To evaluate the Pattern of Oncologic Emergencies seen in Adult cancer patients and the treatment modalities used.
Between January 2004 and December 2008, a total of 1824 (M:F = 1:1.8) new patients ...were seen. 196 (M:F = 1:1.4) consecutive patients with histologically confirmed malignancies presenting with or having oncologic emergencies were treated and have been reviewed. Patients' folders were reviewed retrospectively with a structured pro forma. Results were analysed using Epi Info soft ware Version 3.4.1; 2007 Edition.
The median age was 49 years and mean age of 42 years (range, 15 - 82 years). M: F = 1:1.4. 162 patients had oncologic emergencies at presentation while 21 during treatments and 13 during follow up. At the time of diagnosis of oncologic emergency, 126 were not on any treatment, 42 patients on hormonal therapy and 28 patients were on diverse chemotherapy. All the patients presented late with 108 patients presenting with metastatic disease and 88 patients with locally advanced disease. Only 35 patients were treated within 1 week of onset of emergency. 59 patients had cervical cancer, 31 patients with breast cancer and 28 patients with prostate cancer. Tumour haemorrhage wass the commonest oncologic emergency seen in 107 patients followed by bone pain with imminent cord compression from bone metastases in 59 patients. Of 107 patients with tumour haemorrhage, 54 patients had cardiovascular collapse with 7 having acute renal failure. Similarly, of the 107 with tumour haemorrhage, 56 patients bled from cervical cancer, 12 patients from breast cancer and 8 patients from urinary bladder. 129 patients were treated with teletherapy, 31 patients had chemotherapy, 27 patients had emergency surgery and 5 patients had chemoradiation. Oncologic emergencies were corrected in 126 patients.
Tumour haemorrhage is the commonest oncologic emergency in this environment and teletherapy is the commonest therapy used. More radiotherapy centres are needed for prompt treatment and their usefulness in managing emergencies should be made known. Oncologic emergencies are commonly seen in metastatic and locally advanced disease.
The first nuclear reactor in Nigeria, the Nigeria Research Reactor-1 (NIRR-1), is a Miniature Neutron Source Reactor (MNSR). It was specifically acquired for elemental analysis by the neutron ...activation analysis (NAA) technique. In this work, routine experimental schemes for the determination of over 30 elements of interest in different sample matrices have been developed. In order to validate the experimental procedures the following standard reference materials, IAEA-359 (Cabbage), IAEA-336 (Lichen), GSR-5 and GSD-11 (Chinese Geochemical Rock Standards), as well as IAEA-SL-3 and IAEA-405 (Sediments) were analyzed. Validated results are presented for the following elements Al, As, Au, Ba, Br, Ca, Ce, Cl, Co, Cr, Cs, Cu, Dy, Eu, Fe, Ga, Hf, In, K, La, Lu, Mg, Mn, Na, Rb, Sb, Sc, Sm, Ta, Tb, Th, Ti, U, V, Yb and Zn. To further assess analytical capabilities of the facilities, the detection limits are presented.

Primary chest wall tumors are uncommon and constitute 0.2-2% of all tumors. Metastatic tumors and tumors of local extension are more common. Malignant peripheral nerve sheath tumor (MPNST) of the ...chest wall is even rarer and its incidence on the chest wall not stated in the literature. The incidence in the general population is 0.0001% while the risk is approximately 4600 times higher in patients with type I neurofibromatosis and 3-13% of them will finally develop into MPNST, usually after latent periods of 10-20 years. Clinically, these tumors are aggressive, locally invasive, and highly metastatic. Excision of giant chest wall tumor leaves a defect that is reconstructed using musculocutaneous flaps with or without a mesh. We report the case of a 24-year-old man who presented at the surgical outpatient clinic with 7 months history of persistent left sided chest pain minimally relieved by analgesics, 5 months of cough and worsening dyspnoea, and 3 months history of anterior chest swelling on the left side of the manubrium. Following evaluation and investigations, the tumor was excised and the residual defect closed with methylmetacrylate sandwiched between two prolene meshes and overlaid with both pectoralis major muscles. The histology of the excised mass revealed MPNST He made an uneventful postoperative recovery, but died barely 3 months later from widespread pulmonary metastases. A review of the literature revealed that such tumors hardly ever reach such large-size as in our case.

L'approche par Spinoza de la relation entre raison et foi, telle qu'elle apparaît quand on juxtapose les passages pertinents de l' Éthique et ceux du Traité théologico-politique qui décrivent la prophétie, constitue un cadre théorique susceptible de rendre compte de ce paradoxe inhérent à l'écologie moderne.
