In this interview with ChatGPT, we explore the potential impact of AI on future pediatric research. Our discussion covers a range of topics, including the potential positive effects of AI, such as improved clinical decision-making, enhanced medical education, faster drug development, and better research outcomes. We also examine potential negative effects, such as bias and fairness concerns, safety and security issues, overreliance on technology, and ethical considerations.
While AI continues to advance, it is crucial to remain vigilant about the possible risks and limitations of these technologies and to consider the implications of these technologies and their use in the medical field. The development of AI language models represents a significant advancement in the field of artificial intelligence and has the potential to revolutionize daily clinical practice in every branch of medicine, both surgical and clinical. Ethical and social implications must also be considered to ensure that these technologies are used in a responsible and beneficial manner.
We report on the analysis of NuSTAR observations of the Be-transient X-ray pulsar V 0332+53 during the giant outburst in 2015 and another minor outburst in 2016. We confirm the cyclotron-line ...energy-luminosity correlation previously reported in the source and the line energy decrease during the giant outburst. Based on 2016 observations, we find that a year later the line energy has increased again essentially reaching the pre-outburst values. We discuss this behaviour and conclude that it is likely caused by a change of the emission region geometry rather than previously suggested accretion-induced decay of the neutron stars magnetic field. At lower luminosities, we find for the first time a hint of departure from the anticorrelation of line energy with flux, which we interpret as a transition from super- to sub-critical accretion associated with the disappearance of the accretion column. Finally, we confirm and briefly discuss the orbital modulation observed in the outburst light curve of the source.
Fear and anxiety are adaptive responses but if left unregulated, or inappropriately regulated, they become biologically and socially maladaptive. Dysregulated emotions are manifest in a wide variety ...of psychiatric and neurological conditions but the external expression gives little indication of the underlying causes, which are inevitably multi-determined. To go beyond the overt phenotype and begin to understand the causal mechanisms leading to conditions characterized by anxiety and disorders of mood, it is necessary to identify the base psychological processes that have become dysregulated, and map them on to their associated neural substrates. So far, attention has been focused primarily on the medial regions of prefrontal cortex (PFC) and in particular their contribution to the expression and extinction of conditioned fear. However, functional neuroimaging studies have shown that the sphere of influence within the PFC is not restricted to its medial regions, but extends into dorsal, ventrolateral (vlPFC) and orbitofrontal (OFC) regions too; although the causal role of these other areas in the regulation of fear and anxiety remains to be determined and in the case of the OFC, existing findings are conflicting. Here, we review the evidence for the contribution of these other regions in negative emotion regulation in rodents and old world and new world monkeys. We consider a variety of different contexts, including conditioned and innate fear, learned and unlearned anxiety and cost-benefit decision-making, and a range of physiological and behavioral measures of emotion. It is proposed that both the OFC and vlPFC contribute to emotion regulation via their involvement, respectively, in the prediction of future outcomes and higher-order attentional control. The fractionation of these neurocognitive and neurobehavioral systems that regulate fear and anxiety opens up new opportunities for diagnostic stratification and personalized treatment strategies.
Variation in an upstream repetitive region at the SLC6A4 locus, which encodes the serotonin transporter, is associated with anxiety-related behaviour in a few primate species, including humans and ...rhesus macaques, and has been suggested to be related to ecological adaptability among macaques. In this study, we investigate evolution of SLC6A4 polymorphisms associated with anxiety-related behaviour in common marmosets ( Callithrix jacchus ). Assaying variation in the SLC6A4 repeat region across 14 species in eight genera of callitrichid primates (marmosets and tamarins), we find large interspecific variation in the number of repeats present (24–43). The black tufted-ear marmoset ( C. penicillata ) has sequence polymorphisms similar to those found in the common marmoset, which is its sister species, and no other species has intraspecific variation at these sites. We conclude that, similar to humans and macaques, the functional polymorphism at SLC6A4 in common marmosets has a recent evolutionary origin, and that the anxiety-related allele is evolutionarily derived. Common/black tufted-ear marmosets and rhesus/bonnet macaques share high ecological adaptability and behavioural flexibility that we propose may be related to the maintenance of the polymorphism.
