PTSMA is performed by injection of 1 to 4 ml of 96% to 98% ethanol into the target artery in 0.5 to 1.0 ml aliquots at 1 ml/min. Selection of patients for PTSMA includes those with severe symptoms refractory to maximum medical management associated with LV outflow gradient > 50 mm Hg and basal septal thickness > 18 mm. Successful outcome following septal ablation by LV outflow gradient often reduced to < 20 mm of Hg and improved from symptomatic standpoint. Medical intervention based on the concept of total coronary risk in the asymptomatic population without cardiovascular disease (CVD) is widely advocated throughout the world.All adults above 40 ...years, without history of CVD or diabetes, and who are not on treatment for blood pressure (BP) or lipids, should be considered for a comprehensive risk assessment in primary care once every five years. Our patient is an asymptomatic obese, hypertensive, dyslipidaemic individual who is also a smoker. He has a family history of premature atherosclerotic disease. His co-morbidities include mild asthma and hepatic impairment. His estimated total cardiovascular risk according to Joint British Society risk score1 for 10 year cardiovascular event is 40% which qualifies him in the high risk category.His estimated score according to HEART score based on the SCORE project2 is 7% which refers to a 10-year risk mortality. According to HEARTSCORE, a score of more than 5 is considered to be at high risk. The following discussion aims to cut down his cardiovascular risk by having life-style changes and use of cardioprotective medication where appropriate. The present study was designed to test the hypothesis that the presence of a specific hepatobiliary disease, namely common bile duct obstruction, in the absence of other risk factors will exacerbate ...gentamicin nephrotoxicity. Furthermore, since bile duct ligation decreases urinary calcium excretion, we studied the role of calcium supplementation in the prevention of gentamicin nephrotoxicity in this model. Male Sprague-Dawley rats were allocated to sham groups and common bile duct ligation groups. Gentamicin at 40 and 100 mg per kg per day for 5 days induced a more severe azotemia in common bile duct ligation animals than in sham controls. Furthermore, higher levels of renal gentamicin were found in common bile duct ligation rats than in sham rats early in the course of therapy, at its termination and during the recovery period. Pretreatment of common bile duct ligation animals with dietary calcium supplementation significantly attenuated gentamicin nephrotoxicity and the increased renal gentamicin accumulation, whereas initiation of calcium supplementation concurrent with gentamicin administration had no salutary effect. We conclude that experimental extrahepatic cholestasis in the rat, in the absence of any other factor, potentiates gentamicin nephrotoxicity. The effect is prevented by pretreatment with dietary calcium supplementation but is not modified by concurrent administration of a high-calcium diet.

We have used human CD4+ T lymphocyte clones as primary probes to identify and isolate lambda gt11 rDNA clones that express epitopes recognized by T cells. The method that we describe here permits a ...direct survey of T cell epitope coding sequences in genomic DNA or cDNA libraries. A lambda gt11 library of Mycobacterium leprae DNA was screened with M. leprae-reactive human T cell clones as probes, allowing the isolation of a M. leprae DNA clone encoding the unidentified Ag. This DNA clone differs in restriction maps from those previously identified by antibody probes and encodes an epitope that is unique to vaccine strains of Mycobacterium bovis bacillus Calmette-Guérin and pathogenic mycobacteria. This method is generally applicable and should expedite the study of Ag and epitopes important to the T cell response in infections and in autoimmune diseases.
Leprosy is a chronic infectious disease caused by Mycobacterium leprae. As with other intracellular parasites, protective immunity is dependent on T cells and cell-mediated immunity. In animal ...models, immunization with killed armadillo-derived M. leprae elicits strong T-cell responses, delayed-type hypersensitivity and protection against viable challenge. We have recently shown that killed M. leprae can induce delayed-type hypersensitivity in healthy human volunteers. Identification of the M. leprae antigens that are recognized by T cells and may be involved in protection has been hampered by the inability to cultivate the organism in vitro and by difficulties in antigen purification from limited quantities of armadillo-derived bacillus. Because genes for the major protein antigens of M. leprae as seen by mouse monoclonal antibodies have been isolated, it has become possible to test whether these individual antigens are recognized by T cells. We screened crude lambda gtll phage lysates of Escherichia coli containing individual M. leprae antigens using M. leprae-specific T-cell clones isolated from M. leprae-vaccinated volunteers. Using this method, we find that nearly half of the M. leprae-specific T-cell clones are stimulated to proliferate by lysates containing an epitope of a M. leprae protein of relative molecular mass 18,000 (18K).
Cardiac arrhythmias often present to family physicians with diverse clinical manifestations. This retrospective observational study was carried out in a private cardiology clinic in Dhaka from July ...2004 to December 2008. A total of 1257 patients were referred from family physicians in the locality. 75 (5.96%) patients were diagnosed as having cardiac arrhythmia by ECG. Various types of atrial arrhythmias are more common than ventricular arrhythmias (60% vs 40%). Atrial fibrillation (N=18) and PVC (N=17) are the 2 commonest arrhythmias found in this study. This study identifies the clinical presentation, possible aetiology and management of patients having cardiac arrhythmias. Palpitation (46.66%) and asymptomatic ECG changes (34.66%) were the 2 most common reasons for referral. Most of the patients could be managed on a out-patient basis. Most arrhythmias like 1st degree heart block, PAC & isolated PVC, RBBB, Sinus bradycardia were asymptomatic & did not need any further treatment except assurance & anxiolytics. Other arrhythmias like atrial fibrillation, LBBB, bifascicular or advanced heart block, SSS & SVT needed further evaluation. This article particularly focuses on the general approach of family physicians while dealing with patients with cardiac arrhythmias. In 54 patients who underwent halo traction for an average of 80 days each, there was no laboratory evidence of any change in calcium metabolism. Histologically, there were slight but significant bony ...changes after termination of halo traction treatment, principally "low-turnover" osteoporosis, which was possibly more prevalent in female patients. Where a slight osteoporosis already existed prior to halo traction, it generally became more severe. The study seems to show that no relationship exists between the etiology of scolioses and the development of osteoporosis. It proves that in respect of the development of osteoporosis also, permanent halo traction is not a neutral procedure.

The authors study the performance of FDDI (fiber distributed data interface) as a backplane network connecting SUN4 workstations in a distributed environment. They used the C language and the Express ...software package, developed by Parasoft Inc., to build a distributed system and to develop distributed applications. The performance of fast Fourier transforms is studied for different problem sizes and numbers of workstations. The results are compared to those obtained on a stand-alone workstation and on Ethernet. Results confirm that FDDI can offer considerable speed-up compared to a stand-alone workstations and meets the bandwidth requirements for distributed computing across local area networks.< >
