Our purpose is to introduce transforaminal en bloc resection for the treatment of thoracic ossification of ligamentum flavum, and report outcomes of the procedure.
The record of patients with ...symptomatic thoracic ossification of the ligamentum flavum who received transforaminal en block resection from October 2010 to September 2014 were retrospectively reviewed. Outcomes were Japanese Orthopaedic Association (JOA) lower extremity motor dysfunction and sphincter dysfunction scores.
Seventeen patients (10 males, 7 females) with a median age of 60 years were included in the analysis. Surgery was performed as planned without complications in all patients. One segment decompression was performed in 1 case, 2 segment in 9 cases, 3 segment in 3 cases, and 4 segment in 4 cases. The median lower extremity motor dysfunction score was significantly higher at 2 weeks, 3 months, and 6 months postoperatively, and the end of follow-up, than before surgery (median score: 2, 2, 3, 3, respective, vs. 1, all, p ≤ 0.008). There were no significant differences in JOA sphincter dysfunction score between the different time points.
Transforaminal en bloc resection is a safe and effective method for the treatment of thoracic ossification of the ligamentum flavum.
•A transforaminal procedure was designed to remove the ossified ligamentum flavum.•The procedure allows thoracic ossification to be removed via a posterior approach.•The procedure achieves en bloc resection under direct vision.•The non-space-occupying procedure decreases manipulation of spinal cord and injury potential.

Smad4, a key mediator of the transforming growth factor‐β signaling, is mutated or deleted in 20% of pancreatic ductal adenocarcinoma (PDAC) cancers and significantly affects cancer development. ...However, the effect of Smad4 loss on the immunogenicity and tumor immune microenvironment of PDAC is still unclear. Here, a surprising function of Smad4 in suppressing mouse PDAC tumor immunogenicity is identified. Although Smad4 deletion in tumor cells enhances proliferation in vitro, the in vivo growth of Smad4‐deficient PDAC tumor is significantly inhibited on immunocompetent C57BL/6 (B6) mice, but not on immunodeficient mice or CD8+ cell‐depleted B6 mice. Mechanistically, Smad4 deficiency significantly increases tumor cell immunogenicity by promoting spontaneous DNA damage and stimulating STING‐mediated type I interferon signaling,which contributes to the activation of type 1 conventional dendritic cells (cDC1) and subsequent CD8+ T cells for tumor control. Furthermore, retarded tumor growth of Smad4‐deficient PDAC cells on B6 mice is largely reversed when Sting is codeleted, or when the cells are implanted into interferon‐alpha receptor‐deficientmice or cDC1‐deficientmice. Accordingly, Smad4 deficiency promotes PDAC immunogenicity by inducing tumor‐intrinsic DNA damage‐elicited type I interferon signaling.
The author's findings establish that the commonly inactivated tumor suppressor Smad4 is a critical regulator of pancreatic ductal adenocarcinoma tumor immunogenicity by activating cancer‐autonomous DNA sensing signaling, and suggest a potential role of Smad4 expression as a biomarker in developing future effective immunotherapy for PDACs.

Uroplakin 1A (UPK1A) has recently been found dysregulation in many cancers. However, the functions of UPK1A and its underlying mechanisms in hepatocellular carcinoma (HCC) remain poorly understand. ...In the present study, we found that UPK1A was highly expressed in HCC tumor tissues compared with adjacent non-tumor tissues. Datasets from the Cancer Genome Atlas project (TCGA) and Gene expression Omnibus confirmed that UPK1A was highly expressed in HCC. High expression of UPK1A predicted poor overall survival (OS) in patients with HCC. Univariate and multivariate analysis showed that UPK1A was a significant and independent prognostic predictor for OS of patients with HCC. Functionally, silencing UPK1A suppressed HCC cell glycolysis and proliferation. Mechanistically, hypoxia-inducible factor 1α (HIF-1α) directly bound to the hypoxia response elements (HRE) of UPK1A promoter region, which led to the up-regulation of UPK1A under hypoxia. Furthermore, downregulation of UPK1A reduced key enzyme of glycolysis via regulating HIF-1α. Taken together, these data indicates the existence of a positive feedback loop between HIF-1α and UPK1A that modulates glycolysis and proliferation under hypoxia in HCC cells.
To study the problem of highway traffic safety in cold regions, a thorough statistical analysis using traffic accident historical data is used to evaluate the influencing factors on highway traffic ...safety. A road traffic safety indicator system for cold regions is developed to evaluate the weight of these influence factors, such as driver performance, vehicle capacity, road condition, and general impact of the traffic environment. An index system of highway traffic safety influencing factors for cold regions is developed, and an evaluation model for traffic road safety influencing factors in cold regions based on attribute recognition theory is proposed. The weight of the model is determined using an analytic hierarchy process combined with expert scoring. The evaluation model is based on single-index attribute measure values and multi-index comprehensive attribute measurement values, using the confidence criterion to identify the influence of road traffic safety factors on road traffic safety in cold regions. In addition, the fuzzy comprehensive evaluation method is used for comparative evaluation, and the evaluation results were consistent and verified the correctness and feasibility of the attribute recognition model. Results showed that the evaluation system was in good agreement with actual traffic conditions in cold regions.
The study established a nomogram based on quantitative parameters of spectral computed tomography (CT) and clinical characteristics, aiming to evaluate its predictive value for preoperative ...lymphovascular invasion (LVI) in gastric cancer (GC).
From December 2019 to December 2021, 171 patients with pathologically confirmed GC were retrospectively collected with corresponding clinical data and spectral CT quantitative data. Patients were divided into LVI-positive and LVI-negative groups based on their pathological results. The univariate and multivariate logistic regression analyses were used to identify the risk factors and construct a nomogram. The calibration curve and receiver operating characteristic (ROC) curve were adopted to evaluate the predictive accuracy of nomogram.
Four clinical characteristics or spectral CT quantitative parameters, including Borrmann classification (P = 0.039), CA724 (P = 0.007), tumor thickness (P = 0.031), and iodine concentration in the venous phase (VIC) (P = 0.004) were identified as independent factors for LVI in GC patients. The nomogram was established based on the four factors, which had a potent predictive accuracy in the training, internal validation and external validation cohorts, with the area under the ROC curve (AUC) of 0.864 (95% CI, 0.798–0.930), 0.964 (95% CI, 0.903–1.000) and 0.877 (95% CI, 0.759–0.996), respectively.
This study constructed a comprehensive nomogram consisting spectral CT quantitative parameters and clinical characteristics of GC, which exhibited a robust efficiency in predicting LVI in GC patients.

