Paracoccidio-domycosisis a deep mycosis due to a dimorphic fungus:Paracoccidioides brasiliensis. The principle endemic country is Brazil. We describe the first case of paracoccidio-domycosis, in its ...cutaneous-mucosal form, diagnosed in French Guiana.
A 20 year-old Brazilian man, having mover to French Guiana a few months earlier, presented with multiple disseminated cutaneous lesions, predominating on the face, and composed of multiple nodules and two ulcerations. The clinical examination also revealed voluminous superficial lymph nodes and ulcerations of the pharynx and larynx. Direct examination, anatomopathology and culture of cutaneous biopsies revealed specific images of Paracoccidioides brasiliensis. HIV serology was negative. Treatment combining cotrimoxazole and itraconazole eliminated the lesions in one month.
Because the patient had just moved to Guiana, this observation probably corresponded to an imported disease. The principle differential diagnosis was leishmaniosis.

Dans l'Europe du XIXe siècle, des virtualités nationales s'affirment et aspirent à une consécration étatique. Chaque instance nationale, reconnue ou émergente, voulait alors se fonder sur une ...tradition d'histoire, sur une inscription monumentale dans les annales politiques. Cette prétention de mémoire comportait des aspects archivistiques singuliers. La constitution d'archives nationales pouvait ainsi devenir un enjeu politique puissant, représenter une sorte d'attribut de la souveraineté. Les travaux des érudits et des historiens cherchaient à appuyer les écrits des poètes et des politiques. Plus trivialement, les réalités des archives, leur caractère massif ou résiduel, les nécessités de l'administration courante se conjuguaient avec la culture politique pour justifier l'érection de nouveaux lieux du savoir. Cette carte des nouveaux dépôts nationaux pouvait, ou non, se confondre avec celle des États. Chaque projet archivistique suscitait sans doute sa part de transferts, de démembrements, de destructions ou de sauvegardes de fonds d'archives. Des bâtiments médiocres ou solennels, plus ou moins fonctionnels, pouvaient être les traductions architecturales de ces choix. Des conséquences historiographiques en résultaient certainement. L'interaction des enjeux politiques et des évidences matérielles des fonds offraient sans doute plusieurs modèles possibles, selon les conjonctures propres à chaque nation. De nombreuses pistes de recherche sont donc ouvertes : rapports de la chronique archivistique et des histoires politiques nationales, comparaison avec les évolutions contemporaines des autres lieux de savoir (musées, bibliothèques), liens avec d'autres mouvements culturels, influences sur l'écriture de l'histoire et son enseignement, etc.
This paper reports on the LP-MOCVDgrowth optimisation of GaAlN/GaN heterostructures grown on silicon carbide substrates for HEMT applications, and on the first device performances obtained with these ...structures. The critical impact of some growth parameters on the physical properties of the GaAlN/GaN epilayers has been identified and studied using high-resolution X-ray diffraction, AFM, C2V and sonogauge measurements. SiC substrates from different suppliers were evaluated and their influence on the physical properties of the GaAlN/GaN HEMT structures were investigated. We show that the static characteristics of the devices such as maximum drain current Idss or pinch-off voltage are correlated with the nucleation layer composition ( GaN or GaAlN) of the HEMT structure and with the defect density of the SiC substrate. A maximum drain current Idss around 1 A/mm and a pinch-off voltage of -5 V have been measured for devices with a gate length of 0.3 mm and a GaN nucleation layer, to be compared to an Idss up to 1.3 A/mm obtained for devices with the same geometry but with a GaAlN nucleation layer. I2V characteristics measured under pulsed measurements have also evidenced a clear impact of the nucleation layer composition on the trap density in the GaAlN/GaN HEMT structure. The first devices related to HEMT wafers with GaN nucleation layers were measured at 10 GHz using a load pull system. They show a CW output power in excess of 2.8 W/mm for a gate length of 0.5 mm, while devices related to HEMT wafers with GaAlN nucleation layer exhibit output power up to 4 W/mm at 10 GHz

A nuclear magnetic resonance (NMR) method has been developed to monitor on‐line lipase‐catalyzed esterification reactions without the need to sample the reaction medium. The technique, through 1H ...NMR, measures the concentrations of alcohol, ester, hydroxylic hydrogens in the organic phase, and hydroxylic hydrogens in the aqueous phase, if any. Also, the chemical shift evolution of the two types of hydroxylic hydrogens has been followed, providing information on water content of the organic phase and on the appearance of a distinct aqueous phase. As far as 13C NMR is concerned, it has been possible to measure, first the acid and the ester concentrations in the carbonyl region, and second, the alcohol and the ester concentrations in the methylene region. All 1H and 13C results are in agreement with one another. Furthermore, NMR allows for the choice of detection zone. Preliminary studies on the solid phase proved the presence of much more water in the solid phase than in the organic phase, and also gave evidence of the existence of two types of esters, one in the organic phase, mainly associated with the acid, and the other one not associated with the acid, most probably entrapped within the solid enzyme.

Immune reconstitution syndrome (IRIS) is an unusual inflammatory reaction to an opportunistic infection in an HIV-positive patient. This syndrome occurs when immunity is restored in the first months ...of an effective highly active antiretroviral treatment (HAART). First, we described all patients with a cutaneous form of IRIS. Then, between 1992 and 2004 we conducted a retrospective cohort study comparing Herpes Zoster and Herpes Simplex infections among untreated patients, patients treated by HAART for < or = six months, and patients treated for > six months. We observed three cases of atypical leprosy and three original observations: two of these were fistulisation of lymph node histoplasmosis and tuberculosis, the third one reports the recurrence of a treated cutaneous leishmaniasis. Multivariate analysis showed that, after controlling for age, sex and CD4 counts, patients receiving HAART for < or = six months were more likely to develop Herpes Zoster or herpes simplex infections (p < 0.005). Herpes Simplex and Herpes Zoster infections are the two most frequent dermatological manifestations in our tropical setting. Although mycobacterial infections are more rarely observed than in visceral IRIS, the increased incidence of leprosy may be quite significant when the availability of HAART spreads to developing countries.