Abstract We report timing and spectral studies of the high-mass X-ray binary 4U 1700-37 using Insight-HXMT observations carried out in 2020 during its out-of-eclipse state. We found significant ...variations in flux on a timescale of kilo-seconds, while the hardness (count rate ratio between 10–30 keV and 2–10 keV) remains relatively stable. No evident pulsations were found over a frequency range of 10 −3 –2000 Hz. During the spectral analysis, for the first time, we took the configuration of different Insight-HXMT detectors’ orientations into account, which allows us to obtain reliable results even if stable contamination exists in the field of view. We found that the spectrum could be well described by some phenomenological models that are commonly used in accreting pulsars (e.g., a power law with a high energy cutoff) in the energy range of 2–100 keV. We found hints of cyclotron absorption features around ∼16 keV or/and ∼50 keV.
Poor outcomes are common in individuals with anxiety and depression, and the brain circuits underlying symptoms and treatment responses remain elusive. To elucidate these neural circuits, ...experimental studies must specifically manipulate them, which is only possible in animals. Here, we used a chemogenetics strategy involving engineered designer receptors exclusively activated by designer drugs (DREADDs) to activate a region of the marmoset brain that is dysfunctional in human patients with major depressive disorder, called the subcallosal anterior cingulate cortex area 25 (scACC-25). Using this DREADDs system, we identified separate scACC-25 neural circuits that underlie specific components of anhedonia and anxiety in marmosets. Activation of the neural pathway connecting the scACC-25 to the nucleus accumbens (NAc) caused blunting of anticipatory arousal (a form of anhedonia) in marmosets in response to a reward-associated conditioned stimulus in an appetitive Pavlovian discrimination test. Separately, activation of the circuit between the scACC-25 and the amygdala increased a measure of anxiety (the threat response score) when marmosets were presented with an uncertain threat (human intruder test). Using the anhedonia data, we then showed that the fast-acting antidepressant ketamine when infused into the NAc of marmosets prevented anhedonia after scACC-25 activation for more than 1 week. These neurobiological findings provide targets that could contribute to the development of new treatment strategies.
In view of low prevalence rates, diabetes is discussed as a protective factor for the occurrence of Takotsubo syndrome (TTS). Furthermore, it was associated with improved outcome in a small ...single-center analysis. Therefore, this study assessed the prevalence and prognostic relevance of concomitant diabetes in TTS.
A total of 826 patients with TTS were enrolled in an international, multicenter, registry-based study (eight centers in Italy and Germany). All-cause mortality was compared between patients with diabetes and patients without diabetes, and the independent predictive value of diabetes was evaluated in multivariate regression analysis.
The prevalence of diabetes was 21.1% (
= 174). TTS patients with diabetes were older (
< 0.001), were more frequently male (
= 0.003), had a higher prevalence of hypertension (
< 0.001), physical triggers (
= 0.041), and typical apical ballooning (
= 0.010), had a lower left ventricular ejection fraction (
= 0.008), had a higher rate of pulmonary edema (
= 0.032), and had a longer hospital stay (
= 0.009). However, 28-day all-cause mortality did not differ between patients with diabetes and patients without diabetes (6.4% vs. 5.7%; hazard ratio HR 1.11 95% CI 0.55-2.25;
= 0.772). Longer-term follow-up after a median of 2.5 years revealed a significantly higher mortality among TTS patients with diabetes (31.4% vs. 16.5%;
< 0.001), and multivariate regression analysis identified diabetes as an independent predictor of adverse outcome (HR 1.66 95% CI 1.16-2.39;
= 0.006).
Diabetes is not uncommon in patients with TTS, is associated with increased longer-term mortality rates, and is an independent predictor of adverse outcome irrespective of additional risk factors.
Abstract Using data from the Neutron star Interior Composition ExploreR observatory, we identified a permanent spin frequency decrease of Δ ν = −(1.04 ± 0.07) × 10 −7 Hz around MJD 60132 in the ...rotation-powered pulsar PSR B0540-69, which exhibits a periodic signal at a frequency of ν ∼ 19.6 Hz. This points to an antiglitch event, a sudden decrease of the pulsar’s rotational frequency without any major alteration in the pulse profile or any significant increase of the pulsed flux. Additionally, no burst activity was observed in association with the antiglitch. To date, observations of the few known antiglitches have been made in magnetars or accreting pulsars. This is the first antiglitch detected in a rotation-powered pulsar. Given its radiatively quiet nature, this antiglitch is possibly of internal origin. Therefore, we tentatively frame this event within a proposed mechanism for antiglitches where the partial “evaporation” of the superfluid component leads to an increase in the normal component’s moment of inertia and a decrease in the superfluid one.
