We conducted a population-based cohort study in 7598 white healthy women, aged 75 years and over, recruited from the voting lists. We measured at baseline bone mineral density (BMD g/cm2) of the ...proximal femur (neck, trochanter and Ward's triangle) and the whole body, as well as fat and lean body mass, by dual-energy X-ray absorptiometry (DXA). One hundred and fifty-four women underwent a hip fracture during an average 2 years follow-up. Each standard deviation decrease in BMD increased the risk of hip fracture adjusted for age, weight and centre by 1.9 (95% CL 1.5, 2.3) for the femoral neck, 2.6 times (2.0, 3.3) for the trochanter, 1.8 times (1.4, 2.2) for Ward's triangle, 1.6 times (1.2, 2.0) for the whole body, and 1.3 times (1.0, 1.5) for the fat mass. The areas under the receiver operating characteristic (ROC) curves were not significantly different between trochanter and femoral neck BMD, whereas ROC curves of femoral neck and trochanter BMD were significantly better than those for Ward's triangle and whole-body BMD. Women who sustained an intertrochanteric fracture were older (84 +/- 4.5 years) than women who had a cervical fracture (81 +/- 4.5 years) and trochanter BMD seemed to be a stronger predictor of intertrochanteric (RR = 4.5 (3.1, 6.5) than cervical fractures (RR = 1.8 (1.5, 2.3). In very elderly women aged 80 years and more, hip BMD was still a significant predictor of hip fracture but the relative risk was significantly lower than in women younger than 80 years. In the 48% of women who had a femoral neck BMD T-score less than -2.5, the relative risk of hip fracture was increased by 3, and the unadjusted incidence of hip fracture was 16.4 per 1000 woman-years compared with 1.1 in the population with a femoral neck BMD T-score > or = -1.

The ability of ultrasonographic measurements to discriminate between patients with hip fracture and age-matched controls has until now been tested mainly through cross-sectional studies. We report ...the results of a prospective study to assess the value of measurements with ultrasound in predicting the risk of hip fracture.
5662 elderly women (mean age 80 4 years) had both baseline calcaneal ultrasonography measurements and femoral radiography (dual-photon X-ray absorptiometry, DPXA) to assess their bone quality. Follow-up every 4 months enabled us to identify incident fractures. 115 hip fractures were recorded during a mean follow-up duration of 2 years.
Low calcaneal ultrasonographic variables (obtained from measurements of broadband ultrasound attenuation by, and speed of sound through the bone) were able to predict an increased risk of hip fracture, with similar accuracy to low femoral bone mineral density (BMD) obtained by DPXA. The relative risk of hip fracture for 1 SD reduction was 20 (95% CI 16-24) for ultrasound attenuation and 17 (1 4–2 1) for speed of sound, compared with 19 (16-2 4) for BMD. After control for the femoral neck BMD, ultrasonographic variables remained predictive of hip fracture. The incidence of hip fracture among women with values above the median for both calcaneal ultrasound attenuation and femoral neck BMD was 2 7 per 1000 woman-years, compared with 19 6 per 1000 woman-years for those with values below the median for both measures.
Ultrasonographic measurements of the os calcis predict the risk of hip fracture in elderly women living at home as well as DPXA of the hip does, and the combination of both methods makes possible the identification of women at very high or very low risk of fracture.

The ε4 allele of the apolipoprotein E (APOE) gene is a prominent risk factor for Alzheimer's disease (AD), but its implication in other dementias is less well studied.
We used a data set on 2858 ...subjects (1098 AD, 260 vascular dementia VaD, 145 mixed AD and VaD, 90 other dementia diagnoses, and 1265 controls) to examine the association of APOE polymorphisms with clinical dementia diagnoses, biomarker profiles, and longevity.
The ε4 allele was associated with reduced longevity as ε4 versus ε3 homozygotes lived on average 2.6 years shorter (P = .006). In AD, ε4 carriers lived 1.0 years shorter than noncarriers (P = .028). The ε4 allele was more prevalent in AD, mixed AD and VaD, and VaD patients compared to controls, but not in other dementia disorders.
The APOE ε4 allele is influential in AD but might also be of importance in VaD and in mixed AD and VaD, diseases in which concomitant AD pathology is common.
•The ɛ4 allele is associated with reduced longevity and a more distinct Alzheimer's disease (AD)-like biomarker profile in dementia in general, and in AD in particular.•The ɛ4 allele is prominent in AD, but also in vascular dementia and in mixed vascular dementia and AD.•The ɛ4 allele is associated with an earlier disease onset in AD.•The effects of APOE genotype in frontotemporal dementia, dementia with Lewy bodies, Parkinson's disease dementia, and Creutzfeldt-Jakob disease are small.