Study Design
Retrospective study.
Objective
To analyze the causes of and factors influencing early recurrence after TELD performed in the treatment of lumbar disc herniation.
Methods
We included 285 ...patients with single-segment lumbar disc herniation treated using transforaminal endoscopy lumbar discectomy TELD from January 2017 to December 2019 at the First Affiliated Hospital of the University of South China. Patients were classified into early recurrence and non-early recurrence groups based on clinical symptoms and MRI reexamination. Imaging data (eg, disc height index (DHI), Pfirrman grades, base width, postoperative annulus-fibrosus tear size, cross-sectional area of the foramen (CSAF), etc.)were analyzed, and multivariate, binomial logistic regression was utilized to determine which factors were associated with early recurrence after TELD.
Results
A total of 285 patients completed surgery and clinical follow-up, during which 19 patients relapsed within 6 months postsurgery, for an early recurrence rate of 6.7%. There were statistically significant differences between DHI, Pfirrman grades, base width, postoperative annulus-fibrosus tear size, herniation sites, CSAF and Modic changes between the early recurrence and non-early recurrence groups (P < .05). On multivariate logistic regression,the degree of disc degeneration (OR = .747, P = .037), CSAF (OR = 5.255, P = .006), degree of Modic change (OR = 1.831, P = .018) and base width of the herniation (OR = 4.942, P = .003) were significantly correlated with early recurrence after TELD.
Conclusions
Postoperative annulus-fibrosus tear size, DHI, and location of the disc herniation were associated with early recurrence after TELD. Increased base width of the herniation, severe disc degeneration, decreased CSAF and Modic change were risk factors for early recurrence after TELD.
Chinese sturgeon (Acipenser sinensis), one of the oldest extant actinopterygian fishes with very high evolutionary, economical and conservation interest, is considered to be one of the critically ...endangered aquatic animals in China. Up to date, the immune system of this species remains largely undetermined with little sequence information publicly available. Herein, the first comprehensive transcriptome of immune tissues for Chinese sturgeon was characterized using Illumina deep sequencing. Over 67 million high-quality reads were generated and de novo assembled into the final set of 91,739 unique sequences. The annotation pipeline revealed that 25,871 unigenes were successfully annotated in the public databases, of which only 2002 had significant match to the existing sequences for the genus Acipenser. Overall 22,827 unigenes were categorized into 52 GO terms, 12,742 were classified into 26 KOG categories, and 4968 were assigned to 339 KEGG pathways. A more detailed annotation search showed the presence of a notable representation of immune-related genes, which suggests that this non-teleost actinopterygian fish harbors the same intermediates as in the well known immune pathways from mammals and teleosts, such as pattern recognition receptor (PRR) signaling pathway, JAK-STAT signaling pathway, complement and coagulation pathway, T-cell receptor (TCR) and B-cell receptor (BCR) signaling pathways. Additional genetic marker discovery led to the retrieval of 20,056 simple sequence repeats (SSRs) and 327,140 single nucleotide polymorphisms (SNPs). This immune-enriched transcriptome of Chinese sturgeon represents a rich resource that adds to the currently nascent field of chondrostean fish immunogenetics and furthers the conservation and management of this valuable fish.

Trichiurus lepturus (hairtail) is an important economic component of China’s marine fishing industry. However, due to the difficulty in identifying the appearance of hairtail from different ...geographical distributions, hairtails with geographical indication trademarks were imitated by general varieties. In this study, the texture characteristics, color, basic nutrients, amino acids, mineral, fatty acids, and volatile flavor substances were used as indicators for multivariate statistical analysis to determine whether three origins of hairtails from the habitats of Zhoushan (East China Sea, T.Z), Hainan (South China Sea, T.N), and Qingdao (Yellow Sea, T.Q) in the market could be distinguished.
