The risk of adverse events from ascending thoracic aorta aneurysm (TAA) is poorly understood but drives clinical decision-making.

The Kaiser Permanente Thoracic Aortic Aneurysm (KP-TAA) cohort study was a retrospective cohort study at Kaiser Permanente Northern California, a fully integrated health care delivery system insuring and providing care for more than 4.5 million persons. Nonsyndromic patients from a regional TAA safety net tracking system were included. Imaging data including maximum TAA size were merged with electronic health record (EHR) and comprehensive death data to obtain demographic characteristics, comorbidities, medications, laboratory values, vital signs, and subsequent outcomes. Unadjusted rates were calculated and the association of TAA size with outcomes was evaluated in multivariable competing risk models that categorized TAA size as a baseline and time-updated variable and accounted for potential confounders. Data were analyzed from January 2018 to August 2021.

Aortic dissection (AD), all-cause death, and elective aortic surgery.
Of 6372 patients with TAA identified between 2000 and 2016 (mean SD age, 68.6 13.0 years; 2050 female individuals 32.2% and 4322 male individuals 67.8%), mean (SD) initial TAA size was 4.4 (0.5) cm (828 individuals 13.0% of cohort had initial TAA size 5.0 cm or larger and 280 4.4% 5.5 cm or larger). Rates of AD were low across a mean (SD) 3.7 (2.5) years of follow-up (44 individuals 0.7% of cohort; incidence 0.22 events per 100 person-years). Larger initial aortic size was associated with higher risk of AD and all-cause death in multivariable models, with an inflection point in risk at 6.0 cm. Estimated adjusted risks of AD within 5 years were 0.3% (95% CI, 0.3-0.7), 0.6% (95% CI, 0.4-1.3), 1.5% (95% CI, 1.2-3.9), 3.6% (95% CI, 1.8-12.8), and 10.5% (95% CI, 2.7-44.3) in patients with TAA size of 4.0 to 4.4 cm, 4.5 to 4.9 cm, 5.0 to 5.4 cm, 5.5 to 5.9 cm, and 6.0 cm or larger, respectively, in time-updated models. Rates of the composite outcome of AD and all-cause death were higher than for AD alone, but a similar inflection point for increased risk was observed at 6.0 cm.
In a large sociodemographically diverse cohort of patients with TAA, absolute risk of aortic dissection was low but increased with larger aortic sizes after adjustment for potential confounders and competing risks. Our data support current consensus guidelines recommending prophylactic surgery in nonsyndromic individuals with TAA at a 5.5-cm threshold.

In a multi-user cognitive radio network, there arises a need for coordination among the network users for efficient utilization of the available electromagnetic spectrum. While adaptive modulation alone helps cognitive radios actively determine the channel quality metric for the next transmission, Game theory combined with an adaptive modulation system helps them achieve mutual coordination among channel users and avoids any possible confusion about transmitting/receiving through a channel in the future. This paper highlights how the concepts of game theory and adaptive modulation can be incorporated in a cognitive radio framework to achieve better communication for telemetry applications.

Polymer solar cells (PSCs) based on binary and ternary active layers were built using PBDB-T polymer as donor and two non-fullerene acceptors (MPU2 and MPU3) with different DPP cores and terminal units but different conjugation length. The studied binary PSCs showed PCE (power conversion efficiency) values of 8.22% (PBDB-T:MPU2) and 9.77% (PBDB-T:MPU3). The VOC measured using the MPU3-based acceptor was higher than that obtained using MPU2 – this difference is attributed to a higher LUMO energy level of MPU3. MPU2 and MPU3 present complementary absorptions in the wavelength range where PBDB-T exhibits a poor absorption, thus the combination of these materials offers great potential for the fabrication of ternary PSCs. The solar cell with an optimized ternary layer PBDB-T:MPU2:MPU3 (1:1:1) showed an PCE value of 10.78%, higher than those obtained for the binary devices due to the enhanced of JSC and FF values. And, since the emission of MPU3 partially overlaps with the absorption of MPU2, the transfer of energy from MPU3 to MPU2 can improve the exciton utilization efficiency and achieve enhanced overall power conversion efficiency in this ternary solar cell.

In the modern world, people are too techno-friendly and dependent on technology using the Internet for every work. The same goes for the news. People are shifting from traditional mass media to ...digital news platforms and getting news through websites, news portals, social media, etc. If you are dependent on the Internet for every kind of information, then you will face false information on the Internet. False or fake news is defined as any information that does not have any credible and reliable source behind it or any misleading information that is likely to mislead the public. The aim behind fake news transmission is to damage a person's or entity's reputation or advertising revenue. If you want not to fall into the fake news you should know about fake news detection and media literacy. The main purpose of the study is to check the exposure of fake news awareness and fake news detection methods among social media users. In the current scenario, this is much necessary to know that social media users have the advisable knowledge of fake news detection and media literacy because people easily fall into the rumors. Mob lynching is one of the biggest rumors on the Indian Internet. In this research, the survey method and questionnaire for data collection were used. The questionnaire was distributed randomly over different social media platforms and emails to the intended respondents. The findings obtained reveal that most fake or false news in India is transmitted through WhatsApp, but social media users have adequate knowledge of fake news and media literacy.