Sleep disruption is a key clinical issue in the dementias but the sleep phenotypes of these diseases remain poorly characterised. Here we addressed this issue in a proof-of-principle study of 67 patients representing major syndromes of frontotemporal dementia (FTD) and Alzheimer’s disease (AD), in relation to 25 healthy older individuals. We collected reports on clinically-relevant sleep characteristics - time spent overnight in bed, sleep quality, excessive daytime somnolence and disruptive sleep events. Difficulty falling or staying asleep at night and excessive daytime somnolence were significantly more frequently reported for patients with both FTD and AD than healthy controls. On average, patients with FTD and AD retired earlier and patients with AD spent significantly longer in bed overnight than did healthy controls. Excessive daytime somnolence was significantly more frequent in the FTD group than the AD group; AD syndromic subgroups showed similar sleep symptom profiles while FTD subgroups showed more variable profiles. Sleep disturbance is a significant clinical issue in major FTD and AD variant syndromes and may be even more salient in FTD than AD. These preliminary findings warrant further systematic investigation with electrophysiological and neuroanatomical correlation in major proteinopathies.

Chronic wasting disease (CWD) is a transmissible prion disease first observed in the 1960s in North America. This invariably fatal disease affects multiple cervid species in the wild and in ...captivity. In addition to the several known transmission pathways involving cervid host species, prions have been detected in the feces of crows and coyotes after consumption of experimentally spiked tissues. This raises questions about the role of cervid consumers in the perpetuation of CWD. Mountain lions have been shown to preferentially select CWD-infected prey and are also apparently resistant to infection. In this study, two captive mountain lions were fed ground mule deer muscle tissue spiked with brain-derived CWD prions, and lion feces were collected for 1 week afterward. The input brain and resulting fecal materials were analyzed using the highly sensitive real-time quaking-induced conversion (RT-QuIC) assay to quantify prion seeding activity. We recovered only 2.8 to 3.9% of input CWD prions after passage through the mountain lions' gastrointestinal tracts. Interestingly, CWD prions were shed only in the first defecation following consumption. Our data support the possibility that mountain lions feeding upon infected carcasses could excrete CWD prions in their feces over a short period of time but also suggest that most of the ingested prions are eliminated or sequestered by this large predator.
CWD prions appear to spread naturally among susceptible cervid species in captivity and in the wild. A better understanding of all the ways these prions move, persist, and subsequently infect target species through the environment is critical to developing comprehensive disease control strategies. In our study, we show limited, transient pass-through of CWD prions in an apex predator, the mountain lion, using the highly sensitive RT-QuIC assay on feces collected after lions were fed prion-spiked muscle tissue. Prions were detected in feces only in the first defecation after exposure. Moreover, the amount of CWD prions recovered in feces was reduced by >96% after passing through the lion digestive system. This indicates that mountain lions may have some potential to distribute CWD prions within their home ranges but that they also effectively eliminate most of the CWD prions they consume.
There is a need for more information on the relationship between diseases and fluctuations of wild populations of marine animals. In the case of Callinectes sapidus reovirus 1 (CsRV1, also known as ...RLV), there is a lack of baseline information on range, prevalence and outbreaks, from which to develop an understanding of population‐level impacts. An RT‐qPCR assay was developed that is capable of detecting 10 copies of the CsRV1 genome. In collaboration with state, federal and academic partners, blue crabs were collected from sites throughout the north‐eastern United States to assess the northern range of this pathogen. In addition, archived crab samples from the Chesapeake Bay were assessed for CsRV1 by RT‐qPCR and histology. PCR‐based assessments indicate that CsRV1 was present at all but one site. Prevalence of CsRV1 as assessed by RT‐qPCR was highly variable between locations, and CsRV1 prevalence varied between years at a given location. Mean CsRV1 prevalence as assessed by RT‐qPCR was >15% each year, and peak prevalence was 79%. The wide geographic range and highly variable prevalence of CsRV1 indicate that more study is needed to understand CsRV1 dynamics and the role the virus plays in blue crab natural mortality.

