A recent case of angiomyolipoma (AML) with a prominent Component of polygonal epithelioid cells is described. A 27‐yearoid Japanese male with tuberous sclerosis presented with massive abdominal ...tumors increasing progressively in size. The patient died of respiratory disturbance and the autopsy revealed massive tumors in the bilateral kidneys, liver and lymph nodes, subependymal giant cell glioma of the brain and lymphangiomyomatosis of the lungs. The giant tumors were an unusual type of AML with a component of polygonal epithelioid cells, which showed a hepatocel‐lular carcinoma‐like pattern in some areas. Smooth muscle components comprising spindle cells, short or plump spindle cells and polygonal epithelioid cells frequently exhibited positive staining for HMB‐45 but negative staining for epithelial cell markers. The unusual AML presented in this case was thought to be of low‐grade malignancy and slow growing. it has been suggested that angiomyollpomas with diffuse areas of epithelioid cell component are potentially malignant. Immunostalnings positive for HMB‐45 but negative for epithelial cell markers are considered to be useful in differentiating AML with polygonal epithelioid cell component from other tumors, especially from renal cell carcinoma and hepatocellular carcinoma.

An 88-year-old woman, with a history of resection of stage IIA lung cancer in 1998, was referred to our hospital in August 2010 complaining of upper abdominal pain, vomiting, and dark brown stools. ...After endoscopic examination, she was admitted with a diagnosis of Mallory-Weiss syndrome. Vomiting occurred when food intake was resumed after fasting. Intestinal obstruction was suspected on abdominal radiography, and complete small bowel obstruction was confirmed by contrast-enhanced imaging after placement of an ileus tube. A small intestinal tumor with intussusception was detected by computed tomography. At laparotomy, there was no ascites. Intussusception was found due to an ileal tumor located approximately 50 cm from the ileocecal valve, and we performed partial small bowel resection. The resected small intestine contained a submucosal tumor approximately 40 mm in diameter that had penetrated the bowel wall to reach the serosa. Pathological examination revealed a submucosal tumor that showed poor continuity with the surrounding mucosa, while the histology was squamous cell carcinoma. Immunohistochemistry showed that the tumor was CK7 positive, CK20 negative, TTF-1 negative, and CK10 positive. Based on these findings, we made a diagnosis of small intestinal metastasis at 12 years after radical resection of squamous cell carcinoma of the lung.
Four groups (each consisting of five male Wistar rats) were treated daily by gavage for 30 days with water (control), or an aqueous solution of Ponceau 3R, Amaranth of Sunset Yellow FCF (300mg/kg of ...body weight/day). The effects of the treatment on growth, organ weight, liver components and RNA synthesis in the liver were studied. In the Ponceau 3R treated group, liver weight, protein content, the incorporation of radioactive UTP into nuclear RNA and nuclear RNA content in the liver were significantly increased. In the Amaranth-treated group, radioactive UTP incorporated into nuclear RNA in the liver was significantly increased, and the liver weight, protein and nuclear RNA contents in the liver also showed some increase (but not statistically significant). The treatment with Sunset Yellow FCF had no effect on any item tested. These results suggest that the stimulation of RNA synthesis after the administration of Ponceau 3R and Amaranth might be related to the changes in the liver.
The patient was a female in her 70s without previous laparotomy who visited our hospital for right lower abdominal pain. Marked small intestinal gas was noted on plain abdominal X-ray radiography. ...The patient was diagnosed with ileus and admitted. On contrast imaging through an ileus tube inserted for decompression, the small intestine was obstructed in the right lower abdominal region, and emergency laparotomy was performed. A hernial orifice was present on the lateral side of the cecum, and the small intestine was partially incarcerated, based on which a pericecal hernia was diagnosed. Since no circulatory disorder was noted in the incarcerated intestine, only reduction was performed without enterectomy. The hernial orifice was left open, considering that there was no possibility of re-incarceration. The postoperative course was favorable, and the patient was discharged on the 7th hospital day. Since this was a rare pericecal hernia case of internal hernia, we searched for and reviewed cases reported in Japan. This was a very rare case with a hernial orifice located on the lateral side of the cecum, not included in the current classification of pericecal hernia.
