.

Standardization of surgical instrument trays and doctor preference cards (DPC) are known to reduce the cost of adult surgical cases. The practice in pediatric surgery may be more complex owing to a ...wide range of patient age, leading to difficulty with practice implementation and loss of potential financial savings, which underscore the importance of the review of this topic.
A systematic review of pediatric surgical tray standardization and cost-effectiveness was performed. Original and review articles from 2000 to 2018 were extracted from MEDLINE (via PubMed), Embase, Cinahl, Cochrane, and an electronic search through Scopus. After screening by inclusion and exclusion criteria, articles were selected and reviewed.
Five articles were included. On average, discontinuation of disposable instruments and standardization of equipment resulted in a removal of 40%–70% of surgical instruments per set. This yielded a cost savings of 20% (an average US $200), with no intraoperative complications or perceived safety issues.
Standardization of operating room (OR) doctor preference cards (DPC) and surgical instrument trays in pediatric surgical cases result in lower operative supply costs without impacting OR time or safety.

Appendicitis is one of the most common surgical pathologies in children. It can present with right lower quadrant pain. Scoring systems in combination with selective imaging and surgical examination ...will diagnose most children with appendicitis. Clinical pathways should be used. Most surgical interventions for appendicitis are now almost exclusively laparoscopic, with trials demonstrating better outcomes for children who undergo index hospitalization appendectomies when perforated. Nonoperative management has a role in the treatment of both uncomplicated and complicated appendicitis. This article discusses the workup and management, modes of treatment, and continued areas of controversy in pediatric appendicitis.
Little is known about psychosocial and behavioral factors that impact the quality of life of patient's with anorectal malformations (ARM) and Hirschsprung disease (HSCR). We aimed to highlight the ...psychosocial, emotional, and behavioral themes that affect these patients.
A qualitative literature review of articles published between 1980 and 2019 was performed. Articles that reported quality of life (QoL) measures not directly related to bowel function and incorporated data on patients aged 0–21 years old were included. Data were separated based on distinct developmental time points.
In the neonatal period, parents relayed uncertainty about the future and feeling overwhelmed by lack of social support. Difficulties with anxiety, peer rejection, and behavioral problems were noted in primary grades, while adolescents experienced low self-confidence, poor body image, and depression. Young adults expressed hesitancy to engage in romantic relationships or sexual activity. Lack of long-term follow-up, an incomplete transition to adult healthcare, and lack of psychology services leave young adults without guidance to manage a chronic condition.
Multiple psychosocial stressors are present in the lives of ARM and HSCR patients. Provision of developmentally matched medical, psychological, and community-based supports for ARM and HSCR patients and their families can lead to improved QoL.
