A select few top-tier OMBA (Online Master of Science in Business Administration) programs have disrupted the conventional MBA market, now offering comparable—if not superior—value to students ...compared to many in-person, full-time MBA experiences. Utilizing data collected from interviews with 36 admission staff and administrators representing 24 OMBA programs, as well as a large-scale study of 135 students and alumni of one top-tier OMBA program, we apply Kim and Mauborgne’s Eliminate-Reduce-Raise-Create framework to highlight the emergence of a new value curve for the very best OMBA programs. The article explores the importance, relevance, value, and mechanisms for how some OMBA programs have successfully repositioned. These high-quality, high-impact OMBA programs have sought to reconcile traditional tradeoffs between OMBA and full-time MBA programs and may pave the way for a transformation of the OMBA marketplace from a focus solely on convenience to programs designed to provide the highest value to students.

The diversity and abundance of wild insect pollinators have declined in many agricultural landscapes. Whether such declines reduce crop yields, or are mitigated by managed pollinators such as honey ...bees, is unclear. We found universally positive associations of fruit set with flower visitation by wild insects in 41 crop systems worldwide. In contrast, fruit set increased significantly with flower visitation by honey bees in only 14% of the systems surveyed. Overall, wild insects pollinated crops more effectively; an increase in wild insect visitation enhanced fruit set by twice as much as an equivalent increase in honey bee visitation. Visitation by wild insects and honey bees promoted fruit set independently, so pollination by managed honey bees supplemented, rather than substituted for, pollination by wild insects. Our results suggest that new practices for integrated management of both honey bees and diverse wild insect assemblages will enhance global crop yields.

We examine how thermal annealing affects the fullerene network in conjugated polymer bulk heterojunction (BHJ) solar cells. We begin by creating electron-only devices with a BHJ geometry by blending ...the fullerene derivative 6,6-phenyl-C61-butyric-acid-methyl-ester (PCBM) with polystyrene (PS). These electron-only PS:PCBM diodes function even with a poly(ethylenedioxithiophene):poly(styrenesulfonate) (PEDOT:PSS) layer, indicating that PEDOT:PSS films do not serve as electron blocking layers. Atomic force microscopy shows that the degree of phase segregation in the PS:PCBM blend films is similar to that in the active layer of blends of PCBM with poly(3-hexylthiophene-2,5-diyl) (P3HT), so that the PS:PCBM blends provide a good model for the fullerene part of the BHJ network in P3HT:PCBM solar cells. We find that thermal annealing dramatically decreases the electron current that flows in the PS:PCBM diodes, suggesting that annealing leads to increased phase segregation that lowers the electron mobility on fullerenes in the BHJ geometry. We also find that annealing increases the photoluminesence of P3HT:PCBM blend films, indicating that thermal treatment produces increased phase segregation that leads to decreased exciton harvesting. The fact that annealing decreases both exciton harvesting and electron mobility implies that there is significant room to further improve polymer/fullerene photovoltaics by controlling the amount of phase segregation.

The fullerene adducts 1a and 1b, whose molecular shapes either promote or hinder the formation of 1-D stacks, have been examined for their potential to form 1-D wire-like domains in ...bulk-heterojunction organic solar cells. The photovoltaic efficiency of solar cells based on blends of the stacking fullerene 1a with regioregular poly(3-hexylthiophene-2,5-diyl) (P3HT) is greatly enhanced compared to nonstacking model fullerene 1b.

A multinational outbreak of nosocomial fusarium meningitis occurred among immunocompetent patients who had undergone surgery with epidural anesthesia in Mexico. The pathogen involved had a high ...predilection for the brain stem and vertebrobasilar arterial system and was associated with high mortality from vessel injury. Effective treatment options remain limited; in vitro susceptibility testing of the organism suggested that it is resistant to all currently approved antifungal medications in the United States. To highlight the severe complications associated with fusarium infection acquired in this manner, we report data, clinical courses, and outcomes from 13 patients in the outbreak who presented with symptoms after a median delay of 39 days.

meningitis occurred in early 2023 in association with epidural anesthesia in Mexico. Descriptions of 13 cases and associated management are presented.
The purpose of this case report is to document the occurrence of granulomatous aortitis complicated by formation of a saccular aneurysm and aortobronchial fistula due to Brucella infection.
A ...65-year-old man with a history of feral swine hunting presented with hemoptysis and was found to have a saccular thoracic aortic aneurysm and associated aortobronchial fistula. The aneurysm underwent operative repair with closure of the aortobronchial fistula.
Histopathological examination of the aneurysm wall revealed evidence of granulomatous aortitis. Cultures of the blood and aortic wall tissue were positive for Brucella suis.
Although rare, Brucella infection should be considered in the differential diagnosis of aortic aneurysm with granulomatous aortitis.
•Granulomatous aortitis was complicated by formation of a saccular aneurysm and aortobronchial fistula due to Brucella infection.•Differential diagnosis of granulomatous aortitis should include Brucella infection.•History of feral swine exposure was important for differential diagnosis.•Diagnosis of Brucella suis infection was made with tissue and blood cultures.

During 2007 to 2010, 23 carbapenem-resistant Enterobacteriaceae strains were recovered in two hospitals in Texas among three institutions surveyed. Eighteen (2.0% overall) strains carried bla(KPC-2) ...and were collected in 2008 (3/150; 2.0%) and 2010 (15/289; 5.1%). Clonality was noted within hospitals and ten isolates belonged to ST258. All strains carried bla(KPC-2) on plasmids and 14 had this gene embedded in Tn4401a. Tn3 transposase was detected upstream of bla(KPC-2) in two isolates and another strain had two adjacent copies of bla(KPC). Surveillance in prior years shows sporadic detection of KPC-producers in Texas, however, a recent increase in carbapenem-resistant Enterobacteriaceae in this state was due to the spread of KPC-producing Klebsiella pneumoniae, including isolates from ST258.
. Objective: To develop a clinical prediction rule to identify patients who can be safely discharged one hour after the administration of naloxone for presumed opioid overdose. Methods: Patients who ...received naloxone for known or presumed opioid overdose were formally evaluated one hour later for multiple potential predictor variables. Patients were classified into two groups: those with adverse events within 24 hours and those without. Using classification and regression tree methodology, a decision rule was developed to predict safe discharge. Results: Clinical findings from 573 patients allowed us to develop a clinical prediction rule with a sensitivity of 99% (95% CI = 96% to 100%) and a specificity of 40% (95% CI = 36% to 45%). Patients with presumed opioid overdose can be safely discharged one hour after naloxone administration if they: 1) can mobilize as usual; 2) have oxygen saturation on room air of >92%; 3) have a respiratory rate >10 breaths/min and <20 breaths/min; 4) have a temperature of >35.0°C and <37.5°C; 5) have a heart rate >50 beats/min and <100 beats/min; and 6) have a Glasgow Coma Scale score of 15. Conclusions: This prediction rule for safe early discharge of patients with presumed opioid overdose performs well in this derivation set but requires validation followed by confirmation of safe implementation.