The reproductive behavior, embryonic and morphological development of larvae and juveniles of the damselfish, Amblyglyphidodon leucogaster, are described with laboratory reared specimens. Four adult ...fish (one male and three females) were reared in a 430 l tank (120 x 60 cm transparent acrylic aquarium with water 60 cm deep) at the laboratory of the Marine Science Museum, Tokai University. Two unglazed earthenware pipe (180 mm in length, 150 mm in diameter and 200 mm in length, 110 mm in diameter) were set on the bottom of the tank as a combined egg laying nest and hiding place. The water temperature was maintained at 26 6 27C, which approximates the natural habitat using a thermostat and heater. The observation period was about nine months, from 1 May 2001 to 15 February 2002. Spawning was observed 39 times from 24 May 2001 to 13 February 2002. Spawning occurred from the middle of the night to early morning (04:34 6 05:28 AM) under natural light conditions. The fertilized eggs were demersal cocoon-like in shape, measuring 1.31-1.38 mm in long axis and 0.53-0.55 mm in short axis with one large (0.13-0.15 mm) and a number of small (0.02 6 0.08 mm) oil globules. Hatching took place 5 to 7 days (mainly 6 days) after fertilization at water temperatures of 23.2-27.9C (mean 26.7C). The newly hatched larvae measured 3.13-3.96 mm in total length and possessed 6-7+19 6 20=26 6 27 myomeres and an oil globule in an oval-shaped yolk sac; the anus and the mouth were open. The eyes were completely black. Nineteen days after hatching, they measured 7.73-9.80 mm in total length, attaining the juvenile stage. The larvae were successively fed rotifers and Artemia nauplii and were raised for 51 days after hatching. However, they died due to a heater accident.
症例は67歳,男性.頸部リンパ節腫脹を主訴.穿刺細胞診で悪性リンパ腫が疑われたが自然消退した.腹部CTで腹腔内リンパ節の腫脹(-).胆嚢底部~体部肝側に乳頭状の隆起を認め,胆嚢癌と診断された.2005年2月開腹所見で胆嚢に50×40mm乳頭膨張型の腫瘍,Treitz靱帯から約20cmの空腸に25×20mmの1型腫瘍を認めた.拡大胆嚢摘出+胆管切除+リンパ節郭清+空腸部分切除+胆管空腸吻合術を施行した.病理所見で胆嚢粘膜固有層,空腸粘膜下~漿膜下に中型で均一な異型リンパ球がび漫性に増殖浸潤し,CD10(+),CD20(+),CD79a(+)とB細胞マーカーが陽性,MIBI>90%と核分裂像多数で両者とも悪性リンパ腫(Diffuse large B-cell lymphoma)と診断された.術後リツキシマブ+CHOP療法を8クール施行し,術後4年10カ月無再発生存中である.胆嚢原発の悪性リンパ腫は極めてまれである.MALTリンパ腫もあるが総じて高悪性度であり予後不良であり,病型分類により術後化学療法が必要である.
In this paper four cases of oral malignant tumor occurred in children were reported, at the Department of Dentistry and Oral Surgery, Keio University Hospital from 1984 to 2004. They were malignant ...lymphoma of the lower lip, squamous cell carcinoma of the lower gum, neuroblastoma of the mandible, and osteosarcoma of the mandible. Case 1 A 12 year old-girl came to our hospital with a chief complaint of a painless tumor of lower lip. The tumor was surgically removed under general anesthesia, and a pathological diagnosis of malignant lymphoma was made. After the operation, radiation therapy (40Gy) was given. There is no evidence of recurrence about 17 years and 1 month after the operation. Case 2 A 12 year old-boy was referred to our hospital because he was pointed out to have an ulcer of lower gum at the dental check-up of his school. The pathological diagnosis by the biopsy was squamous cell carcinoma. Case 3 The patient was a 14 year old-boy with a chief complaint of gingival swelling in the left molar region of the mandible. The pathological diagnosis of the mandibular lesion by biopsy was neuroblastoma, and the primary tumor was found in the left adrenal gland. Chemotherapy, surgical resection and radiation were given. Twelve years and 4 months after the mandiblectomy, the patient is alive with no evidence of local recurrence or metastasis. Case 4 A 9 year old-girl complained of a painless swelling in the chin. Pre-and post-operative chemotherapy (MTX+VCR) was administered, mandiblectomy combined with neck dissection was performed. However 10 months later, there was a metastasis of the lung. She died 2 years and 4 months after the first medical examination.
We here report a rare case of esophageal intramural pseudodiverticulosis with dysphagia. A 65-year-old man was suffering from intermittent dysphagia for 10 years, which had been worsening in recent ...months. Endoscopic examination revealed multiple, small, saccular diverticula and mild annular stricture with numerous white plaques from the cervical esophagus to the middle thoracic esophagus. Barium esophagography revealed stricture of the upper thoracic esophagus with multiple tiny flask-shaped outpouchings. Based on this characteristic appearance, we diagnosed esophageal intramural pseudodiverticulosis and inflammation of a narrow segment of the esophagus. Biopsy specimen showed acute and chronic esophagitis with Candida infection. Dysphagia persisted despite resolution of candidiasis, for which we successfully performed endoscopic balloon dilatation of the stricture. After the treatment, the patient has been well and has not been suffering from dysphagia.