The mean horizontal flow field of the tropical Atlantic Ocean is described between 20°N and 20°S from observations and literature results for three layers of the upper ocean, Tropical Surface Water, ...Central Water, and Antarctic Intermediate Water. Compared to the subtropical gyres the tropical circulation shows several zonal current and countercurrent bands of smaller meridional and vertical extent. The wind-driven Ekman layer in the upper tens of meters of the ocean masks at some places the flow structure of the Tropical Surface Water layer as is the case for the Angola Gyre in the eastern tropical South Atlantic. Although there are regions with a strong seasonal cycle of the Tropical Surface Water circulation, such as the North Equatorial Countercurrent, large regions of the tropics do not show a significant seasonal cycle. In the Central Water layer below, the eastward North and South Equatorial undercurrents appear imbedded in the westward-flowing South Equatorial Current. The Antarcic Intermediate Water layer contains several zonal current bands south of 3°N, but only weak flow exists north of 3°N. The sparse available data suggest that the Equatorial Intermediate Current as well as the Southern and Northern Intermediate Countercurrents extend zonally across the entire equatorial basin. Due to the convergence of northern and southern water masses, the western tropical Atlantic north of the equator is an important site for the mixture of water masses, but more work is needed to better understand the role of the various zonal under- and countercurrents in cross-equatorial water mass transfer.

Bone metastases from breast cancer are common, causing significant morbidity. Preclinical data of dasatinib, an oral small molecule inhibitor of multiple oncogenic tyrosine kinases, suggested ...efficacy in tumor control and palliation of bone metastases in metastatic breast cancer (MBC). This clinical trial aimed to determine whether treatment with either of 2 dose schedules of dasatinib results in a progression-free survival (PFS) >50 % at 24 weeks in bone metastasis predominant MBC, to evaluate the toxicity of the 2 dosing regimens, and explore whether treatment results in decreased serum bone turnover markers and patient-reported “worst pain.” Subjects with bone metastasis predominant MBC were randomly assigned to either 100 mg of dasatinib once daily, or 70 mg twice daily, with treatment continued until time of disease progression or intolerable toxicity. Planned accrual was 40 patients in each arm. The primary trial endpoint was PFS, defined as time from registration to progression or death due to any cause. Median PFS for all eligible patients (79) was 12.6 weeks (95 % CI 9.1–16.7). Neither cohort met the threshold for further clinical interest. There were no significant differences in PFS by randomized treatment arm (
p
= 0.85). Toxicity was similar in both cohorts, with no clear trend in serum biomarkers of bone turnover or patient-reported pain. Dasatinib was ineffective in controlling bone-predominant MBC in a patient population, unselected by molecular markers. Further study of dasatinib in breast cancer should not be pursued unless performed in molecularly determined patient subsets, or rational combinations.

Landsat's continuing record of the thermal state of the earth's surface represents the only long term (1982 to the present) global record with spatial scales appropriate for human scale studies ...(i.e., tens of meters). Temperature drives many of the physical and biological processes that impact the global and local environment. As our knowledge of, and interest in, the role of temperature on these processes have grown, the value of Landsat data to monitor trends and process has also grown. The value of the Landsat thermal data archive will continue to grow as we develop more effective ways to study the long term processes and trends affecting the planet. However, in order to take proper advantage of the thermal data, we need to be able to convert the data to surface temperatures. A critical step in this process is to have the entire archive completely and consistently calibrated into absolute radiance so that it can be atmospherically compensated to surface leaving radiance and then to surface radiometric temperature. This paper addresses the methods and procedures that have been used to perform the radiometric calibration of the earliest sizable thermal data set in the archive (Landsat 4 data). The completion of this effort along with the updated calibration of the earlier (1985–1999) Landsat 5 data, also reported here, concludes a comprehensive calibration of the Landsat thermal archive of data from 1982 to the present.
